Penyakit TB paru masih merupakan masalah kesehatan di negara-negara berkembang. Menurut World Health Organization (WHO) tahun 2014, benua Asia menyumbang 56% jumlah penderita TB paru di dunia pada tahun 2013, Afrika 29%, regional mediterania timur 8%, Eropa 4%, dan yang paling kecil beban penderita TB adalah wilayah Amerika 3% dari total jumlah penderita TB paru di dunia. Indonesia menempati posisi keempat tertinggi TB paru di dunia tahun 2013. Jumlah penderita TB paru yang dirawat inap di RSUD Dr. Hadrianus Sinaga Pangururan Kabupaten Samosir Tahun 2014 adalah 131 orang

Untuk mengetahui karakteristik penderita TB paru, dilakukan penelitian di RSUD Dr. Hadrianus Sinaga Pangururan Kabupaten Samosir dengan desain case-series. Populasi dan sampel penelitian berjumlah 131 orang pada tahun 2014 yang tercatat di rekam medik rumah sakit. Data univariat dianalisis secara deskriptif dan data bivariat dianalisis dengan menggunakan uji Chi-square, t-test, Kruskal Wallis, Mann-Whitney.

Proporsi berdasarkan sosiodemografi tertinggi pada kelompok umur 57-65 tahun (29%), laki-laki (75,6%), Suku Batak (100%), pendidikan SD (38,9%), petani (75,6%), di Kabupaten Samosir (96,9%). Proporsi terbesar berdasarkan keluhan utama adalah batuk darah 62,6%, status BTA positif 55,7%, kategori I 74%, lama rawatan rata-rata 5 hari, keadaan sewaktu pulang atas izin dokter 71,0%, sumber biaya BPJS 59,5% Ada perbedaan proporsi yang bermakna antara jenis kelamin berdasarkan keluhan utama (p=0,035), status BTA berdasarkan keluhan utama (p<0,001), lama rawatan berdasarkan sumber biaya (<0,001). Tidak ada perbedaan proporsi yang bermakna antara umur berdasarkan keluhan utama (p=0,736), umur berdasarkan lama rawatan (0,186), lama rawatan berdasarkan keadaan sewaktu pulang(0,095).

Penulis menyarankan tentang pencegahan penyakit TB paru perlu ditingkatkan dan bukan hanya difokuskan pada pengobatan dengan melibatkan instansi terkait. Pengobatan penderita TB paru sampai tuntas sangat diperlukan agar penderita sembuh dan tidak kambuh lagi. Pelayanan kesehatan di RSUD Dr. Hadrianus Sinaga perlu ditingkatkan khususnya dalam menanggulangi penderita TB paru yang mempunyai penyakit komplikasi yang cukup parah. Kepada petugas RSUD. Dr Hadrianus Sinaga paru agar melengkapi data-data pada kartu status pasien seperti riwayat TB paru pada keluarga.

ABSTRACT

Pulmonary tuberculosis is still health problem in developing countries. According to World Health Organization (WHO) in 2014, Asia accounted for 56% of the number of patients with pulmonary tuberculosis in the world in 2013, Africa 29%, eastern Mediterranean region 8%, Europe 4% and the smallest burden of pulmonary tuberculosis isin the American region 3% of the total number of patients with pulmonary tuberculosis in the world. Indonesia occupies the fourth position in the world's highest pulmonary tuberculosis in 2013. The number of patients with pulmonary tuberculosis who inpatient in RSUD. Dr. Hadrianus Sinaga Pangururan Kabupaten Samosir are 131 people.

To know the characteristics of pulmonary tuberculosis, conducted a research in RSUD Dr. Hadrianus Sinaga Pangururan Kabupaten Samosir with case-series design. Population and sample were 131 people who were recorded in hospital medical record in 2014. Univariate data were analyzed by descriptively and bivariate data were analyzed by using Chi square, t-test, Kruskal-Wallis, Mann-Whitney.

Based on the sosiodemograph, the highest proportion is in the age group namely 57-65 years (29%), males (75,6%), Bataknese (100%), primary school / equivalent (38,9%), farmers (75,6%), in regency of Samosir (96,9%). The highest proportion based on main gripe is hemoptysis 62,6%, positive BTA status 55,7%, category I 74%, the average length of stay 5 days, expense source of BPJS 59,5%, outpatient by doctor permission 71,0. There was significant difference between the proportion of sex based on main gripe (p= 0,035), BTA status based on main gripe(< 0,001), length of treatment based on expense source(< 0,001). There was no significant difference between the proportion of age based on main gripe (0,736) , age based on length of treatment (0,186), length of treatment based on out of patient (0,095).

The authors suggest for prevention of pulmonary TB disease should be enhanced and not only focused intreatments with involving related institutions. Patients with pulmonary TB treatment until completed very necessary for patients cured and not relapse again. Health services in RSUD Dr. Hadrianus Sinaga needs to be improved , especially in dealing with pulmonary tuberculosis patients who have severe disease complications. The authors also suggest for the hospital employees of RSUD. Dr Hadrianus Sinaga to complete data on patient status card such as family history of pulmonary TB.
