Uji Diagnostik Pemeriksaan Dermoskopi Dalam Mendiagnosis Skabies Sufina F. Nasution

ABSTRAK
Latar belakang : Skabies adalah suatu penyakit yang sangat menular yang disebabkan oleh tungau Sarcoptes scabiei. Pemeriksaan kerokan kulit untuk mendiagnosis skabies dapat menimbulkan ketidaknyamanan dan rasa takut. Pemeriksaan dermoskopi merupakan pemeriksaan yang tidak invasiv, mudah dan cepat dan meminimalisir risiko infeksi dari agen – agen yang dapat ditransmisikan dari darah.
Tujuan : Untuk mengetahui nilai diagnostik dermoskopi dalam mendiagnosis skabies.
Metode : Penelitian uji diagnostik menggunakan dermoskopi untuk mendiagnosis skabies dengan menggunakan baku emas hasil pemeriksaan kerokan kulit. Analisis statistik uji diagnostik untuk menentukan sensitivitas, spesifisitas, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio dan akurasi.
Subjek : Tiga puluh empat orang yang diduga menderita skabies berdasarkan anamnesis dan pemeriksaan klinis.
Hasil : Pemeriksaan dermoskopi dalam mendiagnosis skabies dibandingkan dengan baku emas kerokan kulit memiliki sensitivitas 66,67%, spesifisitas 61,29%, positive predictive value 14,29%, negative predictive value 95%, positive likelihood ratio 1,72, negative likelihood ratio 0,54 dan akurasi 61,76%.
Kesimpulan : Nilai diagnostik pemeriksaan dermoskopi dalam mendiagnosis skabies dengan baku emas kerokan kulit adalah sedang, sehingga perlu dicari metode diagnostik non invasiv lainnya yang lebih akurat.

Results: The sensitivity of the dermoscopic examination in diagnosing scabies compared with skin scraping was 66,67%, the specificity was 61,29%, the positive predictive value was 14,29% and the negative predictive value was 95%, the positive likelihood ratio was 1,72, the negative likelihood ratio was 0,54 and the accuracy was 61,76%.
Conclusion: The diagnostic value of dermoscopic examination in diagnosing scabies using skin scraping as the gold standard was moderate, so it is necessary to find other more accurate non-invasive diagnostic method.

ABSTRACT
Background: Scabies is a highly transmitted disease caused by Sarcoptes scabiei mite. Skin scraping examination in diagnosing scabies may cause discomfort and fear. Dermoscopic examination is not invasive, easy and quick to perform, and minimizes the risk of accidental infections from blood-transmissible agents.
Objective: To investigate the diagnostic value of dermoscopic examination in diagnosing scabies.
Method: The diagnostic test was done using dermoscopy to diagnose scabies and skin scraping was used as the gold standard. Statistical analysis of the diagnostic test are to determine sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio and accuracy.
Subject : Thirty four people suspected of having scabies based on history and clinical examination.
