POTENSI INTERAKSI OBAT ANTIDIABETES PADA PASIEN RAWAT INAP DIABETES MELITUS TIPE 2 DI RSUD DR. PIRNGADI MEDAN

ABSTRAK
Penyakit diabetes melitus merupakan salah satu penyakit penyebab kematian dan prevalensinya terus bertambah setiap tahunnya. Penderita diabetes melitus tipe 2 mencapai 90-95% dari keseluruhan populasi penderita diabetes dan penyakit ini merupakan penyakit degeneratif yang memerlukan penanganan seksama. Umumnya penderita diabetes menerima terapi antidiabetes kombinasi dengan terapi lainnya atau polifarmasi sehingga berpotensi terjadi interaksi.
Penelitian ini bertujuan untuk mengetahui profil penggunaan, frekuensi, jenis obat antidiabetes, mekanisme, tingkat keparahan yang berpotensi untuk berinteraksi pada pasien diabetes melitus tipe 2 serta melihat hubungan usia dan jumlah obat dengan kejadian potensi interaksi obat antidiabetes.
Metode penelitian ini adalah deskriptif dilakukan dengan survey secara retrospektif. Penelitian dilakukan di RSUD Dr. Pirngadi Medan dengan menggunakan rekam medik pasien rawat inap diabetes melitus tipe 2 pada bulan Juli-Desember 2014 yang memenuhi kriteria inklusi. Analisa data dilakukan secara deskriptif dan menggunakan uji Chi Square pada program IBM SPSS 20.
Hasil penelitian menunjukkan dari 90 rekam medik ditemukan kejadian potensi interaksi obat sebesar 68,90%, obat antidiabetes yang berpotensi interaksi adalah insulin aspart (38,40%), metformin 30%, dan insulin detemir 20,80%, mekanisme potensi tertinggi adalah farmakodinamik (72%), potensi dengan tingkat keparahan terbanyak adalah moderate (82,40%), serta tidak ada hubungan antara usia dengan kejadian potensi interaksi (p=0,215) dan adanya hubungan yang signifikan antara jumlah obat dan kejadian potensi interaksi obat (p=0,000). Berdasarkan uraian di atas dapat disimpulkan bahwa terdapat potensi interaksi antidiabetes pada pasien rawat inap diabetes melitus tipe 2 di RSUD Dr. Pirngadi Medan Juli-Desember 2014.

POTENTIAL ANTIDIABETIC DRUG INTERACTION AMONG INPATIENTS WITH TYPE 2 DIABETES MELLITUS IN DR. PIRNGADI MEDAN HOSPITAL JULY-DECEMBER 2014
ABSTRACT
Diabetes mellitus is one of the leading causes of death, and its prevalence increases every year. Type 2 diabetes mellitus patients reach 90-95% of all diabetic population and the disease is a degenerative disease that requires careful handling. Generally, people with diabetes receive antidiabetic therapy in combination with other therapies or polypharmacy that potentially interact.
This study aims to determine the profile drug use, frequency, type of antidiabetic drugs, mechanisms, severity that potentially to interact in patients with type 2 diabetes mellitus and determine the correlation of age and amount of drugs with the incidence of potential antidiabetic drug interactions.
This research method is descriptive survey conducted by the retrospective approach. The study was conducted at Dr. Pirngadi Medan Hospital using medical records of inpatients with type 2 diabetes mellitus in July-December 2014 that fulfilled the inclusion criteria. The data were analyzed descriptively by using Chi Square test on IBM SPSS 20 program.
The results showed of 90 medical records found the incidence of potential drug interactions amounted 68.90%, antidiabetic drug interaction that could potentially interact was insulin aspart (38.40%), metformin (30%), and insulin detemir (20.80%), the highest potential mechanism interaction was pharmacodynamic (72%), with the highest potential severity was moderate (82.40%), there was no correlation between age and incidence of potential interactions (p = 0.215) and there was a significant relationship between the number of drugs and the incidence of potential drug interaction (p = 0.000). Based on the description above it can be concluded that the potential antidiabetic drug interaction in inpatients with type 2 diabetes mellitus in Dr. Pirngadi Medan hospital July-December 2014 exist.
