Evaluasi Penggunaan Obat Antihipertensi pada Pasien Yang Menjalani Hemodialisis di Klinik Spesialis Ginjal dan Hipertensi Rasyida Kota Medan
Abstrak
Hipertensi merupakan salah satu faktor pemicu terjadinya penyakit ginjal kronik. Dalam terapi pengobatan, obat antihipertensi dapat terakumulasi pada ginjal sehingga bisa memperburuk fungsi ginjal. Gagal ginjal kronik adalah gangguan fungsi ginjal yang bersifat progressif dan irreversibel. Pada saat ini hemodialisis (HD) merupakan terapi yang membantu kerja ginjal yang paling banyak digunakan. Hemodialisis membantu penderita mengekskresi zat sisa hasil metabolisme, garam, dan cairan yang berlebih agar tidak terakumulasi dalam sirkulasi tubuh. Penelitian ini bertujuan untuk mengetahui karakteristik pasien, golongan antihipertensi yang digunakan, regimen dosis, interaksi dengan obat lain, serta rasionalitas pengggunaan obat antihipertensi sehingga meningkatkan kualitas hidup pasien.
Penelitian deskriptif ini dilakukan di Klinik Spesialis Ginjal dan Hipertensi Rasyida Kota Medan pada periode Oktober 2015 – Desember 2015 dengan waktu penelitian dilaksanakan dari bulan Februari 2016 – April 2016. Data yang diambil sebanyak 100 rekam medik pasien yang diberi antihipertensi dijadikan sebagai sampel.
Hasil penelitian menujukkan bahwa karakteristik pasien yang menderita hipertensi adalah lebih banyak pada laki – laki yaitu sebesar 74%. Obat antihipertensi yang digunakan sudah rasional dan sesuai dengan acuan. Obat antihipertensi yang paling banyak digunakan adalah golongan penghambat adrenoreseptor beta yaitu bisoprolol sebesar 48%. Kombinasi antihipertensi yang paling sering diberikan adalah penghambat kanal kalsium dan penghambat adrenoreseptor beta (β-blocker) selektif yaitu amlodipin dengan bisoprolol sebesar 15%. Regimen dosis antihipertensi yang diberikan kepada pasien yang menjalani hemodialisis sudah sesuai dengan acuan serta tidak ditemukan adanya interaksi obat yang mengakibatkan interaksi dengan obat lain yang dapat menurunkan efektivitas antihipertensi.

Evaluation of Antihypertensive Drug Using on Hemodialysis Patients in Klinik Spesialis Ginjal dan Hipertensi Rasyida Medan
Abstract
Hypertension is one of chronic kidney triggers disease.In treatment therapy, antihypertensive drugs can accumulate in the kidneys that could worsen the kidney function.Chronic renal failure is renal function impairment that is progressive and irreversible. Hemodialysis (HD) is a renal replacement therapy which most widely used.Hemodialysis is helping patients to excrete waste materials, salt, and excess fluid that does not accumulate in the body's circulation.The objective of this research is to determine patient characteristics, type of antihypertensive using and antihypertensive drugs rationality that enhanced the quality of patient life.
This descriptive research has to determine the antihypertensive drug using, precision of dosage regimen, and interactions with other medications that might decreased the effectiveness of antihypertensive given to hemodyialysis patients in Klinik Spesialis Ginjal dan Hipertensi Rasyida Medan in October 2015 - December 2015 period, with time research from February 2016 - April 2016. Data were taken as many as 100 medical records of patients who were given antihypertensive used as a sample.
The results showed that the characteristic of patients who had been suffering by hypertension is more in male – is 74%. Antihypertensive drugs using has justifiable and accordance by the reference. Antihypertensive drugs that most widely used is beta-adrenoceptor inhibitor (β- blockers)group ex: bisoprolol with amount 48%. The combination of antihypertensive most often given are calcium channel blockers and beta-adrenoceptor inhibitor (β- blockers) is amlodipine with bisoprolol amounted to 15%. Antihypertensive dosage regimen that had given to hemodialysis patients are accordance by the reference and there is no drug interactions that lead the decreasing of antihypertensive effectiveness.
