Bapak/Ibu yang terhormat, izinkanlah saya Herniza Yusdani dari Program S-2 Ilmu

Kedokteran Gigi Fakultas Kedokteran Gigi Universitas Sumatera Utara untuk melakukan

penelitian Tesis tentang

“Analisis perilaku pemeliharaan kesehatan gigi dan mulut

terhadap status oral higiene dan periodontal pasien kompromis medis rawat inap di

RSUP H. Adam Malik Medan”

.

Tujuan penelitian ini untuk menganalisis hubungan status oral higiene dan periodontal

dengan perilaku pemeliharaan kesehatan gigi mulut pada pasien kompromis dan non

kompromis medis sindrom metabolik yang dirawat inap di rumah sakit.

Manfaat penelitian ini untuk menambah pengetahuan dan memberikan wawasan baru

mengenai kesehatan gigi mulut serta hubungannya dengan penyakit sistemik sehingga dapat

meningkatkan perilaku dalam melaksanakan pemeliharaan kesehatan gigi mulut.

Bapak/Ibu yang terhormat, penelitian ini dilakukan oleh saya sendiri dan dibantu oleh

beberapa teman saya dan saya meminta Bapak/Ibu untuk mengisi kuesioner yang berisi

nama, umur, jenis kelamin, pendidikan, lama perawatan di rumah sakit, riwayat penyakit

jantung dan DM, dan kebiasaan sehari-hari yang berkaitan dengan pemeliharaan kesehatan

gigi mulut. Selanjutnya saya akan melakukan pemeriksaan kondisi oral higiene dan

periodontal Bapak/Ibu dengan menggunakan kaca mulut, pewarna plak, probe periodontal,

dan sonde yang hanya membutuhkan waktu kira-kira 10 menit.

Identitas Bapak/Ibu akan disamarkan, hanya dosen pembimbing peneliti, anggota peneliti,

dan anggota komisi etik yang dapat melihat datanya. Apabila data ini dipublikasikan maka

kerahasiaan akan tetap dijaga. Partisipasi Bapak/Ibu dalam penelitian ini tidak dipungut biaya

serta tidak akan menimbulkan masalah atau komplikasi yang serius. Apabila selama

penelitian ada keluhan yang Bapak/Ibu alami, silahkan menghubungi saya.
