Introduction: Myeloid sarcoma or granulocytic sarcoma is a rare extramedullary manifestation of acute myeloid leukaemia. It is a tumour mass consisting of myeloblastic cells or immature myeloid cells and is associated with a poor prognosis. Diagnosing a myeloid sarcoma is challenging due to its non-specific clinical and radiological findings. Case presentation: We report a case of a 25-year-old gentleman who presented with two days history of right-sided abdominal pain mimicking an atypical acute appendicitis. Discussion: Subsequent imaging studies were proven equivocal in aiding our diagnosis. A diagnostic laparoscopy was then performed which demonstrated an inflamed hepatic flexure epiploic appendices over the sub-hepatic region forming an inflammatory mass which was then removed for biopsy.

The resulting histological examination revealed myeloid sarcoma. Conclusion: A multidisciplinary team discussion was held with the pathologist and the patient was then referred to the haematologist for further workup of leukaemia.
