English is an international language and it is used all over the world. In Indonesia, English is a foreign language. It has been taught at school as one of the main subjects. The most important thing in learning English is learning vocabulary. Without vocabulary we can not speak, write and understand what we are reading and listening. Vocabulary is the most important matter in learning language.
Based on the English curriculum, English teaching covers four skills, namely reading, listening, speaking and writing. The four skills above are supported by the learning of language elements, that is grammar and vocabulary. Vocabulary is also essential for succesful second language use because wthout an extensive vocabulary, one will not be able those the structures and function he or she may learn for comprehensive communication. According to Wilkins in Tornbury (2002: 13), without grammar, without vocabulary nothing can be conveyed.

The average percentage ofstudents who fall into the category lies the difficulty implement concepts needed is 48,04%. Factors that cause learning difficulties ofstudents are three factor , there is Internal factors, External, The student intelligence. The student cognitive capability in SMAN 1 Sidikalang and SMAS Trisakti Medan is low.
Based on the statements above, to solve problem needs a combination of previous knowledge, such as: the use of steps, rules, and concept. In connected with Mathematics is as one of the basic knowledge which gives great importance to the process rather than the result; it means that the answers should be given by someone in solving mathematical problems, he or she should pay attention to the accuracy of the use of steps, strategies, rules, and concepts. The importance of the use of steps and strategies in solving the problems shows that the answers are not easy to get, but it must pass through any kinds of procedural steps and it should relate those concept which have been got before.

The research instrument used is Mathematical Self-Esteem (MSE) scale. MSE scale is used to determine the judgement ofstudents' capability, successfulness, significance, and worthiness him/herself in mathematics. The SEM scale used in this study is a modified from self-esteem scale used by Alhadad (2010). This scale consists of 25 items that are prepared statement with four possible answers (responses), namely Very Often (VO), Often (O), Rare (R), and Very Rare (VR). The statements on a MSE scale consists of positive statements (favorable) and negative statements (unfavorable). With the two types of statements, positive and negative, is expected to encourage students to read each item given statement carefully and respond in earnest, so the data obtained from the SEM scale is more accurate.
The result of data analysis shows that (1) The implementation of CTL in teaching speaking are mentioned to be 5 sections. (1.a) The objectives of CTL in teaching speaking are to introduce English as the first foreign language to be learned, (1.b )the instructional materials in CTL in the classroom, the teacher use modeling with some pictures to teach the lesson of speaking skill. It is used to support the students understanding the lesson, (1.c) the roles of teacher in teaching-learning processes using CTL are as facilitator, controller, transferor and evaluator and the roles ofstudents are as the player and the partner of the study,(1.d)the learning assessments conducted are also items that are used for evaluating the student’s achievement, (1.e) the procedure of teaching is dividing three steps such as initial, central and final activities. (3.a) Some strengths of CTL in teaching speaking are teaching-learning process can be more interesting, give some meaning to the students, the condition will be more productive, the students have more bravery. (3.b) Some weaknesses of CTL in teaching speaking are teacher is more intensive and some students are not happy if doing discussion in the classroom.
As motorcycles need wheel, it is similar with, when talking about the human requirement of language. Language is produced by human being through mouth which is resulted from the combination between tongue and air, so that produce the sounds. A system is used to share what people mean. It is the expression of human communication through their knowledge and experience. Then, it becomes one of the aspects which make the world always change, because language is influenced by their way of thinking, belief and environment.
Reading is not only read the text without understanding the content of the text. The reader must know the information, message, or idea about the text. Each written text has clear purpose, that all of information, message or idea in one form of the texts (description, recount, narration, procedure, explanation, discussion, exposition, news item, report, anecdote, and review). It means that to consider the effectiveness of using language, each form of written texts is preferably created based on genre. Genre can be defined as a type of the text functioned as a frame of reference, so the text can be created effectively; in purpose, election, and arranging of text elements, and accuracy in using grammar.
English and Indonesian language are different. There is no guarantee that the students who have good understanding of Indonesian text will have good understanding of English text. When the students are reading Indonesian text they have mastered vocabulary and the structural system, but when they are reading English text, they are demanded to have adequate knowledge of the target laguage which have different system. Reading comprehension involves a veriaty of skills, which are a key to comprehend the text.
The aims of this research are to describe the students’ capability in answering multiple choice question of reading text, the problem faced by students in answering multiple choice question of reading text, and the sources of the problem faced by students in answering multiple choice question of reading text at third grade of MTs Darul Hijroh Canden, Rejosari, Polokarto. This research is a descriptive qualitative research. The writer collets the data by observing teaching learning process, conducting the interview, and documenting some important data, and also conducting a text. The data are taken from the result of the test conducted by the studentsof third grade MTs Darul Hjroh. The methods of collecting data are observation, interview, documentation and test. The techniques of analyzing data are identifying the result of the observation, identifying the information from the interview with the students, English teacher and staff of boarding school, identifying the result of the test, describing the percentage of each items to categorize and to find the levels ofstudents’ capability in answering multiple choice question of reading text by using Arikunto Standard in categorizing, and the last is describing the result of the students’ test and students’ problems in answering multiple choice question of reading text. The result of the research: 1) The students’ capability in answering multiple choice question of reading text can be divided into three category level. They are 14 (63,63%) students in poor level, 6 (27,27%) students in fair level, and 2 (9,10%) students in good level. So, it can conclude that the capabilityof the students to answer the multiple choice question of reading text is low. 2) The most problem faced by the students in answering multiple choice question of reading passage are almost of all types question become the most problem faced by the students. It is because the percentages of those types of questions are less than 60%. 3) The causes of the problems in answering multiple choice question of reading text are the students lack of vocabulary, the teacher do not give the students effective strategies to comprehend the reading passage and to answer the question, and the students less motivation in English.

To describe what the teacher and students talks constitute in classroom interactions, this study required videotaping as one of the research tools. This study used videotaping to capture natural interaction in detail. As Allwright and Bailey (1991) state the appropriate way to gather more complicated enquiries or more details data is audio record or even video record, so that we can get what was said, by whom, in what tone of voice. In addition, Burns (1999) argues that videotaping provides data of the classroom interaction both in verbal and in non-verbal behavior.
To answer the second research questions “what are the students’ writing errors?” , this study still used students ‘recount texts. The texts were analyzed to seek the frequent errors in term of verb tense, adjective, subject/verb agreement, pronoun, plural/singular noun, preposition, article, word order and spelling. The last, to answer the third research question, “what are students’ writing problems?”, this study used questionnaire and interview. Questionnaires were distributed to fifteen students as respondents. Through the questionnaire in two items, this study identified students’ dominant problems in both linguistics and non-linguistics problems. the interview was adressed to three students and one teacher. The interview covered the students and teachers’ perception towards writing competence. Open interview was applied through this study
Most people still misunderstand the definition of errors and mistakes (the difference between errors and mistakes). According to James (1998:62), “Error analysis is on the other side of the equation, being the study of linguistic ignorance, the investigation of what people do not know and how they attempt to cope with their ignorance. Meanwhile Brown (2000:217) stated that errors as a noticeable deviation from the adult grammar of a native speaker, reflects the competence of the learner; mistake refers to a performance error that is either a random guess or “a slip” in that it is failure to utilize a known system correctly. Harmer (1983:35)points out that error is the result of incorrect rule learning language has been stored in the brain incorrectly; mistake is less serious since it is the retrieval that is faulty not the knowledge. Hubbard (1983 : 134) stated that Errors caused by lack of knowledge about the target language (English) or by incorrect hypotheses about it; and mistakes caused by temporary lapses of memory, confusion, slips of the tongue and so on.
RSUD Salatiga is one of the government-owned hospitals in Central Java province that provide health services for the community, one of which is for outpatient services. In the data processing management RSUD Salatiga have implemented the Hospital Management Information System (SIMRS). Although has been used information technology, its service did not yet performed maximaly, so the quality of services in SIMRS needs to analyzed to find out that IT usage is running as expected. This study, research used the COBIT framework domain 5 Deliver, Service and Support. The purpose of this research are to evaluate, determine the level of capabilities, and the improvement strategies of hospitals IT performance which assessed. This research using qualitative method and supported with PAM to show the process that need to be assesed which DSS01 and DSS06 these are at level 2 (Managed Process), it means that the process has been implemented (planned, monitored and adjusted) and the results can be well defined, controlled and maintained. In DSS02, DSS03, DSS04 and DSS05 are at level 1 (Performed Process) that means the process has already implemented and achieved. Thus COBIT 5 domain Deliver, Service and Support can help to evaluate the performance of IT services at the Hospital Outpatient Installation Salatiga.
final part of the definition: analysis using mathematically-based methods. ɪ n order to be able to use mathematically based methods our data have to be in numerical form. This is not the case for qualitative research. Qualitative data are not necessarily or usually numerical, and therefore cannot be analyzed using statistics. The last term of the definition refers to the use of mathematically based methods, in particular statistics, to analyze the data. This is what people usually think about when they think of quantitative research, and is often seen as the most important part of quantitative studies. This is a bit of a misconception. While it is important to use the right data analysis tools, it is even more important to use the right research design and data collection instruments. Based on Sukamolson (2010,p.2) however, the use of statistics to analyze the data is the element that puts a lot of people off doing quantitative research, because the mathematics underlying the methods seem complicated and frightening. ɪ n addition, quantitative research is actually about collecting numerical data to explain a particular phenomenon, particular questions seem immediately corrected to being answered using quantitative methods.
Language is very important in social life, because language has a tight position to every part of life. That is why nobody lives perfectly without language as an instrument. The main function of language is to communicate and interact among human beings, without language human beings cannot express their feeling and communicate with other people.
