Pengkajian yang mendalam memungkinkan perawat kritikal untuk mendeteksi perubahan cepat, melakukan intervensi dan asuhan keperawatan (Laura A. Talbot et al, 2007). Pengkajian dilakukan pada tanggal 29 maret 2016 jam 09.00 WIB, dengan pasien Ny. S umur 46 tahun, jenis kelamin perempuan, agama nasrani, pendidikan sarjana, pekerjaan ibu rumah tangga, alamat Purwosari Laweyan Surakarta, No. RM 258531, dengan keluahan utama nyeri pada lutut kanan, pasien agak sedikit takut untuk menggerakkan kakinya karena cemas. Pasien mengatakan lutut sebelah kanan terasa nyeri, setelah dilakukan op nyeri bertambah nyeri post op nyeri seperti tertusuk-tusuk nyeri pada bagian lutut nyeri dalam skala 8 nyeri terasa hilang timbul. Sebelumnya pasien melakukan pemeriksaan rawat jalan di puskesmas sekitar tempat tinggal. Tidak ada keluarga yang pernah mengalami penyakit seperti yang dialami pasien saat ini. Pola persepsi dan kognitif, pasien mengatakan nyeri pada kaki dengan skala 8 jika kaki kanan di gerakkan luka post total knee replacement akan terasa nyeri seperti tertusuk-tusuk dan terasa hilang timbul. Terapi injeksi yang diberikan adalah cefazolin 1g / 12 jam, ketorolak 30mg / 12 jam gentamicin 40mg / 12 jam, arixtra 2,5mg

ANALISIS LAPISAN TIN PADA PROTOTYPE TOTAL KNEE REPLACEMENT (TKR) DENGAN METODA PLASMA SPUTTERING

TiN coating has been performed with plasma sputtering method on prototype Total knee replacement (TKR). In this research the TiN is coated on the surface of the prototype knee cap and tibial components made from CoCrMo alloy. The results obtained the hardness of samples before TiN coated were an average of 256.49 VHN. Once in layers with TiN the average hardness is 339.66 VHN. Observations of microstructures with SEM appear to be a solid TiN layer and a homogeneous thickness with an average thickness of 1.9 μm. The phase analysis with XRD shows that the phase of the Total Knee Replacement (TKR) sample is a cobalt-based alloy phase consisting of γ (FCC) and phase ε (HCP) phases. TiN phase no reveal, because it is so thin that it cannot be detected by XRD.

PENGARUH EDUKASI DAN LATIHAN MOBILISASI DINI TERHADAP TINGKAT KECEMASAN DAN KEMANDIRIAN PASIEN POST TOTAL KNEE REPLACEMENT

Background ; Total Knee Replacement (TKR) patients who will have limited motion in the knee function, weakness, immobility and disability as a result is the inability to care for themselves, the individual is not able to do the needs and daily activities as usual. This study was to determine the effect of education and early mobilization exercises of the level of anxiety and independence in patients with after Total Knee Replacement. Research methods ; Research are using quasi- experimental design with pre and post control group design, with 34 respondents (22 experimental group, 12 controls group). Collecting data are using questionnaires and observation. The independent variable is education and early mobilization exercises, while the dependent variable are the level of independence and level of anxiety. The sampling technique using consecutive sampling. The population in this study were patients with after TKR in Prof. Dr. R. Soeharso Surakarta Orthopaedic Hospital and Surgery Main Karima Surakarta Hospital. Data were analyzed using independent t-test and paired t-test. Research result; There are significant differences in the level of independence and anxiety in patients before and after education and early mobilization exercises. The result of independent t-test on the level of anxiety was obtained p value (0.000) <0.05, significant difference of anxiety and self-reliance in patients given education with patients who were not given education and early mobilization exercises. Conclusion; There is an increased independence and decreased anxiety in patients who underwent Total Knee Replacement after education and early mobilization exercises, and there are differences in the level of independence and anxiety in the control group and the experimental group.

In this section, behavior of both tibio-femoral and patello-femoral contact forces during a kicking type of dynamic activity is presented by means of a two-dimensional, three-body segment dynamic model of the human knee joint. It is a well-established fact that in a class of activities such as stair climbing, rising from a seated position, or similar activities, large patello-femoral contact forces naturally accom- pany large knee-flexion angles. For these large knee-flexion angles, a rough estimate of the patello-femoral contact force can be easily obtained by considering a simple static equilibrium of the patella with patella tendon force and quadriceps femoris force. According to the static analysis, at full extension of the knee this force is practically zero; as the knee flexes during the above-mentioned activities, the patella-femoral force increases to very high values, e.g., at 135° knee flexion it can reach four to five times body weight. Results presented here indicate that the patella can be subjected to very large patello-femoral contact forces during a strenuous lower limb activity such as kicking even under conditions of small knee-flexion angles. Finally, under such dynamic conditions the patello-femoral contact force can be higher than the tibio-femoral contact force.

Hasil : Distribusi jenis kelamin perempuan lebih banyak dari laki-laki sebanyak 20 (90%) pasien. Jenis TKA yang terbanyak di lakukan pada pasien osteoartritis lutut adalah Cruciate Retention (CR) 13 (65%) pasien. Kelompok usia tertinggi 60 – 69 tahun sebanyak 13 (65%) pasien. Lokasi osteoartritis terbanyak terjadi pada lutut kanan sebanyak 10 (50%). Hasil kuesioner SF-36 didapatkan rerata skor pasien 71,54 memiliki interpretasi kualitas hidup yang baik dan hasil kuesioner WOMAC memiliki rerata total skor 38,5 memiliki interpretasi pasien osteoartritis lutut tidak mengalami gangguan fungsional lutut.

Penggantian komponen KIT dilakukan dalam interval waktu per 3 bulan dan dalam rentan waktu selama setahun terjadi perawatan per 3 bulan sebanyak 4 kali. Pada 3 bulan pertama terjadi perawatan komponen KIT 3 bulan yaitu penggantian KIT (P3), untuk 3 bulan berikutnya terjadi penggantian komponen KIT dengan periode 6 bulan (P6), periode 3 bulan berikutnya (P3) dan perawatan akhir pada periode 12 bulan (P12). Sistem penggantian komponen KIT untuk P6 dan P12 hampir sama dengan P3, karena dalam di dalam komponen P6 terdapat komponen P3 dan P12 terdapat komponen P3 dan P6. Sering terjadinya kegiatan penggantian komponen KIT per 3 bulan (P3) mengakibatkan terjadinya downtime yang tidak dikehendaki, karena penggantian komponen KIT (P3) CC 201 selama periode januari 2013 hingga desember 2013 terjadi penggantian sebanyak 29 kali. Untuk menentukan waktu penggantian komponen KIT (P3) sesuai atau tidak dengan umur komponen dapat di analisis dengan menggunakan beberapa metode dan metode yang dapat digunakan adalah metode age replacement.

The factor that exhibits the greatest influence during surgery is cutting accuracy, which is necessary to obtain a good fit of the implant component on the cutting plane (Bardakos, 2014). Failure in this aspect can lead to severe post-surgery problems (Clarius et al., 2009). The use of a jig holder compared with a guidance jig for cutting can more efficiently support and facilitate jig placement in terms of accuracy, consistency, time efficiency, and ergonomics. The conventional design process is time-consuming and laborious as well as risky because of the trial-and-error method involved. Computer-aided design (CAD), computer-aided manufacturing (CAM), and virtual reality have been reported to reduce process time. For instance, RP provides great advantages in reducing the development cycle time, cost, and quantity of parts needed for final assembly while improving the quality of the design (Dieter, 2000; Liou, 2008; Bibb et al., 2009). Metal Injection Molding (MIM) is combination method of powder metallurgy and plastic injection molding. MIM is suitable process to produce small and complex geometry in large quantities (Arifin et al., 2015). Moreover, Metal injection molding has been identified as a potential manufacturing method for improving the production of metal parts (German & Bose, 1997)

Protein C, protein S, antithrombin III, and hyperfibrinogenemia in deep vein thrombosis (DVT) among patients who underwent high risk orthopaedic surgery

Kasus trombosis vena dalam (TVD) pasca operasi di Indonesia dianggap jarang, demikian pula dengan trombofilia. Oleh karena itu, penulis berpendapat bahwa diperlukan penelitian untuk mendapat angka kejadian TVD pasca operasi ortopedi risiko tinggi, dan profil trombofilia pada kasus TVD dan non-TVD di Indonesia. Penelitian cross sectional ini dilakukan pada 20 pasien yang menjalani operasi daerah panggul (total hip replacement dan fiksasi fraktur femur proksimal) dan daerah lutut (fiksasi femur distal dan total knee replacement). Pada tiap pasien dilakukan pemeriksaan protein C, protein S, antitrombin III, dan fibrinogen pada hari kelima pasca operasi, kemudian pada periode antara hari kesepuluh dan keduapuluhsatu pasca operasi dilakukan pemeriksaan USG kompresi/Doppler vena. Bila hasil USG-nya menunjukkan adanya TVD, maka dikonfirmasi dengan venografi. TVD ditemukan pada lima pasien (25%). Defisiensi protein C (P= 0,46), protein S (P= 0,81), antitrombin III (P= 0,46), dan hiperfibrinogenemia (P= 0,0547) tidak berkorelasi dengan TVD pasca operasi. Namun demikian, hiperfibrinogenemia merupakan faktor risiko TVD pasca operasi (attributable risk= 1). Faktor penyerta lain seperti diabetes mellitus (P= 1,0), obesitas (P= 0,28), hipertensi (P= 1,0), hipertrigliseridemia, dan hiperkolesterolemia tidak berkorelasi dengan TVD pasca operasi. Penelitian ini menunjukkan adanya kasus TVD pasca operasi di Indonesia. TVD tidak berkorelasi dengan defisiensi protein S, protein C, dan antitrombin III. (Med J Indones 2004; 13: 24-30)

Some metals are used as passive substitutes for hard tissue replacement such as total hip and knee joints, for fracture healing aids as bone plates and screws, spinal fixation devices, and dental implants because of their excellent mechanical properties and corrosion resistance. Some metallic alloys are used for more active roles in devices such as vascular stents, catheter guide wires, orthodontic archwires, and cochlea implants. The first metal alloy developed specifically for human use was the “vanadium steel” which was used to manufacture bone fracture plates (Sherman plates) and screws. Most metals such as iron (Fe), chromium (Cr), cobalt (Co), nickel (Ni), titanium (Ti), tantalum (Ta), niobium (Nb), molybdenum (Mo), and tungsten (W), that were used to make alloys for manufacturing implants can only be tolerated by the body in minute amounts. Sometimes those metallic elements, in naturally occurring forms, are essential in red blood cell functions (Fe) or synthesis of a vitamin 812 (Co), but cannot be tolerated in large amounts in the body [Black, 1992]. The biocompatibility of the metallic implant is of considerable concern because these implants can corrode in an in vivo environment [Williams. 1982]. The consequences of corrosion are the disintegration of the implant material per se, which will weaken the implant, and the harmful effect of corrosion products on the surrounding tissues and organs.
