Outflow flow meter for the Moolart Well WWTP was malfunctioning, resulting in outflow readings between 9 April and 31 December 2020 not being recorded.

What was the actual (or suspected) environmental impact of the non-compliance?

NOTE – please attach maps or diagrams to provide insight into the precise location of where the non- compliance took place.

There was no environmental impact from the non-compliance. Monthly water quality monitoring of the WWTP showed that parameters such as Total Nitrogen, Total Phosphorus and Total Suspended Solids were within standard WWTP parameter ranges (please refer to AER for monitoring results). Waste water in-flow to WWTP was less than the 75m3 daily limit.

Cause (or suspected cause) of non-compliance:

The outflow meter ceased operating due to a malfunction.

Action taken to mitigate any adverse effects of non-compliance and prevent recurrence of the non-compliance:

A new flow meter was ordered in November 2020 and was fitted in March 2021 on arrival to site.

Condition no: 1.2.2 Date(s) of non-

compliance: 14/07/2020 Details of non-compliance:

Decant return water pipeline was ruptured by a wheeled dozer that was tasked to cover the above ground water pipes to allow the passage of dump trucks. Whilst tidying up and reinstating the windrow, the blade made contact with the decant return water pipeline causing minor

damage and a water leak. The operator called the supervisor immediately to report the incident.

What was the actual (or suspected) environmental impact of the non-compliance?

Decant water spilled onto approximately 0.1ha of previously cleared haul road. Water quality monitoring from the decant location in Stirling In-pit TSF for the previous 3 months is as follows:

TDS: 3500 – 3900mg/L CN: 30 – 44mg/L and pH: 9.35 – 10.04 pH units.

Operator miscalculated size/location of blade in line with the pipe line location.

Action taken to mitigate any adverse effects of non-compliance and prevent recurrence of the non-compliance:

Task stopped;

Damage reported immediately to supervisor;

Water line isolated and shut off; and

Area bunded off for containment of the decant water.

The decant water was left to evaporate.

Cause (or suspected cause) of non-compliance:

Inspection was not able to be conducted as there was an increased work load or understaffing issues which prevented the inspections occurring

Action taken to mitigate any adverse effects of non-compliance and prevent recurrence of the non-compliance:

Planning of day work activities and ensure this is planned into the scope of works daily.

Additional compliance will be undertaken by the environmental department. Inspection records shall be validated as part of the monthly Environmental and Social Governance audit.

No water quality parameters were sampled for RRLMWMB045 to RRLMWMB050 due to water pump failure in quarter 1.

There was no environmental impact from the non-compliance. Monitoring was conducted for the remainder of the year with no fluctuation or upward trends in the results. The accompanying map outlines the bores location around Stirling In-pit TSF (please refer to AER for monitoring results).

Mechanical failure with pump on completion of sampling MB01-05 in the first quarter.

Action taken to mitigate any adverse effects of non-compliance and prevent recurrence of the non-compliance:

The sampling pump was sent off-site to be fixed and was returned in time for quarter 2 water sampling. A bailer has also been purchased to allow a sample to be collected on the off chance the pump breaks down during sampling.
