Analytical Exercise Set on Input – output Design Lecture
(This set do not cover the theoretical questions)
1. A doctors' surgery consists of five doctors a receptionist and a manager. They need an
information system to help them to run the facility. A patient may ring the surgery to make an appointment with a doctor. Each patient nominally has a doctor associated with him or her but they may often opt to see any doctor in the surgery that is available. The receptionist sees which doctors are on duty on which days and offers appointment alternatives from which the patient may choose. If an appointment is not available within a short time and the patient must be seen quickly they are asked to attend an emergency surgery that takes place every evening between 5 and 6 p.m. The appointment can be 5, 10 or 20 minutes long, dependent on the reported reason for seeing the doctor. This reason is recorded on the system. Sometimes patients ring to cancel appointments. Appointments may be made for up to six weeks in advance. Appointments that are more than 3 weeks old are automatically deleted from the system. Some appointments are for a doctor to go and visit a patient at home when the patient cannot come to the surgery.
Every day one of the doctors is available for home visits in the afternoon. A record is kept of each patient and the treatments they have received for any ailments they may have had. Typically the doctor who sees a patient will want access to this information before deciding on the relevant treatment to give. When the doctor prescribes treatment, details will be recorded in the patient's record.
Repeat prescriptions are automatically produced by the system and are available for collection at the surgery by the patient. At any time a doctor may suspend or cancel the prescriptions.
Patients may register with the surgery providing the number registered to each doctor is not above a certain maximum. Sometimes patients die or leave the area. In this case the patient is removed from the system and their details are archived. The manager is responsible for dealing with this aspect.

2. Bengali Food company followed a discount policy for the customers as follows.
If the customer want to pay his/her order on credit card and customer's record is good the order will be accepted but do not give any discount. If customer's record is not good do not accept any order.
If the customer want to pay his/her order on cash and sells amount is more than 1000BDT discount will be given 25%.
If the customer want to pay his/her order on cash and sells amount is between 500 and 1000BDT than discount will be given 15%.
If the customer want to pay his/her order on cash and sells amount is less than 500BDT, order is accepted but no discount will be made.
a. Draw an invoice form generated by the "DIU book shop" to customer.
3. "High Tech Computer" receives order from distributors, resellers, and individuals for computers and accessories at a discount, which depends on the size of the order and type of customer. The clerk in charge stores and verifies the order and determines shipment cost. He also identified the
type of customer for each order and sends related information to accounting department. An invoice is generated and accounts receivable are processed through the accounting department.
IBM has following discount policy to generate invoice.
Distributor get a trade discount of 25% on orders of 500 computers or more, otherwise get 15%
discount. For orders from resellers, 5% allowed on orders of 6-19 computers; 10% on orders for 20-49 computers; 15% on orders for 50-99 computers; 20% on order for 100 computers or more.
For order individuals 5% discount is allowed on order of 3 computers or more; otherwise get no discount.

4. "Unity Bank Ltd" has computer-based Banking information management system. They have also computer-based human resource management system. Every employee has unique user ID and password and their access of the information systems are controlled using those user ID and pass ord. They offer on-line customer Bank Account opening process. A customer can apply for bank account from anywhere in the world using web-based interface and submit a]1 the required documents using on-line. To confirm his/her personal in and address the system uses National ID card or TIN certificate and any recent utility bill documents, when all the verification will be completed the customer will received Account opening information. To activate the account number the customer should deposit minimum 1000 taka. After activating account information he or she can apply for Cheque book or ATM card. After getting Cheque book or ATM card he/she can withdraw money from the bank or ATM booth. Every deposit and withdrawn transaction is recorded in a file and the customer can collect the Bank statement from the Bank for a specific period.
a. Design an input screen to deposit money in the Bank from any branch.
5. "FAZ Technology Solutions Ltd" is a Technology and Business Solution company which is founded in 2005. They are providing Computer Repair, Networking. Wireless, Hardware Support, Child Protection Case Management System (CMS), Enterprise Resource Planning (ERP) and
Monitoring, Evaluation and Reporting System (MER). The "FAZ Technology Solutions Ltd"
company provides 50 printers ordered by "BD Soft Limited” software company. After some day they have detected printer problems based upon symptoms described to them over the phone from their clients. They will check the power cable if printer does not print, red light is not flashing and printer is unrecognized. They will check or replace ink printer if the red lights is flashing or also if the printer does not print or printer is unrecognized at the same time the red light is flashing.
a. Draw an invoice form generated by the "FAZ Technology Solutions Ltd" company system for the ‘BD Soft Limited’ company.
6. If you were asked to adopt a method for tagging merchandise for a retail store, what input medium would you chose? Why?
7. Testy Food is a fast-food restaurant offering all kinds of potatoes. The manager has a 30-second rule for serving customers. Servers at the counter say they could achieve that rule if the form they must fill out and give to the kitchen crew were simplified. The information from the
completed form is entered into the computer system at the end of the day, when the data entry person needs to enter the kind of potato purchased, additional purchased, the quantity, and the price charged. The current form is difficult for servers to scan and fill out quickly.
a. Design and draw a input form (you choose the size, but be sensible) that lists possible potatoes and toppings in a manner that is easy for counter servers and kitchen crew to scan, and can also be used as input for the inventory/reorder system that is on the extranet connecting Speedy Spuds and Idaho potato growers. (Hint: Remember to observe all the guidelines for good form design.)
b. Design and draw an output form to display screen that can be used by the servers and clerks to fill in the information captured on the form.
