NOTICE
EXAMINATION ENTRY FORM FILL-UP FOR CMA AUGUST-2015 EXAMINATION
CMA AUGUST-2015 examination will start tentatively from 31st August, 2015 and will be held simultaneously at Dhaka, Chittagong, Rajshahi, Khulna & Comilla Centers. Examination entry forms and fees will be received as per following schedule during office hours on all working days in the above centers:

02.00 p.m. to 4.00 p.m. Professional Levels– III & IV Tk. 500.00 Students will be allowed to fill-up examination entry form on Tuesday, 04th August-2015 with late fee of Tk.300/- (Taka Three hundred) only.
Instructions:
(i) A student will have to fill-up examination entry form for all subjects of a particular level, otherwise examination entry form will be cancelled.
(ii) Due annual subscription must be cleared with examination fees.
(iii) Duly fill-up ‘Examination Application Form’ will have to be submitted along-with 2 (two) copies of passport-size recent photographs. Name and Registration Number must be written on the backside of photographs.
(iv) A student will have to mention their full name, address and contact phone number on the
‘Examination Application Form’ clearly.
(v) Copy of Grade sheet/Admit Card of previous examination will have to show to the officials of the Institute during the time of examination entry form fill-up.
For further details students are advised to contact the Additional Director (Exam)/Deputy Director (Edn.) of Dhaka office and Officer-in-Charge of the respective branch centers.
