BLASTOKISTA PADA ENDOMETRIUM UTERUS

Reproduksi manusia mengandung suatu paradoks yang fundamentaL Meskipun sangat penting bagi kelangsungan keberadaan spesies, prosesnya tidak begitu efisien. Kemungkinan terjadinya konsepsi selama satu siklus menstruasi kurang !t~:"'j 30 persen. Dari seluruh kOll.3Cpsi,hanya 50 sampai 60 persen )"cmg san,;,:::;l.Ip b;tahan melebihi 20 minggu, Sekitar 75 persen kegagalan kehamilan disebabkn.:l kegagalan implantasi sehingga secara klinik tidak diketahui adanya kehamilan. Tujuan penulisan makalah ini adalah untuk mengetahui faktor-faktor apa saja yang dapat menghambat terjadinya proses implantasi.

Hambatan pada proses implantasi dapat disebabkan oleh abnormalitas blastokista, yaitu akibat dari gangguan pembelahan dan kelainan kromosom. Faktor berikutnya adalah abnormalitas endometrium. Kelainan anatomi dan histologi endometrium dapat terjadi akibat kelainan congenital, infeksi dan proses keganasan seperti septum, polip, dan myoma Gangguan pada keseimbangan hormon (progesteron, estrogen, kalsitonin) dan ekspresi dari beberapa factor seperti sitokin, faktor pertumbuhan, dan enzim juga dapat menimbulkan pada perubahan struktur dan fungsi normal endometrium. Selain faktor-faktor tersebut, implantasi dapat terhambat akibat penggunaan IUD, pil kontrasepsi hormonal, serta obat-obatan seperti clomiphene citrate.

Kesimpulan dari karya tulis ini adalah faktor yang menghambat implantasi dapat berasal dari kelainan embrio dan / atau kelainan endometrium. Pef1yebab kelainan dapat berasal dari dalam maupun dari luar tubuh.

Human reproduction entails a fundamental paradox. Although it is critical to

the survival of the ,~pecies,the process is relatively inefficient. The probability (!f

conception during one menstrual cycle is approximately 30 percent. Only 50 to 60

percent

of all conceptions advance beyond 20 lveeks qf gestation

Of the

pregnancies that are lost, 75 percent represent a failure qf implantation and are

therefore not clinically recognized as pregnancies. The aim of this paper is to .find

out factors that can inhibit implantation.

Inhibition on implantation can be caused by blastocyst abnormality, Hhich

happened by cleavage disorder and abnormality in chromosome. Another factor

is endometrial abnormality. Anatomical and histological changes can be caused

by congenital anomalies, i/?fections and malignancy, such as septate, polJ.p, and

myoma.

PENDAHULUAN

1.1 Latar Belakang

Seorang anak merupakan buah cinta kasih pasangan suami istri. Kehadirannya

merupakan dambaan bagi tiap pasangan, dan bagi keluarga mereka. Namun

adakalanya harapan terse but tidak dapat terpenuhi dan dapat menyebabkan

penderitaan dan ketidakharmonisan keluarga.

Ketidakmampuan pasangan suami istri untuk mendapatkan keturunan setelah

satu tahun melakukan hubungan seksual yang teratur tanpa upaya pencegahan

kehamilan, termasuk di dalamnya ketidakmampuan mempertahankan kehamilan

sampai bayi dapat dilahirkan, disebut infertilitas (Waddell, 2001).

Menurut Guyton and Hall (1997) satu dari enam sampai delapan pasangan

suami istri adalah pasangan yang tidak subur, dan 60 % diantaranya disebabkan

karena sterilitas wanita.

Reproduksi manusia mengandung suatu paradoks yang fundamental. Meskipun

diperlukan bagi kelangsungan keberadaan spesies, prosesnya dapat dikatakan

tidak efisien. Kemungkinan terjadinya konsepsi selama 1 siklus menstruasi kurang

lebih 30 %. Oari konsepsi yang berhasil terjadi, hanya 50-60 % yang sanggup

bertahan melebihi 20 minggu. Sekitar 75 % kegagalan kehamilan disebabkan

kegagalan implantasi, karena itu secara klinik tidak diketahui adanya kehamilan.

Kegagalan imp1antasi juga merupakan faktor penyulit utama pada kehamilan

buatan, penyebab terjadinya infertilitas dan abortus dini (Normitz, Schust, Fisher,

2001).

Memberi masukan pada pasangan yang mengalami masalah

infertilitas mengenai tilktor-faktor yang mengganggu implantasi. Tujuan

4.2 Saran

Penanganan pasangan yang mempunyai masalah infertilitas bisa dimulai

dengan mencari penyebabnya, apakah pihak pria atau pihak wanita. Bila pihak

pria tidak bermasalah, kemungkinan penyebab berasal dari wanita. Untuk

mengetahui apakah ada kelainan pada implantasi, dapat dilakukan pemeriksaan

struktur uterus (anatomi dan histologi), kadar hormonal, serta pemeriksaan

imunologis. Sebelumnya perlu disingkirkan kemungkinan penyebab yang berasal

dari luar sepert obat atau alat kontrasepsi. Bila hasil pemeriksaan normal, ada

kemungkinan

penyebabnya
