Some items in each kit are the same. For

example: toothbrush. Can we provide just

one toothbrush for reference/sample for

each kit?

Sample of each kit must include all items stated

in the Annex III, although there are similar items.

Items can be packed in a clear plastic bag and

labeled with name of kit.

Should we send sample of items on 6

March 2017 or before?

Sample of all items can be sent before 6 March

2017 at 17.00WIB

For item "shampoo size 90ml", apparently

it is no longer available in the market. Can

we provide the item with different size?

Correction: requirement for shampoo is size

70ml

The requirement for material of the bag is

canvas D420 or equivalent. Our

understanding is that canvas is not D420.

Terdapat beberapa barang yang sama di

tiap kit. Contoh: sikat gigi. Apakah boleh

mengirimkan contoh barang hanya satu

sikat gigi saja?

Contoh setiap kit harus lengkap berisi semua

barang yang tersebut pada Annex III, meskipun

terdapat beberapa barang yang sama. Isi kit

dapat dikemas dalam kantong plastik bening dan

diberi label nama kit tersebut.

ANNEX II

SPECIAL CONDITIONS

1. Vendor must submit the proposal strictly in accordance with Price Quotation Form along with (i) sample of all items; (ii) sample material for bags, not later than Monday, 6 March 2017 at 17.00 WIB to Ms. Ira Novita, 7th Floor Menara Thamrin, Jl. MH Thamrin Kav.3, Jakarta 10250. Phone: +62-21-29802300.

2. If selected, vendor must procure/provide goods based on the samples submitted. Failure to comply

to this requirement will lead to cancellation of the contract. 3. Delivery: partial delivery is not allowed.

4. Vendor is expected to submit quotation for goods with quality that equivalent to the following price estimation:

Quotations will be evaluated based on highest scoring overall method with the following combination:  Price: 80 point
