• A 22-month-old male presented with repetitive vomiting occasionally associated with behavioral arrest, right arm jerking, and rightward head deviation.
• MRI showed extensive left hemisphere polymicrogyria, most prominent in the left temporal lobe (Figure 1).
• These episodes were identified on video EEG (video 1)

: A Video Case
• Ictal vomiting is a rare seizure semiology, with a few distinctive features, reported in 2.7% of children with epilepsy (1,3).
• It can be seen in various epilepsy syndrome, particularly benign occipital epilepsies of childhood, to MRI-lesional localization-related epilepsy.
• It is a complex phenomenon hypothesized to involve the connectivity between insular cortex and vomiting center (2).
