Abstract

COVID-19 is caused by Coronavirus, which is a group of viruses that infect the respiratory system. Transmission is very fast because it is through droplets. The profession of dentists and dental therapists is a profession that is prone to transmission of COVID 19. In the new normal era, the role of dental therapists is key in preventing the transmission of COVID 19. This study aims to determine the role of dental assistants in preventing the spread of SARS-CoV-2 in dental practice in Semarang Regency. This research is a quantitative research with the type of research used is analytic observational with cross sectional approach. The sample selection in this study was based on the principle of appropriateness. Data collection was done by observation, interviews, and filling out questionnaires. The results of this study are that 81% of dental and oral therapists play a good role in preventing COVID at the Semarang Regency Health Center in the new normal era and 92% of the use of PPE in the medium category. It can be concluded that the level of PPE used is a combination of levels 2 and 3 used by dental assistants in Semarang Regency in the new normal era.

1. Introduction (Book Antiqua 10pt Bold) In December 2019, a new type of corona virus that can cause pneumonia was first discovered in Wuhan, China. (Li et al., 2020) This virus was originally referred to as 2019 novel coronavirus (2019 - nCoV), later officially named severe acute respiratory syndrome coronavirus 2 (SARS - CoV - 2) by the International Committee on Taxonomy of Viruses (ICTV). ) on 11 February 2020. On the

name of the disease caused by the virus (WHO, 2020). Coronavirus disease 2019 (COVID-19) is spreading rapidly to various countries and on March 11, 2020 WHO declared COVID-19 as a pandemic. (WHO, 2020a). In Indonesia, the first case of COVID-19 was announced on March 2, 2020 and up to the time this draft was lowered, there had been 64,958 confirmed cases and 3,241 (4.99%) deaths. (Ministry of Health RI, 2020).

Central Java Province alone the number of positive numbers for COVID 19 on December 31, 2020 reached 19,663 with the death toll reaching 1,652 people (Semarang City COVID Data Dashboard, 2020). The profession of dentist and dental nurse or Dental Assistant is a profession

SARS-CoV-2, the virus that causes COVID-19 also allows transmission in dental practice due to the possibility of inhalation of aerosols or droplets containing the virus or direct contact with mucous membranes, oral fluids, and instruments and surfaces contaminated with the virus. (Chen et al., 2020).

Treatment in dentistry is carried out with four handed dentistry , namely two operator hands and two assistant hands. They work as a team in order to produce quality dental care with a minimum of time, movement and stress. Role of the dental assistant in New Normal era will certainly increase, namely before the patient enters the waiting room, the body temperature must be checked first. After the patient is declared fine, then the patient is allowed to enter the clinic waiting room. With a note that you have to wash your hands with soap or use the hand sanitizer at the clinic. While in the waiting room, the dental assistant advised the patient to provide distance.

Patients are required to wear masks, if not, the clinic must provide masks for patients. When entering the examination room, patients are required to continue to wear masks, while telling

Equipment (PPE) at least using masks, goggles, and gloves. Before being examined, the patient was directed to rinse his mouth with an antiseptic.

After the examination is complete, the patient is required to return to wearing a mask again and then listen to the results of the examination. After the consultation and evaluation session from the dentist is over, the patient will be asked to rinse his mouth with antiseptic before leaving the room. The equipment used can be a single-use type. However, if the equipment is not a disposable type, the dental assistant will sterilize the medical equipment so that it kills the organism and can be used without transmitting it to the next patient. So it is hoped that the role of dental assistants is needed in compliance with health protocol enforcement for patients and other health workers in dental and oral health services in the New Normal era.

2. Method

This research is a quantitative research with the type of research used is deskriptif observational with cross sectional approach. The sample selection in this study was based on the principle of appropriateness. Data were collected by observation, interviews, and filling out questionnaires.

3. Result and Discussion

a. The role of dental assistants in the actions taken at the Semarang District Health Center in the new normal era.

Figure 1. The role of dental assistants in the actions taken at the Semarang District Health Center in the

new normal era

Based on Figure 1, it is known that the most respondents found that they had a good role (81%) in preventing COVID in the new normal

dental assistants in preventing COVID in the new normal era is in a good category.

During this COVID-19 pandemic, the public is advised to postpone dental treatment or not.

Assistants work in close proximity to the patient's oral cavity droplets. It is feared that there will be transmission between patients and medical personnel and between patients and patients. In addition, dental care is risky because of actions that cause aerosols (Purnamasari & Raharyani, 2020). The COVID-19 pandemic has had a significant impact on dental health. The emergence of anxiety in dental health workers and patients regarding the transmission of COVID 19 is one of the impacts. Therefore, both the community itself and especially dental health workers must be equipped with good knowledge in efforts to prevent the transmission of COVID 19 (Balbeid1 et al., 2021). Facing the new normal necessary to make adjustments to dental and oral health services, to prevent transmission so as to save the lives of patients and dental health workers. The role of dental health services, both doctors and dental assistants and mouths, is important in preventing the transmission of COVID 19 in the new normal era (Rokom, 2021).

The results of the study show that the role of the Dental Assistant for the Dental Polyclinic of the Puskesmas in preventing COVID in the new normal era has mostly been carried out well.

Dental Assistants have been able to carry out additional new normal activities such as:

1. Dental Assistants instruct patients to wait for queue numbers by keeping their distance.

2. Dental Assistant instructs patients to wash their hands with soap or use hand sanitizer when entering the operating room.

3. Dental Assistant instructs the patient to rinse the antiseptic solution before the procedure 4. Dental Assistant sterilizes the equipment after every procedure

5. Dental Assistant disinfects the dental chair after each procedure

6. Dental Assistant replaces PPE every time a patient changes

Based on the results of this study it can be seen that there are differences in the treatment by Dental Assistants to patients during the pandemic. This different treatment is to prevent cross-infection due to the infectious disease COVID 19. This is in line with Friska's research

7

(22)

the COVID-19 pandemic because there are several additional infection controls that must be applied.

The most visible difference is that before the pandemic there was no obligation to use N95 masks, there was no body temperature check at the dentist and/or dental nurse before work and there was no application of social distancing (Friska, 2021). So this is where the role of dental assistants is needed in preventing the spread of COVID 19 in the new normal era. So that it is necessary to have good behavior and awareness possessed by dentists and dental assistants to prevent cross-infection that can cause transmission of COVID 19. Compliance behavior of health workers can be influenced by knowledge and attitudes about COVID-19 (Limbu et al., 2020) .

The results of this study indicate that in preventing COVID-19 disease at the dental clinic at the Semarang Regency Health Center, Dental Assistant actively carries out cross-infection control. Research conducted by Kampf et al (2020) found several types of disinfectants that were effective against several types of coronavirus, including 78-95% ethanol, 70-100%

2-propanol, a combination of 45% 2-propanol and 30% 1-propanol, formaldehyde. 0.7-1%, povidone iodine 0.23%-7.5%, hydrogen peroxide 0.5%. The latest WHO guidelines state that a combination of water, detergent and disinfectant (1:50) is effective for SARS-CoV-2. Meanwhile, the dental treatment services served at the Semarang Regency Health Center were:

1. 97.2% had served dental examinations 2. 81.9% had served extractions

3. 78.3% had served fillings 4. 21.7% had served Scaling action

One of the actions in dentistry that can cause aerosol transmission is dental cavity preparation because it is handled using a dental handpiece (Rantela'bi', 2020). In the action that has been taken, aerosol suction is used to prevent the transmission of COVID-19, so that it can be ensured that the action is safe.

b. Use of Personal Protective Equipment Dental Assistant

Figure 2. Use of Personal Protective Equipment Dental Assistant

Based on Figure 2 it is known that most respondents found the use of PPE in the medium category (92%), good (5%) and bad (3%). It can be concluded that the level of PPE used is a combination of levels 2 and 3 used by dental assistants in Semarang Regency in the new normal era.

The results showed that most of the respondents found the use of PPE in the moderate category (92%). It can be concluded that the level of PPE used is a combination of levels 2 and 3 used by dental assistants in Semarang Regency in the new normal era. The PPE used by Dental Assistants when practicing dental health at the Semarang District Health Center used more gowns than hazmats as personal protective equipment.

The masks that are used more are 2-layer medical masks, while the KN95 masks and 1-ply masks are used less. Dental Assistant uses more faceshields than googles as eye protection. Most of the dental assistants use a nurse cap as a protective headgear. The handscoon used by the Dental Assistant uses more 1-layer handschoon than 2-layer handschoon as a protective tool for the palms. Dental Assistants use more ordinary shoes than booth shoes as a foot protection device. During the COVID-19 pandemic, it is recommended for dentists and/or dental nurses to wear level 3 PPE in performing dental care, which consists of a head covering, eye and face protection (face shield), N95 mask or its equivalent, scrub clothes / guard clothes, coveralls / gowns and aprons, latex surgical gloves and boots / rubber boots with shoe protection (Fatmawati et al., 2021).

To avoid work accidents, personal protective equipment must be used in accordance with standard operating procedures or hazards that can occur when working (Arbianti and Hanirizqy, 2019). From the results of the data obtained in this study, there are several PPE that are appropriate, some are still not appropriate based on the

8

(23)

Association regarding the use of PPE. PDGI in the new normal era recommends that PPE that must be used by dentists and dental nurses is a surgical scrub gown all cover and apron, eye protection, head cover, N95 mask, disposable sterile double gloves, and boots or closed shoes with all covers.

wear (Amtha, 2019). Dental Assistants are starting to realize and start complying with health protocols because they are starting to become aware of the dangers of the COVID-19 virus. So the results of this study are as follows: Dental Assistants at the Semarang district health center to be more alert in providing prevention against COVID-19 To make the public more aware of the dangers of the COVID-19 virus, it is necessary to monitor and evaluate as well as assist in providing the practice of preventing the COVID- 19 virus in the dental polyclinic of the Semarang Regency Health Center.

Based on the results of research and discussion, it can be concluded as follows:

a. Based on the results of the bivariate test on the role of dental assistants on the actions taken at the Semarang District Health Center in the new normal era, it was found that the most respondents found that they had a good role (81%) in preventing COVID in the new normal era, followed by the moderate category (19%) and bad(0%). It can be concluded that the role of dental assistants in preventing COVID in the new normal era is in a good category.

b.Based on the results of the bivariate test of the use of PPE on dental assistants for the actions taken at the Semarang District Health Center in the new normal era, it was found that the most respondents found the use of PPE in the medium category (92%), good (5%) and bad (3%). It can be concluded that the level of PPE used is a combination of levels 2 and 3 used by dental assistants in Semarang Regency in the new normal era.

4. Conclusion and Sugestion

Based on the results of research and discussion, it can be concluded as follows:

a. Based on the results of the bivariate test on the role of dental assistants on the actions taken at the Semarang District Health Center in the new normal era, it was found that the most respondents found that they had a good role
