ABSTRACT
Bengkalis State of Polytechnic is an educational institution that is committed to producing quality education, and continues to improve and meet the expectations of its customers. The Center for Development and Quality Assurance of Education (P4MP) is one of the work units responsible for ensuring the quality of service and teaching at the Bengkalis State Polytechnic. The P4MP task is the performance of the Bengkalis State Polytechnic BAAK in the form of a questionnaire that will be filled out by each student. Questionnaires conducted by P4MP are still done manually by filling out questionnaire sheets. This is considered quite slow from the results of the questionnaire, P4MP officers still have to manually calculate the average BAAK performance. The online questionnaire system was created to facilitate the process of filling out questionnaires filled out by students. The online questionnaire system is made web-based to make it easier for students to access it. The system is built using the laravel framework, while the database uses MySQL and is designed using UML (Unified Models Language).
