1 Witness see the accident at Plant site or out side like fire, injury or environment impact, etc and immediatelly report to Shift Leader on duty.
Shift Leader should be check the rightness of witness to now exactly the last condition.
2 Shift Leader instructs to Fire Fighting Team members through mobile phone or by pagging to go immediatelly to accident location and to take action as needed.
3 Shift Leader instructs to all communication radio (HT) switch to channel 9 (151,380 KHz) for coordination and communication during accident occurs.
4 Shift Leader also instructs to Fire Commander at Fire Garage by phone no. 14701 or cell phone (see chart above) or paging to go accident location completely with fire truck, fire protection and safety protection and tools as needed. At occurrence location, Fire Commader will coordinate fire fighting team and evaluate condition also control progress for handle condition and continue keep communication with Shift Leader.
5 Shift Leader instructs one persons of Main Plant operator go to location to isolation, switching and others activities as needed to keep and maintain plant operation and report the latest situation to Shift Leader.
6 Shift Leader informs to EHS Manager with phone 12008 or 0813 2643 6874 and reported the condition and action which already done. EHS manager can ask Shift Leader for update recent emergency situation.
7 EHS Manager and/ or Shift Leader inform and report to TJB PS Management by extention line phone or cell phone (see chart above). TJBPS management can ask to EHS Manager for the update recent emergency situation.
8 EHS Manager and / or Shift Leader instruct the Support Team through extention line phone or cell phone to go to accident location for helping the Main Team for handle the situation. Support team consists of First aid team, General Service and Security.
9 F&A Manager and HRD Manager Support EHS manager to inform about this occurrence to PLN Representative, CJP Representative, and KPJB Representative through mobile phone.
10 Depend on situation of accident, EHS Manager and/or Shift Leader and/or PLN Representative can inform to external parties such as: Fire Brigade Jepara Region with phone 0291 592113 / 593982 Kembang Sector Police phone 0291 7730110, Jepara Region Police phone 0291 591110, Bangsri District Military phone 0291- 771003, Graha Husada Hospital phone 0291 592067 ,Mardi Rahayu Hospital Hospital phone 0291 434707 /434708 and Kartini Hospital 0291 591175
11 For execution the emergency situation, Fire Commander; Fire fighting team; main plant operator and supporting team go to occurrence location to take immediately action.
Note: If EHS Manager not at site (evening/night shift, off day or week end), action as above can do by Shift Leader in charge.
