Template for Case Reports
CASE REPORTS (Fewer than 2000 words)
Authors must include a comprehensive literature review if a rare event.
Must contain Introduction, Case Report, and Discussion. The Introduction should include the need and rationale for the new technique.
Must use text TEMPLATE for guide: (see below).
Authorship
We believe it important to document the adequate participation of all authors in at least three major elements of a study and report; the number of authors will generally relate to the scope of the project. For case reports we request no more than 3 authors.
Title page
This page must include the following:
Title (containing fewer than 80 characters including spaces) and the words
“case report” should be in the title
Running title (containing fewer than 40 characters including spaces)
Author name(s) and final degree(s)
The affiliation(s), and address(es), and e-mail addresses of all author(s)
Word Count (Introduction through Discussion and Conclusions): Limit manuscript to fewer than 2500 words.
The Corresponding Author name , affiliation, address, and e-mail address
Abstract: Your Abstract must be structured with the following four sections and contain fewer than 250 words.
Background and provide the purpose of the case report
Case Description
Conclusions
Keywords: [Four to seven keywords for indexing purpose]
Introduction
Why you think this case is important – why you decided to write it up
We suggest one paragraph of background (citing but not amplifying relevant literature), one or two of rationale, and a final paragraph only stating the purpose of the report.
Case Report
Patient’s demographics and history.
Patient’s laboratory and diagnostic data.
Patient’s medication history.
Patient’s diet.
Discussion
Begin with a restatement of the novelty of the case or cases.
Including very brief review of similar published cases (how many similar cases have been published?) Compare and contrast your case or cases with those in the literature; when relatively few cases have been reported, provide a table comparing the key findings or outcomes.
List the limitations of the case report and describe their relevance.
Justify the uniqueness of the case
Describe how the information learned applies to one’s own practice.
End with a summary of the literature.
Conclusions
Provide a justified conclusion.
Provide evidence-based recommendations.

Declare here if any financial interest or any conflict of interest
Acknowledgments
Note any nonfinancial acknowledgments. Begin with, “We thank…” and note the nature of the contribution.
References
Ensure your references are complete and in number order and proper format using Vancouver style
Please limit the number of relevant references up to 20
Figure Legends
Provide brief legends to include the major point.
Figure legends should be written in complete sentences.
Please limit the number of figures and tables to 5 or less.
Tables
All tables should be double spaced. Each table on a separate page
