Referring to the requirement of your company to provide Intercity & Backbone connection, herewith we would like to submit our proposal as follows:

SCOPE OF WORKS:

 iForte will provide the FO equipments (belongs to iForte) for the FO Internet Access Connection installed at the customer premise.

 iForte will do the installation of FO equipments at the customer premise.

 iForte will provide the bandwidth as ordered by customer.

 iForte will give the technical support assistance and maintenance for the Internet Access.

 iForte will give Ip Public /29

MINIMUM CONTRACT PERIOD : 1 (One) Year PAYMENT INSTRUCTION:

1. Monthly subscription fee : Monthly payment in advance 2. Installation fee : Rp. 5.000.000

- SERVICE LEVEL AGREEMENT :

Uptime is 99,5 % (count per hour) (for dedicated internet only) - Mean Time To Repair ( MTTR ) :

Anytime a problem occurs, IFORTE will do at its best to resolve at a certain time. Whenever the repairing time is more than IFORTE’s standard (see table 1), IFORTE will apply a restitution based on the agreed SLG (Service Level Guarantee).
