SECTION II – ATTACHMENT 1A PERSONNEL QUALIFICATIONS

(Attached to and Part of CONTRACT No.4420003265A) 1.0 GENERAL

CONTRACTOR guarantees that all CONTRACTOR’s PERSONNEL are fully qualified and experienced as per minimum requirements set forth in the followings to safely perform the WORK in accordance with section II “Scope of Work” and CONTRACT.

COMPANY reserves the right conduct CONTRACTOR’s PERSONNEL competency assessment prior to deployment. COMPANY, at its sole discretion, may reject CONTRACTOR’s PERSONNEL not meeting the qualifications specified below or fail on COMPANY’s assessment and will request CONTRACTOR to provide PERSONNEL replacement with the same qualifications in a timely manner for further COMPANY’s review and APPROVAL.

CONTRACTOR shall be required to maintain validity of any PERSONNEL certification required as per the requirements set forth below during performance of the WORK.

The following abbreviations and definitions shall be used as references:

1. BS shall mean Bachelor of Science degree (engineering, project management) unless specified differently 2. CMRP shall mean Certified Maintenance and Reliability Professional

3. CON-MON shall mean condition monitoring of mechanical equipment (static/rotating) and/or electrical equipment and/or lube oil data collection and analysis 4. CONST shall mean construction/installation

5. D3 shall mean 3-year associate diploma (technical) unless specified differently 6. ENG shall mean engineering and design

7. EPC shall mean Engineering Procurement Construction

8. MTC shall mean mechanical equipment maintenance (static/rotating) 9. MIGAS shall mean Minyak dan Gas Bumi

10. OSHA shall mean Occupational Safety and Health Administration 11. PA shall mean Performing Authority

12. SMA / SMK shall mean Sekolah Menengah Atas / Sekolah Menengah Kejuruan 13. WPS shall mean Welding Procedure Specification

14. Total Experience shall mean total years of relevant experience in oil and gas industry relating to hydrocarbon process/facilities

15. Experience in Position shall mean total years of relevant experience in oil and gas industry relating to hydrocarbon process/facilities for position being specified

16. Computer literacy shall mean proficient in using Microsoft Windows and Office applications (e.g. Words, Excel, Power Point)

3.0 PERSONNEL QUALIFICATIONS – ORIGINAL EQUIPMENT MANUFACTURERS (OEMs)

As required by COMPANY, CONTRACTOR shall deploy Field Service Engineers (FSEs) authorized by respective Original Equipment Manufacturers (OEMs) listed below to carry out COMPANY’s equipment healthiness check, troubleshooting, maintenance (preventive, corrective, predictive), modifications, calibration, and other necessary services:

CONTRACTOR shall secure and provide PERSONNEL’s appointment letters issued by relevant OEMs authorizing that such PERSONNEL being deployed to site are qualified to carry out the above WORK accordingly.

Any tools required by the above Original Equipment Manufacturer (OEM) representatives and/or vendor specialists shall be provided by CONTRACTOR and deemed fully inclusive in respective PERSONNEL rates specified in section III.
