The data was classified using published criteria1 as mechanical (oxygenator failure, oxygenator clots, cannula problems, heat exchanger malfunction); hemorrhagic (cannulation site bleeding, surgical site bleeding, hemolysis, GI bleed, DIC requiring transfusion); renal (creatinine between 1.5 and 3, creatinine >3 regardless of intervention, dialysis requirement, hemofiltration
requirement, continuous arteriovenous hemodialysis [CAVHD] requirement); cardiovascular (requirement for inotropes, CPR after the initiation of ECLS, myocardial stun or cardiac tamponade diagnosed by echocardiography, cardiac arrhythmia requiring intervention, hypertension requiring vasodilators); pulmonary (pneumothorax, pulmonary hemorrhage);
infectious (culture proven infections, or WBC <1500); metabolic (glucose <40 or >240; pH <7.20 or >7.60)
