Based Assistance Continuum of Care Can Increase Family Independence in Overcoming Health Problems Pregnant-

Women

Abstract

Introduction

With an approach based on the continuum of care during pregnancy, it will be able to care for pregnant women, not only in the physical state, but also in the social-mental state, but also in the social health of pregnant women. The effect of continuity of midwifery care in pregnant women was 63.0% higher probability of spontaneous vaginal delivery and 23.1% less episiotomy.4. The aim of this study is to develop a minimum standard program of care for pregnant women by providing a continuum of care based on mentoring, with an emphasis on strengthening prevention, well-being, providing instructional functions, helping families to solve problems, with the end results of seeing family independence in 1-recognition . danger signs of pregnancy, 2-dealing with minor problems during pregnancy, 3-meeting the nutritional needs of pregnant women, 4-carrying out antenatal care, 5-taking blood pills.

This research was an experimental study with control, namely providing ongoing assistance to pregnant women using developmental assistance modules from maternal and child health books, with a population of 120 pregnant women (60 = treatment groups and 60 = control groups ), selected by simple. 1-implementation of guidance of families, 2-researchers and enumerators who act as facilitators and make 15 visits to pregnant women, 3-the companion conducts a survey of characteristics of pregnant women and pregnancy, 4-observations on the first and second week will be observed in 5 aspects of independence, 5-mentoring starts from the third week to the week before delivery, 6-the facilitator will record the results of the mentoring activities and carry out an evaluation of the mentoring results, 7-the researchers communicate actively, and once a month meet for discussion of mentoring, 8-at the end of the week will be observed preservation of the level of family independence. The stage of the data analysis was: calculating the difference between the mean value after being reduced before; makes equality by taking the logarithmic value calculated using excel with the formula log10, then multiplied by 100%, the final multiplication value was used to evaluate the efficiency.

Findings and Discussion

Differences in changes in the level of independence before and after assistance in intervention and control groups.

Discussion

The family does not recommend that pregnant women do not use sandals or high-heeled shoes, deliver mothers and accompany them to participate in pregnancy exercises to avoid low back pain. When pregnant women experience leg cramps, the family helps the mother to sit and stand. The food given and eaten by pregnant women is the same as other family meals, there is no difference in portion or frequency.

If pregnant women experience nausea-vomiting, the mother has difficulty defecating the family as a normal thing for pregnant women, so they have not tried to overcome it with food. During pregnancy, the family brings the pregnant women to check the pregnancy 9 times, which is 1 time at the gestational age of 1-3 months, 3 times at the gestational age of 4-6 months and 5 times at the age of 7-9 months. During the mentoring, the family as a companion assisted by the companion coordinator was educated about the importance of consuming tablets with increased blood for pregnant women contributing to the independence of the family to help consume tablets with more blood.

The results of observations under guidance, every day pregnant women routinely take tablets plus blood one tablet before going to bed with warm water, this is done to avoid possible side effects that occur, namely headache, nausea and difficulty in defecation. The effectiveness aid for the continuum of foster care independence in overcoming the health problems of pregnant women. It was found that based mentoring continuum of care had high effectiveness on family independence to overcome health problems in pregnant women.

As long as health care for pregnant women is properly monitored, pregnant women do not experience health problems that lead to pain, according to 98% of mothers. Mentoring with a one-based approach to care during pregnancy effectively increases family independence in caring for pregnant women, not only in terms of physical condition, but also in social and mental conditions, but also in caring for the social health of pregnant women. This is also reinforced by the results of previous studies by McLachlan, HL, Forster, DA and friends, showing that continued care by midwives during pregnancy for low-risk women can reduce the rate of caesarean section.4 Case management using continuous care or continuum of care for pregnant women diagnosed with HIV, shows lower outcomes for transmission to infants.6.

Conclusions

1 blood pressure within normal range, 98% of maternal nutritional status with a BMI between 23-24, high fundus uterus according to gestational age, 98% of fetal heart rate within normal limits. Assistance Continuum of Care performs functions in guiding, sustaining, protecting, supporting and empowering functions, namely functioning to guide the assisted family in the decision making process, activating and empowering the assisted family and coping fully or independently when problems arise health for pregnant -women-based assistance Continuum of care carried out in this study is mentoring, which refers to quality care using the principle of family empowerment, guided and tracks pregnant women from time to time during the mentoring period, optimizing family independence in overcoming health problems for pregnant women.1 Family empowerment acts as a link between maternal and child care and development of a health systems approach to strengthen maternal and child health services.5 Based mentoring Continuum of care with reference to quality of care from research findings effectively increases family involvement as human resources is the potential to empower oneself in achieving independence to solve and overcome health problems for pregnant women, to reduce the incidence of morbidity in pregnant women, this is appropriate Van den Broek, NR and Graham WJ's opinion that quality care must be at the heart of all strategies to accelerate the reduction of maternal and child mortality.6. With based assistance continuum of care maternal health problems can be overcome, mothers give birth normally, mothers are healthy, babies are also healthy.
