Enroll first on the schedule given according to your year level;
After having enrolled, present your assessed form to the Dorm matron & get application form;
Fill-up the Resident Information Data form (have all signatories sign) and pay at the Cashier’s Office the amount of Php500.00;
Submit the accomplished Resident Information Data Form with three (3) 2x2 ID pictures attached to the form & place it in a short folder, an
index card ½ (white), and a brown short envelope;
You are now privileged to have a space in the Habitat.
