1. De La Salle Lipa (DLSL) has a Trademark Policy and a licensing program that regulates the use of the DLSL registered trademarks, as well as the use of unregistered names, seals, logos, emblems, images, symbols and taglines that are representative of DLSL (together referred herein as “Marks”) by colleges, departments, academic or administrative units and student organizations within the community, and other external stakeholders, as authorized.
2. All use of DLSL Marks that are associated with DLSL, must be approved before any item can be ordered and produced.
3. All merchandise bearing the name and Marks are royalty- bearing at the rate established by the Chief Finance Officer.
Only items intended for internal use by DLSL and not offered for re-sale, are not royalty-bearing. Central Purchasing Department is responsible in transacting business with accredited and licensed vendors (with brand approval) only.

4. The Trademark Requst Form should be accomplished when faculty, staff, students, and other businesses and
organizations outside DLSL intent to use the names and/or marks of the institution for purposes of sponsorships, lectures, student events, programs, publications, fundraising activities, and retail ventures. The form may be downloaded from DLSL website (https://www.dlsl.edu.ph/intellectual- property/).

6. Upon completion of the form, please email a signed copy, along with all supporting documentation to: iptto.office@dlsl.
edu.ph
7. Provide Central Purchasing Department a copy of this approved form with the Terms of Reference or MSQ if applicable.
