STUDENT VISA (9-F) CONVERSION CHECK LIST
A. Bureau of Quarantine & National Intelligence Coordinating Agency (NICA)
1. Pick-up the Health Exam form and NICA clearance form from the Registrar’s office.
2. The closest place to take your Health Exam is Manila East Medical Center. This is located across and up the street from Jollibee here at Tickling.
3. Obtain your Health Exam test results.
4. You will need several 2x2 picture’s with white background.
5. Take your Health Exam test results to the Bureau of Quarantine. Your passport will be stamped. You will need two (2) 2x2 picture’s with white background.
6. Take your NICA form to Bureau of Immigration (Room 307). You will need two (2) 2x2 picture’s with white background. This process will take about 2 weeks to process. You will receive a cell phone number.
(It is advised that you go to Bureau of Quarantine early in the morning. Then you can go the same day to Bureau of Immigration. They are in separate locations.)
B. When your NICA clearance will be ready (after about 2 weeks as noted above) you can proceed with the rest of the application process
1. Student to request from Mrs. Jill Land, at the Immigration Desk in the Registrar’s office a. Notice of Acceptance (NOA)
Original copy containing a clear impression of the school’s official dry seal or duly notarized written endorsement from the school for conversion of the applicant’s status signed by the School Registrar. (P50)
b. Written Endorsement from the school for the conversion of applicant’s status to student on the school’s official stationary signed by the school’s Registrar and stamped with the school’s official dry seal. (P50)
(Please request these letters 1 week before you plan to go to immigration. Request forms are in envelope taped to Student Visa Officer’s desk.)
2. For Student to bring to Immigration a. The two letters requested above.
b. Letter of request from applicant. This letter must be typed and Duly notarized c. Passport and a photo copy of bio-page and latest admission and authorized stay d. Original copy of Medical clearance certificate issued by the Bureau of Quarantine and
International Health Surveillance or government medical institution with competence to certify that the applicant is not afflicted with any dangerous, contagious or
loathsome disease and is mentally fit.
e. Two (2), 2x2 pictures
3. For Student to complete at Immigration
a. General application form (BI Form No. MCL-07-001)
Duly notarized and accomplished by the applicant. 2x2 picture.
b. Personal History Statement. Signed by applicant and with a recent 2x2 picture c. Bureau of Immigration (BI) Clearance Certificate
C. ACR I-card
1. Application form
2. Finger printing, photo, signature
