Template Texts of Injury Compensation
(I) is liable for the compensation of injury attributed to adverse reactions due to the subjects’ participation in the clinical trial conducted according to the trial/study protocol. However, compensation is not applicable to any expected/anticipated adverse reactions stated in the informed consent form.
(II) The hospital is willing to provide professional medical care and consultation concerning any adverse reaction or injury due to the subjects’
participation in the clinical trial conducted according to the trial/study protocol. You will not be charged for any essential medical treatments for the adverse reaction(s) or injury(ies).
(III) Apart from the compensation and medical care described in the previous two paragraphs, no other forms of compensation will be provided. If you do not want to take such risk, please do not take part in the trial/study.
(IV) Signing this consent form will not affect your legal right.
