Purpose of NCP Intervention
“To resolve or improve the identified nutrition problem by
planning and implementing appropriate nutrition
interventions that are tailored to the patient/client’s needs.”

Methods for NCP Intervention
Address the problem- use a set of activities and materials (Assessment and Diagnosis)
Planning intervention- based etiology (form PES).
General objective of the plan- to relieve and reduce signs and symptom (form PES).
Implementation of the plan.

NCP-Intervention step- Planning
Prioritize the nutrition diagnoses.
Consult ADA’s Evidence-Based Nutrition Practice Guidelines and other appropriate practice guides.
Jointly determine patient-focused expected outcomes with the client and relevant care givers.
Develop a nutrition prescription and recommend specific strategies for altering dietary intake of energy and selected foods or nutrients.
Offer well defined, measureable, and achievable goals.
Determine timeand frequencyof intervention activities.
