Muc tieu: ddnh gia hieu qua cua phdu thudt cat he cung mgc (CliCM) de i/icu tri gldcdm a tre em vd mot so yeu td nguy co lien quan den thdnh cdng cua phdu thudt nay

Doi tu'ong v;i phirang phsip: nghien cuu hdi cim 5 nam tren r')'/' benh nhdn (106 incit) tir 0 14 ludi vin glocom tre em nguyen phat va thir phat trong thdi gian 2004 2008 dieu tri lgi Benh vien Mdt TP.HCM. lat ca benh nhdn duac phau thugt ('Bi '.M dcm thudn.

Ket qua: thai gian theo ddi trung binh 54.83 ' 36.66 thdng (12-192 thdng). Sau phdu thudt, 44 mat (41,5%) dugc ddnh gia thdnh cong hodn loan. 13 mdt (12.3%) thdnh cong vd 49 mdt that hgi (46.2%). Khd ndng sdng tich liiy cua phdu thudt ('BCM theo then gian dgt 72% liic 1 ndm, 64% liic 2 ndm, 54% liic 3 ndm, 43% luc 4 ndm vii 31% sau 5 ndm. Phdn tich hot quy ('ox cho Ihay tudi tgi thin diem phdu thugt cdng nho vd mire do ndng cua bCmh vui ti le C D 0,7 co nguy ca tdng nhdn dp tdi phdt cao hon.

Gldcdm tre em la benh hiem gap tren the gidi chiem ti le 1:15.000 den 1:10.000 tre sinh song a cac nude Chau Au va 1:2500 tai A-rap Saudi, Day la mdt trong nguyen nhan chinh gav mil d tre em.

tri gldcdm tre em va tim hieu nhirng yeu to nguy ca lien quan den ket qua ciia phau thuat CBCM d tre em Viet Nam.

Tat ca benh nhi tir 0-14 tuoi dugc chan doan gldcdm bao gdm gldcdm ticn trien va gldcdm thir phat dugc phau thuat cat be cung mac dan thuan (khdng sir dung chat chdng chuyen hoa), tir 2004 den 2008 cd ho sa theo ddi ngoai tni day du it nhat tren 12 thang.

Cd 71 benh nhan (110 mat) dugc phau thuat cat be cung mac, 3 benh nhan (4 mat) bi loai khoi nghien ciru v i khdng du ticu chuan nhan benh. phau thuat dudi 12 thang, nhdm 2 cd dp tupi tir 12-36 thang va nhdm 3 tren 36 thang. Thep bang 1, nhdm 1 cd 59 mat (55,7%), nhdm 2 cd 21 mat (19,8%) va nhdm 3 cd 26 mit (24,5%).

IV. BAN LUAN

Nghien ciiu Beauchamp [ 1] bao cao ti Ic thanh cdng cao nhat (50%) vdi thdi gian theo ddi ngin (trung binh 15,5 thang). Nhung nghien ciru khac vdi thdi gian theo ddi lau ban ciing cho ket qua tir 30-35% [4.5]. Trong nghien cim ciia chiing tdi, ti le thanh cdng sau 5 nam theo ddi la 31%, tuang tir nhu cac nghien ciru ciia cac tac gia nude ngoai. V. KET LUAN

Phau thuat cat be ciing mac nguyen phat khdng diing thudc chong tang sinh sgi trong gldcdm tre em cd ti le tai phat cao. Nghien ciru ciing chirng minh dugc rang nhung )'eu to nhu dp tuoi va mire dp nang ciia benh Uic phau thuat la nhiing nguy ca anh hudng den thanh cdng ciia phau thuat cat be cimg mac d tre em.

SUMMARY

EFFICACY OF TRABECULECTOMY FOR CHILDHOOD GLAUCOMA: A STUDY OF RESULTS WITH 5 YEAR FOLLOW - UP

Purpose: to evaluate the effectiveness of trabeculectomy m management ol childhood glaucoma and the risk factors affected the success of this surgery.

Methods: the 5 year retrospective study of 69 patients (106 eyes) from 0-14 years of age with congenital or development glaucoma al Ho Chi Minh City f'ye Ho.spital from 2004 lo 2008. These patients were performed trabeculectomies without using antifibrotic agents.

Results: the mean follow-up time was 54.83 -t 36.66 months with a range of 12-192 months. Fiftyfr'c eyes (53.8%) were considered successfiilly treated m which 44 eyes (41.5%) were absolute successes. The failure rate was 46.2% (49 eyes). Life table analysis identified that the success rates for intraocular pressure

control at 12, 24, 36, 48 and 60 mcmlhs were 72%>, 64%o, 54%. 43%. 31%, respectively Cox' regression analyses showed thai younger age al .surgery and glaucomatous eyes having CDR over 0.7 were more likely to manage lOP unsuccessfully with hazard ratio 0. 70 (p 0.033) and 1.97 (p-0.023), respectively

Conclusions: Ihe rale of success of trabeciileclomy in childhood glaucoma was not high. The patient age at Ihe lime of surgery and severity of the disease were proved to be the major risk factors for primary trabeculectomy.
