THE REDUCING IN THE POSTPRANDIAL GLYGEMIA OF CLEISTOCALYX OPERCULATUS'S

The study in the reducing in postprandial glycemia of Cleistocalyx operculatus (Roxb.) Merr and Perry flower bud in 20 type 2 diabetic patients was carried out in Hanoi. There are 2 days to do the experiments: the first day, patients were given 150 ml of water with eating the meal (330 kcal);

the second day (after the first day is 7 days), all these 20 patient were given 150 ml of Nu Voi tea and eating the same meal. The blood glucose levels were determined at initial time and at 15, 30, 60 and 120 min after eahng. Results, increase of the postprandial blood glucose level of subjects in the nu voi tea day was significantly lower than that in control day. Significant differences of blood glucose increrments at 30 min was established between the water day and the nu voi tea day (10,67 and 11,92 mmol/L; p<0,05). The difference of blood glucose AUCs between each meal with and without nu voi tea was statishcally significant (23,89 vs. 25,66 mmoVL.hr; p<0,01). The results of study suggested that nu voi can be a potential material supporting to control the post pran- dial blood glucose level in diabetic patients. And, nu voi tea canbe used as a beverage for prevent- ing diabet.

Nghien ciiu cat ngang tren 1217 doi tugng d thanh pho Hai Duong xae dinh ty le thua can beo phi (TCBP). Nhiing doi tugng cd BMI > 25 tu nguyen tham gia trong nghien ciru se dugc xet nghiem Cholesterol TP, Triglycerid, HDL-C, LDL-C nham xae dinh ty le rdi loan lipid mau (RLLPM) d ngudi thiia can beo phi. Ket qua: Ty le thira can - beo phi la 27,9%, beo phi do I la 1,6%, beo phi dp II la 0,4%. Ty le thira can beo phi d nam la 31,2% cao ban nii la 26,5%. Ty le rdi loan lipid mau d ngudi thira can beo phi la 86,4%; ty le rdi loan lipidmau d nam 87,5% cao hon d nii 85,8%. Ty Ic roi loan cholesterol TP 71% (nam 73,2%; nii 69,8%). Ty le rdi loan triglycerid cao 40,7% (nam 60,7% cao hon so vdi nii 30,2%). Ty le rdi loan HDL-C thip 0,6%. Ty le rli loan LDL-C 2,5%.

I. DAT VAN DE

Trong nhii'ng nam gan day cac bang chiing Idioa hge cho thiy cac benh cd lien quan den dinli dudng nhu: thira can - beo phi, tang huyet ap, •vu'a xo ddng mach, dai thao dudng typ II... cd su gia tang d Viet Nam, nhit la khu vuc cac thanh phd ldn [1]. Rdi loan lipid mau (RLLPM) la mdt trong cac van de quan trgng hang diu vl viia xo ddng mach va dang ngay cang gia tang theo nhjp do phat triln ciia xa hdi.

Theo bao cao ciia Td chiic Y t l Thi gidi (WHO), ty le thira can, beo phi tren toan cau da d miic bao ddng. Sd lieu thong ke tii' 84 nude tii nam 1999-2000 da cho thay chi rieng ty le beo phi (BMI

> 30 kg/m^) la 8,7%. 6 My va nhilu nude phat trien khac, TCBP dugc coi la mdt trong nhiing van de siic khoe hang diu trong nhieu nam qua.

Cac benh man tinh khdng lay nhiim la ganh nang benh tat, hi' vong d nhilu nude cdng nghiep nhu'ng cQng dang trd thanh

mdi de doa quan trgng ddi vdi cac nuae dang phat trien, nai md hinh benh tat dang thay ddi theo hudng gia tang cac benh man tinh khdng lay nhiem [12]. Rd rang van de thua can beo phi, mot bieu hien de nhan thay ciia tinh trang rdi loan dinh dudng - lipid dang dugc quan tam dac biet tren the gidi va trong khu vuc.

Muc tieu: Xae dinh ty le thira can beo phi va ty le rdi loan lipid mau oi ngudi 30-59 tudi bj thira can beo phi.

II. DOI TU'ONG VA FHlTONG PHAP NGHIEN GtltJ.

1. Doi tu'OTig: Ngudi tradng thanh 30-59 tudi d 8 phudng thanh phd Hai Duong (Thanh Binh, Pham NgG.Lao, Le Thanh Nghj, Quang Trung, Trin Phii, Cim Thugng, Nguyin Trai, Trin Phii) ;, Tieu chuan chon lua: Ngudi tradng thanh 30-59 hidi, khdng bj dj tat bim sinh hoae mac phai nhu gii, veo cot sdng mic cac benh cap tinh, man tinh (nhu lao, dai thao dudng, suy than..) ; phu nir cd thai, cho con bli; va cd su tham gia tu nguyen cua ddi tugng.

2. Phu'cmg phap nghien ciiu:

Thiet ke nghien cmi: Nghien ciiu cit ngang md ta.

p : Ty le beo phi ude tinh lay theo mdt kit qua nghien cim trade cua Nguyen Cdng Khin nam 2006 la 12%.

Theo tinh toan, mdi nhdm tudi (10 nam /I nhdm tir 30-59) ddi tu'gng nghien ciru se la 163 ngudi. Them 5%o ddi tu'gng du phdng, tdng sd ddi tugng cho mot nhdm tudi la 171 ngudi. Nhu vay tdng sd ddi tugng nghien ciru cho 3 nhdm tudi se la 513 ngudi. Do day la cude dieu tra cat ngang, lay mau chum nen nhan ddi ca mau da tinh toan d tren, ca mau can dugc chgn la: 513 X 2 = 1026 ngudi. Thuc te trong nghien cuu, da dieu tra cat ngang cho 1217 ddi tugng.

3. Danh gia va phan loai cac chi tieu.

- Danh gia RLLPM theo NCEP ATP III 2001: Cholesterol TP > 5,2 mmol/l, hoae HDL-C < 0,9 mmol/l, hoae LDL-C >3,36 mmol/l, hoae Triglycerid huyet thanh

>2,26 mmol/l.

- Danh gia TTDD: Chi so BMI= Can nang (kg)/Chieu cao-(m):

Dua theo phan loai cua td chii'c WHO (2000) khuyen nghi cho ngudi tra'dng thanh:

Bang 1 cho thay cd 1217 ddi tugng tai 8 phudng tham gia nghien cii'u cua thanh phd Hai Duong. Ty le cd 30,57% nam va 69,43%o nii'. Nhdm tudi 50 - 59 chiem ty le cao nhat (44 j % ) , tilp dd din nhdm tudi 40 - 49 (30,2%), sau ciing la nhdm tudi 30 - 39 (25,1%).

Ket qua cua bang 3 cho thiy d ca ba nhdm tuoi diu cd ngudi bi thira can - beo phi, dac biet d nhdm tudi 50-59 ca nam va nii diu cd ty le thira can - beo phi rit cao ( 37,6%o va 35,3%)). Tudi cang cao thi ty le thua can beo phi cang cd xu hudng tang cao ca a 2 gidi nam va nii.

IV. BAN LUAN.

BMI theo phan loai cua WHO, ty le ddi tu'O'ng thira can beo phi chiem ty le klia cao la 27,9%, trong dd ty le thira can beo phi d nam gidi la 31,2%, nii gidi la 26,5%).

Ty le beo phi do I la l,6%o; beo phi do II la 0,4%. Ket qua nghien ciiu nay cao ban so vdi ket qua tdng dieu tra beo phi toan qudc nam 2005 ciia Vien Dinh Duong tren 17213 ddi tugng tudi 25-64 cd ty le thira can beo phi la 16,3%), [6]. So sanh vdi kit qua nghien cuu cua Dinh Van Trang va cdng su nam 2007 tren ddi tugng sT quan Quan chiing Phdng khdng khdng quan thi ty le thua can beo phi cua chiing tdi thip ban (27,9% so vdi 55,8%)[8]. 6 ca nam va nii' tudi cang cao thi cang cd xu hudng bi thira can beo phi tang.

Ty le rdi loan lipid mau d ngudi cd BMI > 25 la 84%, theo nghien ciiu ciia Dinh Van Trung va cdng sir tren ddi tugng la can bd thudc dien quan ly sire khoe cua Ban Bao ve Sire khoe tinh VTnh Long (2003) thi ty le RLLPM la 80,9%, cdn theo Trin Dire Thg va cdng su (2000- 2001) ty le RLLPM tren benh nhan giam dung nap glucose la 84%; cac kit qua nay

So sanh vai ket qua cua Dd Dinh Xuan va Tran Due Long tien hanh nghien ciiu ngang thuc hien d 3 tinh ddng bang Bac Bd, 630 ngudi tren 40 tudi dugc chgn ngau nhien vao nghien ciiu de xet nghiem cac chi sd lipid mau co 41,9% tang cho- lesterol, ty le tang cholesterol cua chung tdi cao han (71%) so vai 41,09%)) nhung day la ty le rdi loan cholesterol tren nguai CO thua can beo phi. Nghien ciiu cua Rafal S. Filip va cdng sir (2006) d -vung Lubin (Ba Lan) thi ty le cholesterol cao hanvdi77,3%[ll].

Ty le triglycerid cao a nguai TCBP la 40,7%, phu hgp vai nghien ciru ciia Le Khac Due va cpng su (2007) khoang 40%, va ciia Nguyin Minh Tuan (2005) tai Thai Nguyen la 32,5%o a nguai thua can; 37%) a nghien cuu ciia Do Dinh Xuan va cdng su. So sanh ty le rdi loan

triglycerid a nguoi TCBP thi ty le trong nghien cuu cua chung tdi thap han so vai nghien cuu cua Dinh Van Trang la 47,9%[7].

Cholesterol toan phan va triglycerid dugc coi la cac chi sd danh gia nguy ca vua xa ddng mach. Neu cac chi sd nay cang cao, nguy ca vua xa ddng mach cang cao. Ben canli do, chi sd HDL-C lai dugc coi la cholesterol "tdt", co lgi vi no van chuyen cholesterol ra khoi td chuc de giam nguy ca vua xa ddng mach. Ket qua ehi CO 0,6%) sd nguoi TCBP trong nghien cuu cua chung tdi co HDL-C thap.

Ty le LDL-C cao la 2,5%o a nguoi thua can beo phi, thap ban so vdi ket qua nghien euu cua Nguyen Minh Tuan (2005) tai Thai Nguyen la 46,5%; thip hon so voi ket qua cua Le Bach Mai va cdng su (2004) la 8,23%.

V. KET LUAN.

1. Thuc trang thua can beo phi fr ngu'di tru'O'ng thanh tuoi 30-59 tai thanh ph6 Hai Du'cng:

- Ty le thira can - beo phi la 27,9%), beo phi do I la 1,6%, beo phi do II la 0,4 %.

- Ty le thua can beo phi d nam la 31,2%) cao ban d nii la 26,5%o.

- Ty le thiia can beo phi cd xu hudng tang len theo tudi. Tudi cang cao, ty le thii'a can beo phi cang cao.

2. Ty le roi loan lipid mau 6" nguoi thiia can - beo phi.

- Ty le rdi loan lipid mau chung d ngudi cd BMI >25 la 86,4 %; d nam la 87,5%,, cao ban d nu.' 85,8%)

- Ty le rdi loan cholesterol toan phan 71%o (nam 73,2%; nii' 69,8%).

- Ty le rdi loan triglycerid cao 40,7%o (nam 60,7% cao ban so vdi nii 30,2%))

Summary

OVERWEIGHT/OBESITY AND BLOOD LIPID DISORDERS IN PEOPLE 30-59 YEARS OLD IN HAI DUONG CITY

A cross -sectional study was conducted in 1217 subjects in Hai Duong city to determine the rate of overweight / obesity and blood lipid disorders among overweight/obesity targets ageds 30-59.

The subjects with BMI > 25 voluntarily participated in the study. Their total cholesterol, triglyc- eride, HDL-C, LDL-C were identified to determine the rate of blood lipid disorders in obese peo- ple. The results showed that: The rate of overweight was 27.9%, obesity was 20.6%, obesity grade I was 25.9%, and obesity grade II was 27.9%. Overweight and obesity in females (26.5%) was lower than in males (31.2%). The rate of lipid disorders in obese people was 84% and that in males (87.5%) was higher than in females (56.2%). The rate of high total cholesterol was 71%

(males 73.2%, females 69.8%). The rate of high triglyceride was 40.7% (males 60.7% higher than females 30.2%), low HDL - C was 0.6%, high LDL-C was 2.5%.
