Cac tac gia tien hanh nghiSn euU dac diem lam sang, xet nghiem ciia b?nh thieu mau thieu sat d benh nhi tai Vien Huyet hpc - Truyen mau Trung Udng trong cac nSm 2015-2016. Mpc tieu. Mo ta mgt so dac diem lam sang, xet nghiem eiia benh nhi bi thieu mau thieu sat. Ddi tu'dng va phddng phap nghien cuir 57 benh nhi, 6iihc chan doan xac dmh la thieu mau thieu sat tJieo tieu' chuan cua WHO 2001. PhUdng phap nghien cUu: mo ta cat ngang, tien cdu. Ket qua nghiin clhr. (1) Cae tn|u chUng lam sang noi bat:

Trieu chirng met mdi d nhom chay mau va nhom phoi hdp chiem 100%, d nhom chUa ro nguyen nhan chi cd 52.4%; 100% benh nhi eo trieu chUng da xanh niem mac nhdt; Tneu chUng nhip tim nhanh gap ty le cao tU 71,4 % trd len. + Nhdm chay mau: ed 13 benh nhi. Nguyen nhan gay chay mau do: loet da day, tri, ung t h u dudng tieu hda, giun mdc.

+ Nhdm khong cung cap dii va kem hap thu sat (KCCO va KHT): co 14 benh nhi. Nguyen nhan do:

che dp an kieng, cat da day, eat doan rupt.

+ Nhom phoi hdp: cd 9 beiih nhi. NhfJYig benh nhi nay bj thieu mau thigu sat do 2 hoae 3 nguy§n nhan gay neu tren.

+ Nhdm ehUa ro nguyen nhan (CRNN): cd 21 benh nhi.

2.1.2. TiSu chuan loai triy

- Nhutig benh nhi hoae bd, me b§nh nhi Widng dong y tham gia nghien eUu.

- Nhutrg benh nhi thieu mau thieu sat cd suy than, benh ly bam sinh hong cau, benh mau ac tfnh.

2.2. Phu'dng phap nghien cu'u 2.2.1. Thiet ke nghiSn cdu: Nghien ciru mdta eat ngang.

IV. BAN LUAN

4 . 1 . Mot sdi dac diem ve tuoi va gidi ciia doi tu'dng nghi§n cii'u: Trong 57 benh nhi, thieu mau thieu sat gap d tat ea eae giai doan phat trien eua tre, trong do nhdm tudi tir 12 den 15 tudi chiem ty le eao nhat la 50.9%, tiep theo la lira tuoi nhd hdn 2 chiem 29,8%, tudi tir 3-11 chiem t / le thap nhat la 19.3%. Ket qua nghien ciiu tUdng tU nghien eiTu cua M. Lan va P.T.L.

HUdng. Theo tac gia Mai Lan thi nhdm benh nhi tren 10 tuoi chiem 5 4 , 1 % , cdn tac gia Lan HUdng thi Mia tuoi ttr 11 - 16 tuoi ty le cao nhat la 43%. Ty le tre nam 58%, nu' 42%. Ket qua nay tUdng tU vdi tac gia M.Lan (nam 62,4%, nD' 37,6%) va t^c gia P.T.L.HUdng, nam 6 2 % .

4.2. Dac diem lam sang cua benh nhi Uiieu mau thieu sat

a nhdm nghien cufu, cae trieu chu'ng cd nang nhU: met mdi, chii'ng hoa mat chdng mat chiem t / le rat cao, cac trieu dau dau, an kem, kem tap truiig CO ty le thap hdn. Ve eae theu chimg thUe the, khi tham kham thi 100% benh nhi cd trieu chiing da xanh, niem mac nhdt. Trieu chii'ng nhip tim nhanh gap ty le kha cao: nhdm chay mau la 76,9%, nhom khong cung cap du va kem hap thu sat la 78,6%, nhom phoi hdp la 88,9%

va nhdm chua ro nguyen nhan la 71,4%. Trieu chutig tarn sang ciia thieu mau thieu sat phu thupc vao mu'c dp nang eiia benh. Neu thieu mau nhe thUdng khong co trieu chirng, khi xuat hien trieu chirng ttii benh da dien bien sang mire dd thieu mau nang hdn.

4.3.1. Dac dle'm dong hong cau vdi cac nhom nguyen nhan gay Isenh

Ket qua bang 4, 5 cho thay sd lUOng hong cau trung binh ciia nhdm nghien ciru la 3,8±0,4, nong do Hemoglobin la 73.2 ± 14.3, the tfch trung binh ciia hong cau, lUdng huyet sac to trung binh hong eau va nong dp huyet sae to trung binh eiia hong cau deu giam. Chf cd ty le phan tram hong cau lUdi va phan bo kfch thUde hong cau tang cao. Gia tri trung binh ciia MCV d benh nhi la 62.4 ± 11.2 (fl) va gia tri trung binh ciia RDW d benh nhi la 20.3 ± 3.3 (%), dayja nhD'ng chf so cd ban giiip eho djnh hudng ehan doan benh nhi bi thieu mau la do thieu sat.

Ket qua bang 6,7,8 cho thay the tfeh trung binh cua hong cau rat thap. Nhdm phoi hdp cac nguyen nhan gay benh ed 9 benh nhi chiem 100% cd mUc dp I^CV < 80 (fl). TUdng tU d eae nhdm nguyen nhan gay benh khae, mUe dd MCV

< 80 (fl) ehiem tren 80%. Khdng ed benh nhan nao cd mUe MCV > 100 (fl). LUdng huyet sac to trung binh ciia hong eau eho ket qua: 100%

benh nhi ed miTc dp MCH < 28 (pg).

4.3.2. Dac diem chuyen hda sat d cac nhdm nguyen nhan gay benh

a. Sat huyet thanh: Trong nghien eiTu nay, ehi so sat huyet thanh trung binh d tre em la 2.9 ± 2.0 (pmol/l). Chirng t d dai da sd benh nhi den vien vdi tinh trang thieu mau nang. Cu the d bang 9, 96.5% benh nhi co lUdng Fe huyet thanh < 7(pmol/l). Cd 2 trUdng hdp cd sat huyet thanh trong gidi han 7-27(Mmol/l), khdng ed trUdng hdp nao ed nong dp sat huyet thanh

>27(pmol/l). Dieu nay ed the cho thay benh nhi da thieu mau nang va keo dai. NhU vay xet nghiem sat huy€t thanh se ho trd eho chan doan ma chUa phan anh ehinh xac mu'c dp thieu mau thieu sat.

cho thay, nong dd trung binh eua ferritin d tre em la 6.7 ± 6.4 (hg/ml). Trong do ed den 86%

benh nhi cd nong do ferritin < 15 (ng/ml). Theo ket qua nghien cu'u eiia N.T.T. Ha thi ty le nay la 88%, tae gia H.T.T. Anh la 78.6%. Cd sU khac biet ve nong do ferritin giO'a nhdm chay mau vdi nhdm khdng cimg cap dii sat - kem hap thu va nhdm chay mau vdi nhdm phoi hdp d ngUdi Idn.

c Transferrin: Nghien euU cho ket qua vdi nong dd transferrin trung binh d benh nhi la 349.6 ± 6 5 . 2 (mg/dl). Nong dp transferrin > 250 (mg/dl) d tre em la 94.7%, trong dd nhdm khdng cung cap dii - kem hap thu sat va nhdm phdi hdp chiem 100%. Khdng cd trUdng hdp nao ed nong dp transferrin <200 (mg/dl). Cd den 97.1% benh nhi ed nong dp transferrin > 250 (mg/dl) thupc nhdm chay mau, tiep theo la nhdm phoi hdp vdi 93,2%, nhdm khdng cung cap dii va kerri hap thu sat la 92,3% edn nhdm ehu^ rd nguyen nhan la 91.3%.

V. KET LUAN

- Cac trieu chirtig lam sang ndi bat: Trieu chu'ng met mdi d nhdm chay mau va nlidm phdi hdp chiem 100%, d nhdm chUa rd nguyen nhan clil ed 52.4%; 100% benh nhi co trieu chii'ng da xanh niem mae nhdt; Trieu chutig nhip tim nhanh gap t / le cao tir 71,4 % trd len.

- Cac trieu chiTng can lam sang ndi bat; Mire iJ6 thieu mau la pho bien (Hb: 73,2 ± 14,3); Gap ty Ig 93% benh nhi cd mu'c MCV nhd hdn 80 fl; c6 100% tre cd nong dp MCH <28 (pg); cd 95,5%

mUc dd sat huyet thanh < 7pmol/l; cd 86% m lUdng ferritin<15ng/ml; lUdng Transfernn tang cao.

CLINICAL AND SUBCLINICAL CHARACTBRISnCS OF LABOUR INDUCnON I N NATIONAL HOSPITAL

OF OBSTETRICS AND GYNEACOLOGY Objective: Tc describe dinieal and subdinical characteristics of labour induction in National hospital of Obstetrics and Gyneacology, Methods: This Is cross sectional study among 65 pregnant women with 37 to 42 gestational age who were induced labour actively. Results: Beforelabour induction, Bishop score was ranging from 1 to 5 and mean score was 3,42 ± l,3n. There was 69,23% in group of amnionic fluid index under 60mm. Chorionic sac was still intact was 87,69%. Gestational age ranging from 40-41 was 82,54% (53 cases). Conclusion: Pregnant womai wffe induced labour whose mean Bishop score was 3,42±1,31 and mostly from 40 to 41 week of gestation.
