Setiap jenis pelayanan diidentifikasi seluruh aktifitasnya, dan urutan/langkah kegiatan pelayanannya. Dimulai sejak kegiatan awal melayani ketika pengguna datang, diproses menurut prosedur layanan sampai selesai menerima pelayanan. Pada proses ini perlu dicatat siapa saja pelaksana/petugas/meja/simpul yang terlibat menyelesaikan setiap tahapan pekerjaan tersebut. Juga dicatat bagaimana peran masing-masing pelaksana/petugas melakukan pekerjaan apa dalam proses penyelesaian pelayanan tersebut dan memerlukan waktu beberapa menit/jam.
Setelah melakukan pengamatan dan mempelajari, meneliti dan mencatat realita, kemudian hasilnya dituangkan dalam alat bantu pencatatan pada tabel berikut:
