A Player may only represent the declared club in the Champion of Club Champions – Disciplines of Singles, Pairs and Fours. A Player may only be selected to play in the State District Sides for a District that they have declared membership in a club in that district.
A member cannot change their declared club (for any of the above events) during that season/year.
Failure to complete this form may affect participation in these events.
