Department of Family Services Brisbane, 19th May, 1988 HIS Excellency the Governor, by and with the advice of the Executive Council and in terms of the provisions of the Adoption of Children Act, has been pleased to make the following regulations. The matters specified in Column 2 of the Sixth Schedule hereto shall be matters to be taken into account in making an assessment under section 13B of this Act in respect of persons whose names are included in an adoption list referred to in Column 1 of such Schedule is referred to. An applicant to become a foster parent must contact the Director-. a) the of the below mentioned as requested by the Director- (i) full birth certificate of applicant;. ii) deed survey or other evidence of change of name of applicant;. iii) marriage certificate of applicant;. iv) certificate of Australian citizenship of applicant;. v) a certificate of decree nisi which has become absolute;. vi) custody, access or guardianship order and any other orders in relation to a child to be adopted;. viii) a specialist medical practitioner's report indicating the reason for infertility;.

A person who has made an application to become an adoptive parent must, until an adoption order is made or the application is otherwise disposed of: notify the Director immediately of a change in any of the circumstances which may affect the eligibility of such person. No person, other than an officer of the Department engaged in the execution of this Act or other person engaged in the execution of this Act, shall have access to the registers, except by order of the Supreme Court. 17 ADOPTION OF CHILDREN REGULATIONS 1988 Reg. when the office of the Registrar General is open for the transaction of public business-. a) to be obtained from the Registrar General certified under his hand-. i) where the Registrar-General has registered an adoption order made in another State, a Territory of the Commonwealth, or a country outside the Commonwealth-.

REGULATIONS OF ADOPTION OF CHILDREN 1988 First Sch. APPLICATION TO BECOME AN ADOPTION PARENT AND TO HAVE THE NAME REGISTRED IN AN APPROPRIATE ADOPTION LIST. I understand the provisions of the Adoption of Children Act 1964-1987 relating to the giving of consent (particularly sections 19, 20 and 22 thereof). I agree to the adoption of the above-mentioned child in favor of the person(s) whose details are stated above.

I have explained the provisions of the Adoption of Children Act 1964-1987 in relation to the provision of consent (especially sections 19, 20 and 22 thereof) to the person giving this consent. CONSENT OF PARENT OR GUARDIAN TO ADOPTION OF CHILD INTO CARE IN FAVOR OF PARTICULAR APPLICANT. I explained the provisions of the Adoption of Children Act 1964-1987 in relation to giving consent (especially sections 19, 20 and 22 thereof) to the person giving the consent.

I wish the child to grow up in the religion of the prospective adopters approved by the Director, other than the religion(s) specified below-. enter details) OTHER PERSON'S CONSENT.
