PRE-ADMISSION INTERVIEW

It is essential that you attend the Hospital for a pre-admission interview during which details for admission will be t.1ken and method of payment of hospital account (if appropriate) established.

TL'1E:

HOSPITAL ACCOUNT:

Between 9 am and 6 pm Monday to Friday, 9 am and 5 pm Saturday, Sunday and Public Holidays approximately one week prior to tentative admission date (for obstetric patients approximately 4 weeks prior). If you have an appointment with the Anaesthetic Clinic it may be convenient to attend for your pre-admission interview during the same visit.

If you elect to be a private patient you are required to bring either your Health Fund Membership Book/Card (if your fund does not provide a book/card you must bring other evidence of membership to assign the debt).

You remain responsible for the debt until it is paid.

If you cannot establish adequate private health insurance cover, full prepayment of charges must be made for routine admissions expected to be of one or two days duration and 50% prepayment of charges anticipated periods of more than two days. Should you be unable to meet this requirement, you may only be admitted as a public patient. You will be advised at this interview of the amount involved which will be calculated from your expected length of stay in Hospital. The balance of the account is payable on discharge.

Bankcard or Mastercard facilities are available for payment of accounts.

UNLESS YOU HAVE ATTENDED FOR A PRE-ADMISSION INTERVIEW AND SATISFIED THE ABOVE REQUIREMENTS, ADMISSION TO HOSPITAL WILL NOT TAKE PLACE.

CONSENT

By coming to Hospital you have given tacit agreement to general treatment which may be required for your condition. Certain procedures eg. operations, administration of anaesthesia and certain diagnostic procedures require your specific consent in writing. Before you sign the consent form, the nature of the procedure should be explained to you, by your doctor, and it is important that you understand what is involved. You are entitled to insist on adequate explanation if you do not understand the procedure and its likely effects.

PATIENTS FOR SURGERY

(a) You may be required to attend the Hospital before the operation for examination by the anaesthetist. (Please bring a fresh urine sample to the clinic on that day).

(b) If you are a smoker you are requested NOT TO SMOKE for two days before admission.

(c) I f a blood test, x-ray or E.C.G. has been requested please

ensure that this has been taken before your arrival at Hospital.

(d) I f you are to have special tests you may be required to carry

out some preparation before admission to hospital. Please follow the instructions given closely.

NEWS OF BABY'S ARRIVAL

Information will be given to the father or next of kin only. Relatives and friends are requested to contact tha father for information.

Fathers may visit at any time, refer to Page 6 for visiting hours for friends and relatives.

Liaison Sisters visit mothers in the ward and will be pleased to advise location of the Baby Clinic in your area.

GENERAL INFORMATION

You are required to provide your own:

Toilet requisites - washers, soap, comb, toothbrush, toothpaste, talc, deodorant, shaving gear, etc

Storage facilities are limited, so please keep only street clothes and toilet articles. Anything you do not need, particularly valuables, should be left home.

Patients should arrange for transport home on discharge with an attendant if necessary.

PARKING

Public car parks are available for visitors on the Hospital grounds.

There is no provision for you to park your vehicle in Hospital grounds for the period of yonr hospitalisation.

ALCOHOL AND OTHKR DRUGS

Alcohol drinks and other drugs are NOT to be brought into Hospital by either patients or visitors.

SKOK.ING

PATIENTS ARE EXPECTED TO Lnt:IT SMOKING TO AREAS SET ASIDE FOR THAT PURPOSE.

SKOK.ING IN HOSPITAL IS DISCOURAGED.

MEDICINES (TABLETS, MIXTURES, OINTMENT OR OTIIER DRUG PREPARATIONS) Should you have any medicines, tablets or other drugs with you on admission please hand them to the Ward Sister.

GIFTS OF FOOD, DRINKS, SWEETS, ETC

Visitors are asked to consult with the Ward Sister before giving patients any articles of food or drink.

PATIENTS VALUABLES, INCLUDING CASH

You are requested NOT to bring valuables or large amounts of money into Hospital.

If this is unavoidable deposit valuables for safe keeping (before arrival in the ward) with the Hospital Security Section, located in the ground floor foyer.

NO RESPONSIBILITY WILL BE ACCEPTED FOR. VALUABLES OR. CASH RETAINED BY YOU IR THE WARD.

In the interests of patient care (your own or another patient in the area) it may be necessary for staff to request your visitors to leave the Ward.

It is less tiring to patients if visitors are restricted to two at a time. Long visits can be tiring and visitors are encouraged to stay short period only.

Visiting children under 12 years MUST be accompanied by an Adult.

Permission for use must be obtained from the Registered Nurse in Charge of the Ward ••

Small radio sets are permitted if equipped with an ear plug.

Television sets, maxim1nn screen size 46cm, equipped with ear plug are permitted, with restrictions, in some wards.

Before use, television sets must be checked for safety by Hospital Staff.

LEAVING HOSPITAL PH.EKISES

You must remain within the Hospital premises until you are discharged unless special arrangements are made with the Registered Nurse in Charge of the Ward.

If you leave the ward please advise the Registered Nurse in Charge of the Ward.

DISCHARGE

When your doctor has authorised your discharge the Registered Nurse in Charge will assist you to make arrangements for your relatives to be advised of your pending discharge.

It may be necessary for you to have home visits from the Community Health Sister and these will be arranged by your Medical Officer or the Ward Sister before your discharge.

You are requested to attend the Admission/Discharge Office before leaving the Hospital to complete your discharge.
