The
Epstein-Barr virus
hasbeen
linked
with several
malignancies particularly
those occuring
in the head
and neck,
including
nasopharyngeal
carcinoma,
Burkitt's
lymphoma,
Hodgkin's
lymphoma,
and TÀ{K cell
lymphoma
.As
nasopharyngealcarcinoma is common
in
Indonesia,it
raises
the
question whether
the
EBV
might play
arole
in the
tumorigenesis
of
tonsillar lymphoma.
A
clinico-pathologic study was
conducted
to
examine thehistologic
type, immunophenotype
and expressionof
EBV
in
casesof tonsillar lymphoma in
Jakarta.*

AND METHODS
Twenty
cases
of tonsillar lymphoma
selected
for
adequacy
of material
which
were
diagnosed
at
theDepartment
of Anatomic
Pathology,
Faculty
of
Medicine, University
of
Indonesia
during
1995-1997
were
studied.
The
histologic
typing was
doneaccording
to
the
updated
Kiel
classification.2
Immunohistochemical
stains
with
the
pAp
method,using
antibodies
to
cD45 RO
(UCHL)
and cD20
(L26)
wasemployed.
In
siru
hybridization for
EBER,
and immunostaining
for
LMPI
was
done.
Theimmunohistochemistry
and
in
situ
hybridization
examination was
doneby
one
of
the authors (A
S-y
L).
Clinical
data
was
obtained
from the
hospital
medical records
(Dr. Cipto
Mangunkusumo National
Central GeneralHospital).

Tonsillar lymphoma was found equally
in
men
(10 cases) and women(10
cases). Clinicallythe
patientspresented
with:
lumps
in
the
neck,
dysphagia,headache,
nasal
congestion, reduced
smell,
tinnitus
and
diplopia. The duration
of
these symptoms
andsigns were
between
3 weeks and
7 months.
Theinvolvement
of lymphnodes in the neék was
found in
13 cases (65Vo),6
was
found ipsilateral
and7
casesbilateral.
Only in 3
cases
there were
no
necklymphnode
involvement,
while in 4
cases
noinformation
was available.The immunotyping
and expressionof EBV are shown
in Table

DISCUSSION
Data
from
the
files
of
Department
of
Anatomic
Pathology,
Faculty
of
Medicine,
University
of
Indonesia showed
that
cancer
of
the tonsil
was
only
l.OBVoof all
cancers.
Cancerof the tonsil
comprised3.6%o
of
head andneck
cancer,while
head and neck
cancer was 29.9Vo
of all
cancer.
Of
the oral
andoropharyngeal cancers, cancer
of
the-
tonsil
wassecônd in itequen"y,
after palate
cancer.rDespite
theIow frequency
of
tonsil
cancer,it
is interesting to
note thatlymphoma
in
our hospital
was morefrequent
thancarcinoma. The
ratio
of
lymphoma
to
carcinoma
was2 :
1.This
data isnot similar to
reportsin the
Westem
literature,
in
which more than
90
percent
of
tonsil
malignancies
are
squamous
cell
carcinomas.3 This
dissimilarity
poses the question,what is
the reason forthis difference
and
what factors may be
operative
?Because
of
the
prevalence of
EB
virus in
the Indonesiancommunity, we performed
aclinicopatho-logic
study
to
examine
for
possible relationship of
EBV expression
andtonsillar lymphoma.
The lymphoma
was
found mostly
in
the
7'h
decade.The median age was 57.5 years.
This
finding
was alsoreported
by
Shimaet
al.a
Lymphnode involvement in
the neck
was foundin
13out
20 cases (65Vo).

wasdifficult to ascertain
the
clinical
staging
asthere
wasno information
of
other
lymphnodes
in
the chest or
abdomen.
Reports
in
the literature
showed that

lympho_ma,were
found in
stageI
orII
whenfirst
presented.a'sThe
histologic
types
of
the lymphoma were 14
('70Vo)intermediate grade (centrocytic, centroblastic-centrocytic
and centroblastic)
and
6
(30Vo)were the high
grade(immunoblastic and lymphoblastic). Lymphoma of
the
low
gradetype was not observed.
Concurrent
with
reports
from
other
centers,the
majority
of
lymphoma
were
of
the intermediate
grade,a'5'6although
it
hasbeen reported that, among lymphomas
of
the head andneck,
,
lymphomas
of
the
tonsil were
of
the
high
grade.'All
the cases reactedwith
CD20
(L26)
anribody,
indi-cating
B
cell
lineage.
This finding
is in
accordancewith the report
by
Watson and Crockers
who
found
that
20
out
of
2l
patients
with
palatine
tonsil
lymphoma were of
the
B
cell origin. However,
in
8cases
,
positive
reaction was also
seenwith
CD45RO
(UCHL-1)
antibody, although
nor
abundant.
Thereason
for
this is that
they may
beT
cells entrappedin
the B
lymphomas
or
possibly there
was
T
cell
activation
in
these tumors,particularly in high
gradelymphomas,
ascoined
by
Wang et
al.eOut
of
these 8cases
in
our
series,
4
were
of
the high grade
(3immunoblastic and
I
lymphoblastic).
In
this
study,
expressionof
Epstein-Ban virus, either
EBER or
LMPI,
was negativein
all
cases.How
would
this
compare
with
other
reports?
O'
Leary
andKennedy'' in their
study
of
sinonasal and Waldeyer'sring
lymphoma
found
out
that
only 2
out
lOlymphoma
in
Waldeyer's
ring (all
of
the large
cell
type)
were positive
for
EBER, while EBER
wasnegative
for
small
cleaved
cell lymphoma. Chan
etalll
who
investigated
the
strength
of
association
of
EBV with
non-Hodgkin's lymphoma
of
the
upper aerodigestive tract reported that among the2l
casesof
lymphoma
of
tonsil and
back
of
tongue none
wasEBER positive. They
also showed that
in
the normal
mucosaof
nose/nasopharynx andtonsil
only very rare
EBER-positive small
lymphocytes
were found.
This
fin^dingis in
accordancewith
study
doneby Hirao
etalr2
who
concluded
that the
palatine
tonsii plays
animportant
role
in
controlling
EBV
infection
by
the persistenceof EBV-specific cellular immunity
which
efficiently
eliminatesEBV-transformed
cells.Different
finding
was reportedby Cirone
et alr3who
demonstratedby
nestedPCR
technique
that
B
lymphocytes
in
thegerminal centers
of
tonsils
of
EBV
seropositiveindividuals contain
EBV
DNA.
However, study
in
children
with tonsillar lymphoma
done
by Jevon
etEbv in tonsillar non-Hodgkin
lymphoma

alra showed that
EBV is of little
consequencein
thepathogenesis
of
malignant lymphoma.
Using
PCR techniquethey detected
EBV DNA in only
I
caseout
of 20.
In fact,
study done
by Ott et alr5
suggestedthat
among malignant lymphoma
of the
upperaerodigestive
tract,
EBV
is
not
involved
in
lymphomagenesisof B cell
tumors,
but is
associatedwith
angiocentric
TA{K cell
lymphoma.
Thesevaried
reports
of
the
involvement
of
EBV in
tonsillar
lymphoma could be comprehended
if
we refer to
thefindings of
Hardarson
et alr6 who did a
study
of
thepresence
of
EBV
in
many types
of
benign
andmalignant lymphoid
lesions using PCR and a sensitiveISH
method.
They
concluded
that EBV may play
avariety of
roles
in
the development
of
malignant
andnon-malignant lymphoid lesions
as they
found
avariable extent
of
EBV
infection,
from
the
low
number
until
the abundantEBV+
cells.The
inapparent
relationship
of
EBV involvement in
our
tonsillar lymphoma
suggeststhat another
route
of
etio-pathogenetic
explanation needs
to
be
sought.Concurrently,
scientific
explanation
warrants
clarifi-cation
of
the moreprevalent
lymphoma
to
carcinoma
of
the tonsil in Jakarta.SUMMARY AND CONCLUSION
Twenty
casesof
tonsillar non-Hodgkin's
lymphoma
seen
at the
Department
of
Anatomic
Pathology,Faculty
of
Medicine,
University
of
Indonesia, Jakartawas
analysed
clinico-pathologically.
The
peak
ageincidence was the 7'h decade, ttre mate to
female
ratio
was 1:1. Sixty-five percent
of
cases
showed
necklymphnode involvement. The histopathologic types
if
referred
to
the
Working
Formulation
was
iOVointermediate
grade
and
30Va
of
the high
grademalignancy.
All
tonsillar
lymphoma
in our
study
wasofthe B
cell type.
None
of
the cases
of
tonsillar lymphoma
showedEBV
expression,
and there
is
no
explanation
why
lymphoma
of
the
tonsil
is
more prevalent
than carcinomain
