On behalf of the Organizing Committee, I would like to welcome you all to the 4th International Nursing Conference in Bali, Indonesia on September 12 – 14, 2013, which is done by the Faculty of Nursing Universitas Indonesia.

The main theme of the Conference is “Safety for All: Protect Patients, Personnel, and Environment. A Mutlidiscipline Approach” which is addressing how to integrate all of those

aspects in education, clinical research, clinical management and health care practice. The conference will preceded by a half day workshop to discuss the plan of action of some issues related to patient safety within the nation and their own working environment.

We believe that nurses need to get together globally to share our nursing experiences in the different part of the world and prepare ourselves for the future success of nursing.

Decision to give breastfeeding in teenage mothers is a dilemma, the mother must meet the developmental task of adolescence and need to perform the role as parents. This study aim to identify the meaning of teenage mothers’ experience in implementing the early initiation and providing the exclusive breastfeeding. This study use a phenomenological research design. The data are analyzed using thematic content analysis from the interview of 8 participants. The researcher found eleven theme, namely the understanding and responses during the early initiation implementation, the understanding, the problem, the decision, a various attempts, and the teenage mothers’ needs and hopes in providing the exclusive breastfeeding. It is recommended that the teenage mothers need a family and health professionals’ supports to be able to provide the exclusive breastfeeding.

The Effect of Kangaroo Care Health Education on Mothers Skill and

Confidence for Breastfeeding Low Birth Weight Infant

Kangaroo care health education is an attempt to guide parent achieving mother’s role in the baby care. The objective of this research was to determine the influence of kangaroo care health education on mothers skill and confidence to breastfeeding their baby. The study design used a quasi-experimental pre and post-test without control group design. Respondents were 17 mothers with low birth weight (LBW) infant in the Perinatology Sanglah Hospital in Denpasar. In the collection of the data, confidence questionnaires and breastfeeding skills observation sheets were used as instrument tool. The data was analyzed with paired t test. This research found that there were a significant influence on mother’s skill and confidence in breastfeed their baby as the result of kangaroo care health education (skills: p=0.000, α=0.05; confidence: p=0.000, α=0.05). it was recommended that kangaroo care health education is should be implemented in the care of LBW infant.

Maternal Necessity to Overcome Pain During Labor Process

Background: The number of mothers’ deaths (AKI) in Indonesia is still high, ie: 228 per 100.000 living-births. One of the causes is obstructed labor, which is 5%. Obstructed Labor is dealing with the presence of anxiety, stress, and pain. Therefore, it takes a discussion on pain and maternal needs to cope with pain during the labor process.

Objective: to explore the maternal necessity to overcome pain during the labor process

Method: qualitative research with phenomenology approach. 6 participants were chosen by using purposive sampling method. Data analysis was made by using qualitative analysis, based on data analysis steps according to Colaizzi.

Result: Identified six themes that are interconnected with a specific purpose. In the experience of pain, themes emerged, namely physical responses and psychological responses to pain where the response will affect the labor process. On efforts made efforts to address maternal themes emerged independently, the mother trying to cope with how to belly rubbed, pray, moving and breathing. While on the theme of success handlers found the pain somewhat diminished but not disappeared. On the efforts made by provider themes emerged in the form of efforts provider recommended that the mother be patient, seek forgiveness, a deep breath and some participants revealed no action taken by the provider. On maternal expectations for pain problems that arise, there is a theme of professional care services such as rubbing his belly, accompanying mothers, or care about and better understand the condition of the mother as well as a complete equipment.

Suggestion: specific interventions need to be developed to overcome the pain, like development of equipment that can be used to overcome pain by her self.

The Effect of Kangaroo Mother Care on Infant Weight

and Oxygen Saturation

Low birth weight (LBW) infants experienced respiratory and growth problems so they need special treatment such as kangaroo mother care (KMC)methode. The objectives of this study were to investigated effects of KMC on LBW’s weight gain and oxygen saturation. A quasi eksperimental pre and post with control group design was performed in LBW infants. A total of 34 LBW infants who choosen by purposive sampling were divided into intervention group (17 LBW infants received intermittent KMC two hours/day for seven days) and control group ( 17 LBW infants received conventional care incubator). Independent t test was performed to compared mean weigt gain and oxygen saturation between control group and intervention group. Better weight gain (p<0.001) and oxygen saturation (p=0.019) were seen in infants who receiving intermittent KMC. Future study need to be carried out to combine KMC with baby massage.

The coverage exclusive breastfeeding less than 80%. The purpose of this study was to get an overview of experience in motivating members of self-help groups to provide exclusif breastfeeding. It was a qualitative research with phenomenological approach. Data was collected on 8 participants in the health center Ciracas area. Data analysis was performed with the technique Collaizi. The results found twelve themes, there are obtaining information and knowledge, get solutions to problems, improve self-confidence, socialization, emotional support, support awards, informative support, instrument support, cognitive enhancement, given the strengthening of the group, the group can increase knowledge, and operational assistance. Conclusion this model can be implementation in Indonesia and community nurses can develop self-help groups to overcome the discontinuity problem so that early breastfeeding mothers able to overcome the problems encountered during breastfeeding; and this model is needed.

Food-stall on the street is so plentiful in the neighborhood around the school and consumed by school children. The high interest and consumption spending on school age children not accompanied by the provision of healthy snacks. One of the efforts to improve the behavior of the run is through health promotion. One of the right health promotion media used on school children, especially those aged nine to 12 years or who generally sit in classes IV and V of elementary school are the main consument. Children aged nine to 12 years has been able to hold a logical and logic synthesis child begins to develop. This study aimed to determine the effect of health promotion through the medium of comics snacks on snacks behavior grade IV and V in SD Negeri 3 Senganan. This study was pre-experimental studies (one-group pre-post test design without control (one-group). The sample consisted of 43 grade IV and V SD obtained with a total sampling. The data was collected using a questionnaire that had been tested for validity and reliability. All respondents are given three different comic series every week for three weeks to read. The results showed that behavioral changes snacks, where before reading comics snacks, snack behavior with both categories increased by 9.3% to 93% after reading the comic snacks. Snack behaviors were categorized enough of 90.7% decreased to 7% after reading the comic snacks. Based on the Wilcoxon Signed Rank Test, this difference was statistically significant, with a value Asymp.Sig. (2-tailed) 0.000 which means there is the effect of health promotion through the medium of comics snacks on snacks behavior grade IV and V in SD Negeri 3 Senganan.

Child stimulation development services in Campaka District was not available yet. The aims of this research is to know the effect of therapeutic group therapy and token economy to developmental aspects and development of initiative in preschool age children. This research uses quasi-experimental with non equivalent control group. Respondents in this study were 50 pairs of mother and preschool-age children which is taken using consecutive sampling technique. There were 24 pairs who received therapeutic group therapy and token economy and 26 pairs who did not receive therapy. Independent t-test and paired t-test were used for analysis. The result showed developmental aspects and development of initiative in preschool age children was significantly higher in the group receiving therapeutic group therapy and token economy than the group who did not receive therapy (p-value < 0.05). Developmental aspects has a significant relationship to development of initiative in preschool age children. Therapeutic group therapy and token economy can be one therapeutic option to improve develompental aspects and development of initiative in preschool age children.

The Maternal mortality rates in Indonesia is still the highest in the Asian countries. One of the reason is due to prolong labor that is influenced by anxiety and fear of mother during labor. Anxiety and pain is very connected and influence its other. Due to anxiety mother feel more pain and it will disturb the labor process. Increasing catecholamine hormone willdisturb the uterine contraction that will be increase the stimulation of respiratory an decrease the CO2 in mother and fetal. This will make the fetal oxygenation decrease and baby will in danger. This in turn will influence the wellness of mother and fetal. Some researchers have been done in order to decrease the labor pain, one of them is acupressure technique. But in reality health provider very seldom to use this technique, due to short of staff that has to be in the mother bedside during labor time. From study by Heni (2012) all mother during labor feel anxiety about the pain, and the need help to reduce the pain. Mother has to do it by themselves to reduce pain. So with the acupressure technique that will be developed in a self management that call Pain Digital Acupressure, mother will use this instrument by themselves. The aim of this study is to develop the Pain Digital Acupressure (PDA) and will measure its effectiveness to pain management of mother during the latent stage of labor process. The design is pre experimental study as part of the whole study a clinical study experimental study . the whole study consist of three stages : stage one : pre experimental study to develop PDA in the bio technical laboratory, the second stage is a trial study of PDA in the laboratory of deer to measure the effectiveness PDA in labor process, and the last stage will be done in the clinical setting by using PDA in the mothers during labor and measure their pain. This report is report of the first stage that we have develop the PDA and measure the suitable PDA by analysis the PDA with the principles of acupressure. The variables that used in the PDA is Waves type, amplitude, frequencies, and volume. The result shown that the suitable of PDA with acupressure principles are Square wave, frequency 30 Hertz, Amplitude 0.8-1, and volume 2.

The next step will be measured the effectiveness of PDA to the labor process and contraction in the deer during labor in the deer laboratory.

The incidence rate of decubitus in February 2012 at PKU Muhammadiyah Hospital of Bantul stood at 6.25%. This percentage exceeds the minimum standards set by the management of PKU Muhammadiyah Hospital of Bantul (an amount of <5%). This research aimed to discover nurses’ behavior in preventing decubitus ulcer as an effort toward patient safety implementation at PKU Muhammadiyah Hospital of Bantul. This research used quantitative methods in a cross-sectional study design as well as qualitative methods with case study research design. The study sample of 11 people consisted of; a head of the nursing services department, heads of adult wards (5 persons), and associate nurses (5 persons). Based on observations the scores of nurses in preventing decubitus ulcer in the ICU was 11 (84.62%); a score found in the “good” category. Conversely, in the surgical ward "Al-Insan", nurses obtained a score of 5 (38.46%), a score found in the “poor” category. Nurses’ behavior in preventing the occurrence of decubitus ulcer included; mobilizing patients, educating patients’ families, assessing the risk of decubitus ulcer using the Braden scale, identifying patient groups at risk, and implementing the anti decubitus mattress and baby oil. Also, nurses were discovered to be still doing work that was not within their given duties and responsibilities. Not all wards have the adequate facilities to support the efforts in preventing decubitus ulcer. Policies of PKU Muhammadiyah Hospital of Bantul related to injury prevention include decubitus incidence rate which is one quality nursing indicator. Support of hospital leadership such as motivation and facilities are necessary in order to prevent decubitus ulcer.

Purpose: Communication is done by nurses has not performed optimally match the correct standard. Many patient complaints against communication that has not been in accordance with the culture of the people.The purpose of research to identify the communication nurse and patient in accordance with the local culture (culture Priangan).Method: This research is an ethnographic study with participants nurse practitioner working in the inpatient at Immanuel Hospital Bandung were taken by purposive sampling. Data collection was carried out for one year, with in-depth interviews, participant observation, field notes and interview the subjects related to the participants (clients and their families).Results: Research found five themes: 1) nurse requires physical and mental preparation, 2) nurses behave friendly, 3) nurses being concerned about his clients, 4). Ineffective communication, 5) socio-cultural assessment is incomplete. Conclusion: These results constitute the basic data for the conducted training for nurses, as well as recommended training modules to create culturally sensitive nursing care for nurses working in hospitals in the area Priangan.

A Descriptive Study of Social Support and Family Coping for Indonesian Elderly EttyRekawati

The elderly is one of vulnerable group. They have alimited in various functions to do their daily activities so they need the families support. However, not all families are able to meet the basic needs of the elderly. The family support and family coping are necessary to improve the well being of the elderly. The objectives of the study identified kinds of family support to care the elderly people who living in the household and kinds of the family coping strategies to care the elderly in Depok, West Java. The method of the study used cross sectional approach. Data was collected by the proportional cluster sampling. Analysis ofthedatadescribedin the form ofdistributivefrequency.The result of study reported that the elderly people received the family support in various supports which istheinformational support ( 55.6 % ), affirmation support ( 55.1 % ), instrumental support ( 56.3 % ), and emotional support ( 50.4 % ). Most of the family stated that they were not burdened caregiver up to the light load levels ( 89.6 % ). Most families were used the various coping strategies to care of the elderly and the Seeking Spiritual Support coping ( 66.7 % ) was the most family coping strategy to care of the elderly in the family. The discussion of the studyindicatedthelownumberof family supportstilllessadaptivefamilycopingstrategiesandthehighburdenof caring forelderlyrelativeswillaffectthe health ofthe elderly.The conclusion of the study was families as those closest to the elderly still have to be upgraded in caring for the elderly at home, so itis necessary to developanursingmodelthatimprovethe ability ofthefamilyin caring fortheelderlyathome.

The Effect of Topical Bromelain on Wound Healing in Diabetic Rat

Abstract

Background/Aim: The prevalence of diabetes is increasing worldwide and has been forecasted to double in the next 20 yeras. The major increase in morbidity and mortality of diabetes is due to the development of complication, including failure of the wound healing process. Prolonged inflamation reaction in diabetic wounds contribute to wound chronicity. Foot ulcers occur in 25 % of all patients with diabetes and failure of healing eventually leads to infection and amputation. Bromelain is a crude extract from pineaplle (Annanas comosus) plant that contains such as proteinases, peroxidises, phosphatises, and protease inhibitor. These factors might contribute to the result of study both in vitro and in vivo several therapeutic properties including inflamation, dermatological and skin debridement among others. Thus, the topical use of bromelain might influence and promote wound healing. The aim of this study is to investigate the effectivenessof bromelain on diabetic wound healing that is examined on diabetic rat model. Methods: Twenty four Sprague Dawley rats weighing 150 -200 gram were randomly assigned to either the control group or the streptozotocin-induced diabetic group. Full-thickness excision wounds were created on the back of rats, and topical bromelain was applied to treatment groups and normal saline were used to control groups. Topical treatment of standar bromelain (Sigma-Aldrich, #B-5882) and and isolated local bromelain(Pusat Kajian Buah Tropis (PKBT), Bogor Agricultural University) were used in doses 25 % and 50 % respectively. Results:The rate of wound healing in bromelain treatment groups is faster than control rats.

Conclusion: Further investigation is needed for the identification of the molecular mechanism of bromelain on wound healing.

Physical Exercise Effect on Decreasing Level of Glucose on Diabetes

Diabetes mellitus patients, who don’t follow treatment such as regular, good and right physical exercise, can have complications such as gangrene, kidney failure, blindness, impotence and coronary heart disease. The purpose of study was to analyze physical exercise effect on decreasing level of glucose on diabetes mellitus patient in St. Carolus BorromeusKupang hospital. This study set 20 respondents for treatment and control group. Research typewasquasy experiment usednon-randomized control group pre-post test design. Both groups had bad level of glucose category before treatment with averagefor treatment group 221.95 mg/dL and 224.10 mg/dL for control group. After physical exercise 12 respondents of treatment group had good level of glucose category and 8 had moderate categorywith average 137.55 mg/dL and decreasing reached 84.4 mg/dL. Based on paired t test result of this study proves that there is significant effect of physical exercise on decreasing level of glucose on diabetes mellitus patient, because paired t test results indicatet count> t table and α > p. Whereas in control group, 7 respondents had moderate level of glucose category and 13 had bad category with average 214.70 mg/dL and decreasing reached 9.4 mg/dL. Paired t test results indicate t count <t table and p>α, so it isn’t shownsignificant effect. Researcher suggests further researcher identify deeper about gradually changes from physical exercise effect in any giving to diabetes mellituspatients and identify differences each therapy effectivenessand combination therapy effectiveness on diabetes mellitus.

Knowledge and compliance in performing hand hygiene are absolutely required by nurses so that hand hygiene can be performed on every nursing action. Hand hygiene is a major and important factor in preventing the spread of pathogens and antibiotic resistance in hospitals due to the number of infections that can be transmitted through the hands of the health care team. The aim of this study is to identify the knowledge and compliance of nurses in performing hand hygiene. The study design is descriptive quantitative research. The samples for this study are 70 nurses. The mean for the variable of nurses’ knowledge is 13.15 (65.75%), while the mean for compliance of nurses in performing hand hygiene is 40.85 (68.08%). The hospital management team needs to develop policies, plan budgeting and establish sustainable hand hygiene training.

Background:.Medication errors are one of serious problems in patient safety and recognized as a high priority health care issue. The responsibility for the error is often placed on the nurse, as she or he is the last person who administers the drugs. Medication administration is a fundamental task widely undertaken by nurses, and is underpinned by legal and professional requirements. Doing correctly in medication administration is a nurse highest responsibility.Nurses understanding in theprocesses of medication administration is critical to minimize the medication errors. Objective:To investigate the frequency, and the type of medication errors done by nurse. Method: A descriptive study of 61 nurses, observing in 799 medication administration in adults patients in medical-surgical units at one general hospital. The direct observation technique was used and conducted continously in 3 working shifts (morning, afternoon and night) which formed a gold standard. The samples are collected by consecutive sampling. Results: We detected a total of 241 errors in 799 opportunities for errors (30.2%).Medication administration error mostly founded in this research were dominated by error in timing of administration and error in documentation (30.2% each), and only a small number of error in route administration (0,1%). No errors was founded in doses and types of drugs or in patient identification. Conclusion: The evaluation and improvement of drug administration process are key element in patient safety. Relevance to clinical practice: We assume that the number of errors could be reduced by simple changes of existing procedures and by implementing nurses understanding and disciplin in theprocesses of medication administration. Medications must be given within a ½ hour of the time that is listed on the medication log.

Dengue/Dengue Haemorrhagic Fever is an acute viral infectious disease caused by dengue viruses. The viruses are transmitted to human body by the Aedes mosquito especially Aedesaegypti. Dengue fever is a major public health issue in countries in the South East Asia region. Researches for a vaccine to prevent this disease are being developed. In the absent of a vaccine, therefore, the only way to minimise the impact of a Dengue viruses outbreak is by avoiding mosquitoes. The WHO issued Guidelines to prevent and control the disease. The aim of this study is to evaluate the effectiveness of the WHO South-EastAsiaRegional Guidelines on Dengue/Dengue Fever Haemorrhagic Prevention and Control, 1999. The study will evaluate the effectiveness of environmental management methods, biological control methods, chemical control methods and how well the Guidelines are being communicated. The dissertation is a development evaluation which focuses on a literature review. Twenty eight publications related to dengue prevention and twelve sources related to the policies of five countries in the South-East Asia region were researched. The study reveals that, in general, dengue prevention methods have been not effective and were not well communicated. The evaluation also suggests that dengue prevention programmes in some countries of the South East Asia region are insufficiently utilizing scientific evidence. Some dengue prevention programmes lack intersectoral coordination, the evaluation of programmes seems to be patchy, some lack proper assessment, some provide incomplete information about chemical methods and health staff and volunteers have only limited knowledge to deliver information to the public in the fight against dengue fever.

Abstract

Infection is still one of the main causes of mortality and morbidity include maternal mortality and newborn in Indonesia. The purpose of this paper is to describe the implementation of patient safety programs related to the Prevention and Control of Infection in unit midwifery general hospital Bekasi. Form of this program is the development program of Prevention and Control of Infection for the people who intended to change public attitudes in maintaining their own health and the prevention of infection more extends in the community. The method used is to use a problem solving approach that starts from the assessment related with Prevention and Control of Infection, both to health workers, patients and families, providing information such as the training for health workers, manufacture of standard operational procedures, audio-visual media and modules, as a learning media as well as the provision of education by health workers to patients and families associated with the Prevention and Control of Infection. The program is expected to help reduce the length of stay of patients in hospitals and dissemination of knowledge or information about the prevention of infections they get while in hospital for other people.

Pressure ulcers are becoming a major health problem as a result of increasing severity of illness, lack of support systems at home, and shortage of nurses.Pressure ulcers prevention has been known as a fundamental aspect of nursing care. The problem of pressure ulcers belongs to no one of healthcare professionals, because pressure ulcers were not just a nursing problem but a multidisciplinary problem. All of the healthcare team must work together to diminish the incidence and severity of pressure ulcers.

Thisstudy was focused on exploring the nurses’ knowledge regarding to pressure ulcer prevention at inpatient wards of Mitra Keluarga Kelapa Gading Hospital Jakarta. This research used a correlational-descriptive design with cross sectional approach. The subjects were 112 nurses who worked at inpatient wards. We found that nurses had apoor level of knowledge aboutpressure ulcer prevention (M = 68,03; SD = 8,41). Further analysis showed that there were significant relationships between nurses’ knowledge of pressure ulcer prevention to nursing educational status (p = 0,007), clinical competencies in pressure ulcer (p = 0,030) and exposure time to information sources about pressure ulcer (p = 0,010). Our recommendation for nurses is to improve their knowledge to reduce the incidence of pressure ulcers.

The Effect of Passive Leg Raising towards Hemodnamics on Patient with

One of the conditions needing immediate response in the emergency ward is hypovolemic shock. The patient in shock requires close monitoring for clinical signs and hemodynamic status and intravascular status. The circulatory assistance and medication provision on the patient are given based on the accuracy of the intravascular volume status of the patient. Fluid responsiveness can be considered as basis for considering administering fluid onlyor inotropic and vasopressor medication for the patient to maintain circulatory homeostatis. Passive Leg Raising (PLR) is a reversible method to evaluate fluid responsiveness.The study aims at evaluating the effect of PLR on hemodynamic. It employs the quasi-experimental design with the within subject repeated measurement design approach. Twenty–four respondents took part as samples, with the consecutive sampling method implemented. The samples were then categorized into responsive and non-responsive groups based on the increase of pulse pressure amounting to or beyond 9% during PLR implementation. The data was tested using the Wilcoxon test. The results indicated that 17 respondents belonged to the responsive group while 7 respondents were categorized as non-responsive. There was a significant relationship between PLR and hemodynamic parameters of systolic blood pressure, diastolic blood pressure, mean arterial pressure, heart rate and pulse pressure (p>0.05). It implied that PLR may be used as an evaluation method for fluid responsiveness on patients with hypovolemic shock. Further investigation is needed for another type of shock

Coronary Artery Disease (CAD) is the number one killer disease in the world included Indonesia. The disease has impact on all aspect of patients and family life, if the heart attack relapses to become worst eventually results to the death. The relapse could be related to the insaficient knowledge of prevention and monitoring of the condition, we put why health education and suitable life style should be part post discharge preparation. Health education needed to improve their quality of life. The number of patient with CAD in High Care Unit Immanuel Hospital placing first position and three months earlier in 2010 there are 60 patients, which is 3.85% has relapse within 3-20 days period.The aim of this study is to identifing quality of life Immanuel Hospital’s CAD patient. The designed of the research was descripe and 58 of respondents participated in this study, through purposive sampling technique. Statistic analysis is median, if the statistic result <= median then the quality of life is in bad category, otherwise it’s in good category.The result showed that there is a quality of life bad category 33 (56,9%) and good category 25(43.1%), where domain quality of life: physical domain bad 38 (65,5%) and good 20 (34,5%) respondent, psychological domain bad 40(69%) and good 18(31%) respondent, social domain bad 30 (51,7%) and good 28(48,3%) responden, enviromental domain bad 34(58,6%) and good 24 (41,4%) respondent.In conclusion, quality of life in CAD patient more than half of the bad quality of life.Health professionals have to provide the health education to improving the quality of life in patient CAD.

An Analysis of the Triage System in Emergency Departments in Indonesia

Triage is a process to assess and sort patients presenting to Emergency Departments (EDs) and prioritize care requirements based on clinical urgency in a timely manner. The rapid development of triage systems in some countries, in order to respond to the demand for emergency services by growing populations and emergency health needs, does not appear to have been matched in Indonesian hospitals. There may be many systems in use and this is likely to impact on both patient care and workforce issues. No published research on triage practices in Indonesia has been identified. Therefore, this study aims to capture and analyze the views of those who work with the triage systems in EDs in Indonesia from a range of different perspectives. The research design of this study was descriptive qualitative using semi-structured interviews of twelve policy makers or persons responsible from five different organizations which informed triage practice in Indonesia. Participants reported that the most broadly used triage system in Indonesia was a four-level triage system according to the decree of the Health Minister in 1992. However, this system had never been evaluated. They also described that there was inadequate implementation of the Indonesian triage system even though some benefits from the current triage system were reported. Triage doctors were reported as the main trieurs in Indonesian triage practice though there was a good precedent to use triage nurses in the future. Four steps of triage process ranging from receiving to prioritizing were reported as the triaging procedures in Indonesia which were almost similar to the international literature except for a re-triage step. In addition, participants explained to the difficulties associated with the qualifications and roles of trieurs. Finally, bed access block, which increased ED workload and decreased the quality of emergency care service including triage was revealed as a significant factor in the patient flow. Some recommendations were made for the stakeholders. First, evaluation of the current triage system should be carried out before implementing new triage systems. Second, a new health policy in triage practice with a national approach and setting the guideline, standards, and quality indicators for evaluation should be carried out by The Ministry of Health. Third, the roles of nurses in the triage practice and in the policy making positions should be developed by including these roles in the curriculum of nursing education. Last, the accuracy of triage decisions should be improved by endorsing some qualifications for trieurs and preparing emergency practitioners for the role of trieurs through triage education.

Implementation ofdischargeplanningprogramaims topreparepatients andfamiliesanticipating problemsin thepost-hospitalization, as well asefforts toovercome them.This study aimedtoanalyze theeffect ofdischargeplanning program for CAD patientscopingmechanisms in theHighCare UnitImmanuelHospital.The study used quasiexperimentaldesign withnon-equivalent controlgroup, which66respondentsobtained throughsystematic randomsampling withconsecutiveadmissionsapproach. Moreover it is dividedin 33respondent’streatment groupand 33controlgrouprespondents.It is found that generally most of CAD patient coping mechanism are adaptive. In the early discharge planning program (71, 2%) and post program (83,4%). There was a difference coping mechanisms before and after given discharge planning in control group (p =0.00). Meanwhile the same result also show in the treated group with (p =0.00). Based on the average interval measurement before and after intervention, the treated group significantly results higher than control group. It can be concludedthat the designed dischargeplanning ismore effective inimprovingCAD patient’s coping mechanisms compare to the original hospital discharge. Moreover it is recommended to use the designed discharge planning as guidelinesto improvethe standards operating procedures ofhospitaldischargeplanning.

Daily Life and Mental Health of the Survivors in the Relocation Area

after Mt. Merapi Eruption in Indonesia

Purpose:The explosion of Mt. Merapiin 2010 had claimed 277 deathsand 2,083 relocated families. Previous studies revealed that the survivors of natural disastersexperiencednot onlyfamily members' death, injury, house damage, and loss of livelihoods, but also some emotional problems. Thedisaster impact of the survivors’ mental health is even greater for ones who are relocated.This preliminary study elucidates the survivors’ daily life and mental health after Mt. Merapi eruption in Yogyakarta Indonesia.

Method:The study was conducted in threedifferent relocation areas of the most damaged village. Three community leaders and seven residents (the eruption survivors) participate in the study. The daily life including daily life stress and coping in daily life, and mental health of the survivors were surveyed using interview questionnaires. In addition, the effects of the eruption were also investigated by interviewing the district and local government officers.

Result:The result indicates that almost 2.5 years after the eruption the survivors who are relocated experience change in daily life including the change in eating habit, sleeping habit, and participation in social and daily activities. The survivors also experience change in housing condition and social network. The questionnaire of mental health reveals that the survivors still have the symptoms of emotional distress. During the interview the survivors also reported change of appetite, anxiety, and sleep difficulty during rainy and windy.

Conclusion:This study reveals that relocated survivors experience changes in daily life and suffer from mental health problems. The changes in the survivors’ daily life may have influence on higher number of mental health problems of the relocated survivors. Further study including an extensive number of samples is still needed to examine the influence between the change of survivors’ daily life in the relocation area after the disaster and the survivors’ mental health.

Women’s Experience During Labor And Birth At Home Attended By Health Professionals (A Phenomenology Study At Singosari Public Health Center, Malang

Events of labor and birth are normal and healthy events in the life of a family. Home birth assisted by health personnel can be chosen by women. Qualitative researches with phenomenological method to explore the women’s experience of labor and birth at home attended by health professionals are rarely found. Participants were selected by specific criteria with purposive method. Seven participants, women who have experienced the process of labor and birth at home attended by health personnel at Singosari Public Health Center Malang Regency, were participated in this study. Data were collected by conducting twice in-depth interviews equipped by field notes. The interviews were recorded and then the transcript was made. The results revealed the phenomenon of the mothers’ experiences during childbirth at home assisted by health professionals, included the meaning of giving birth to a woman, the reasons of choosing home as a place to give birth attended by health personnel, and the support given by health professionals during childbirth that was needed by every mother, and the mother's expectations to health workers as the birth attendants. The results provide useful information to determine a policy in creating a program design or nursing interventions in order to help providing the health needs of women giving birth at home.

The incidence of malaria infection during pregnancy can lead to complications in the mother and fetus. These complications can be prevented if pregnant women are able to overcome clinical symptoms of malaria attack. This study aimed to investigate the influence of "Healthy Mothers Package" on knowledge and behavior during an attack of malaria among pregnant women in Southwest Sumba. The study design was a quasi-experiment with a post-test only and control group. Quota sampling technique was used to obtain 62 respondents. Intervention group was provided with health education, followed by practice and home visits. Data collected were analyzed using Chi square tests. Results indicated that knowledge and behavior were significant different in the intervention group compared with the control group (p= 0,001 and p=0,027; α=0,05). It is recommended that the "Healthy Mothers Package" can be used to guide pregnant mothers overcoming clinical symptoms during malaria attack.
