Bi-articular Muscle
These muscles generally cross two joints and influence movement at both.
A direct head and a reflected head form its proximal attachments. The direct
attachment arises from the anterior inferior iliac spine (AIIS) of the pelvis, while the reflected attachment emerges from the rim of the acetabulum and the fibrous capsule of the hip joint. The two proximal attachments quickly blend into a common belly that coalesces into the quadriceps tendon and, in conjunction with the vasti muscles, then inserts on the tibial tuberosity

Narasi Soal
Seorang ibu rumah tangga berusia 55 tahun dengan indeks massa tubuh 32kg/m2 mengeluh nyeri lutut kanan. Nyeri dirasakan sejak dua tahun yang lalu, namun hilang timbul. Nyeri timbul apabila pasien berjalan jauh, berdiri lama, atau sering naik turun tangga. Apabila merasakan nyeri, ia berobat ke dokter praktek di dekat rumahnya, diberikan obat penghilang rasa nyeri, dan
nyeri menghilang. Namun 1 bulan ini nyeri yang dirasakan lebih berat, dan tidak menghilang walaupun minum obat anti nyeri. Pada pemeriksaan fisik ekstremitas inferior dekstra tidak menunjukkan adanya gangguan neurologis.
Status lokalis genu dekstra didapatkan tanda-tanda inflamasi (merah, eodem, hangat), dan krepitasi. Pemeriksaan radiologis right knee joint menunjukkan
adanya osteofit dan penyempitan celah sendi.

Painful shoulder with restricted glenohumeral motion.
Contracture of the shoulder joint.
Unclear etiology may be autoimmune, trauma, inflammatory.
More common in women over the age of 40 years.Stages
Painful stage: Progressive vague pain lasting roughly 8 months.
Stiffening stage: Decreasing ROM lasting roughly 8 months.
Thawing stage: Increasing ROM with decrease of shoulder pain.Pathology
Synovial tissue of the capsule and bursa become adherent.Treatment
Rehabilitation–– Restoring PROM and AROM.

–– Corticosteroid injection: Subacromial and glenohumeral will decrease pain to maximize therapy.

Surgical–– Manipulation under anesthesia may be indicated if there is no substantial progress after 12 weeks of conservative treatment.

Ny. A, seorang guru berusia 39 tahun mengeluh nyeri dan kesulitan menggerakkan bahu kanan. Nyeri yang dirasakan pertama kali terjadi pada bahu bagian anterior
sejak 5 bulan yang lalu setelah mengangkat tangannya secara berlebihan karena hendak menulis di bagian papan tulis yang terlampau tinggi. Sejak saat itu, ia mulai
merasakan nyeri jika melakukan gerakan fleksi dan abduksi pada bahu sehingga mulai membatasi gerakan bahunya. Saat ini nyeri terus menerus terjadi walaupun

Narasi Soal
LK, 41 th mengeluh nyeri punggung bawah kanan yang menjalar hingga ke tungkai kanan. Nyeri memburuk apabila pasien melakukan aktivitas dengan membungkuk.
Pada pemeriksaan fisik, didapatkan nyeri tekan pada regio lumbosacral kanan dan nyeri sepanjang tungkai posterior kanan. Terdapat penurunan rasa raba pada area di sekitar malleolus lateral, dan penurunan refleks fisiologis pada achilles tendon
refleks tungkai kanan. Tes provokasi menunjukkan hasil postif pada lasseque, bragard pada tungkai kanan. Pada pemeriksaan radiologis didapatkan adanya penyempitan foramen interveterbralis pada segmen L5 - S1 akibat bulging discus.
Tidak ada riwayat jatuh sebelum kejadian, namun terdapat riwayat pekerjaan mengangkat berat (karung beras 20-25 kg) setiap hari selama kurang lebih 10
tahun terakhir BAB dan BAK dalam batas normal.

FRACTURE
DEFINISI
Fraktur adalah :
Hilangnya kontinuitas tulang, tulang rawan sendi,
tulang rawan epifisis baik yang bersifat total maupun
parsial.
Penyembuhan Fraktur 1. Fase hematoma
2. Fase proliferatif
3. Fase pembentukan callus
4. Fase konsolidasi
5. Fase remodeling
FASE HEMATOMA
Fraktur biasanya disertai putusnya pembuluh darah kecil yg melewati kanalikuli dalam sistem haversian
sehingga terdapat penimbunan darah di sekitar fraktur yang menyebabkan hematoma dan pembengkakan jaringan lunak.
Pada ujung tulang yang patah terjadi iskemia sampai beberapa milimeter dari garis patahan yang
mengakibatkan matinya osteosit pada daerah fraktur tersebut.
Stadium ini berlangsung 24 – 48 jam.
FASE PROLIFERATIF
Proliferasi dan differensiasi sel-sel periosteal dan endoosteal menjadi fibro kartilago yang berasal dari periosteum, endosteum,dan bone marrow yang telah mengalami trauma.
Hematom akan terdesak oleh proliferasi ini dan diabsorbsi oleh tubuh.
Osteoblast beregenerasi, aktivitas sel-sel sub periosteal, aktifitas sel-sel dari kanalis medularis pada lapisan endosteum dan dari bone marrow masing- masing fragmen → proses osteogenesis.
Dalam beberapa hari terbentuklah tulang baru yang menggabungkan kedua fragmen tulang yang patah, terjadi pengendapan kalsium.
Fase ini berlangsung selama 8 jam setelah fraktur sampai selesai, tergantung frakturnya.FASE PEMBENTUKAN CALLUS
Terbentuk fibrous callus, tulang menjadi osteoporotik akibat resorbsi kalsium untuk penyembuhan.
Sel–sel yang berkembang memiliki potensi yang kondrogenik dan osteogenik mulai membentuk tulang dan juga kartilago.
Kegiatan osteoblast dan osteoklast mulai mengabsorbsi sel tulang yang mati.Osteoblas mengeluarkan matriks intra selluler (kolagen dan polisakarida) bersatu dengan garam kalsium, membentuk tulang immature atau young callus.
Pada akhir stadium akan terdapat dua macam callus:◦ ◦
didalam disebut internal callusdiluar disebut external callus
Sementara tulang yang imatur (anyaman tulang ) menjadi lebih padat sehingga gerakan pada tempat fraktur berkurang pada 4 minggu setelah fraktur menyatu.FASE KONSOLIDASI
Callus mengalami maturisasi lebih lanjut oleh aktivitas osteoblas, membentuk lamela-lamela.
Terjadi pergantian fibrous callus menjadi primary callus.
Secara berangsur-angsur primary bone callus diresorbsi dan diganti dengan second bone callus yang sudah mirip dengan jaringan tulang yang normal.
Ini adalah proses yang lambat dan mungkin perlu beberapa bulan sebelum tulang kuat untuk membawa beban yang normal.
Fase ini terjadi sesudah empat minggu, namun pada umur-umur lebih mudah lebih cepat.
Umumnya terjadi: 120 hari (3 bulan)FASE REMODELLING
Secondary bone callus sudah ditimbuni dengan kalsium yang banyak dan tulang sudah terbentuk dengan baik, serta terjadi pembentukan kembali dari medula tulang.
Tulang baru yang terbentuk pada umumnya berlebihan
Dengan mengikuti stress/tekanan dan tarik mekanis, (gerakan, kontraksi otot) callus yang sudah mature secara pelan-pelan terhisap kembalidengan kecepatan yang konstan sehingga terbentuk tulang yang sesuai dengan aslinya.

Radial Nerve Palsy
originates in the brachial plexus and has nerve roots from C5 to T1.
It crosses through the spiral groove on the posterior side of the humerus shaft. The spiral groove is located about 14 cm proximal to the lateral epicondyle and 20 cm proximal to the medial epicondyle.
PE: wrist drop (loss or weakness of wrist extensors), loss or weakness of finger extension, and decreased or absent sensation to the posterior forearm, digits 1 to 3, and the radial half of the fourth digit.Other Fractures of Humerus

Bones
The most distinct palpable bony landmarks are the epicondyles (medial and lateral)
The medial epicondyle serves as the proximal attachment site for a primary forearm pronator (pronator teres), for a major stabilizing ligament (the ulnarcollateral ligament), and for most of the wrist andfinger flexor muscles.
The lateral epicondyle : attachment for many of the wrist and finger/thumb extensors and the forearm supinator.
Lateral supracondylar ridge, a landmark that is palpable between the lateral head of the triceps posteriorly and the brachioradialis muscle anteriorly.Common Elbow Pathologies
Lateral epicondylitis, also know as tennis elbow, is a verycommon overuse condition of the common extensor tendon where it inserts into the lateral epicondyle of the humerus.
Medial epicondylitis, also know as golfer’s elbow, is aninflammation of the common flexor tendon that inserts into the medial epicondyle.
Volkmann’s ischemic contracture is the result of ischemic necrosis of the forearm muscles caused by trauma to the brachial artery.102

ACL injury
Patients typically develop a rapidly developing effusion (hemarthrosis) and ‘‘pop’’ sign, usually during a pivot motion. The most common tests are the Lachman, anterior drawer sign, and pivot shift test
Lachman test: Patient lies supine. Knee is flexed approximately 20 degrees and the proximal tibia is pulled forward to assess excessive translation (more than 5 mm).
Anterior drawer test: Patient lies supine. Knee is flexed at 90 degrees and the hip is flexed at 45 degrees. The proximal tibia is pulled anteriorly to assess excessive translation (more than 5 mm).
Pivot shift test: Patient lies supine. Knee is placed in extension. The examiner supports the leg by the upper tibia and flexes the knee while applying a slight valgus stress to the knee (pushing the knee toward the midline) and internal rotation stress about the femur. In a knee with an ACL injury, the femur sags backward on the tibia (or conversely, the tibia moves forward on the femur), creating a subluxation of the lateral tibiofemoral compartment.109
Ligaments of the Knee
Anterior cruciate ligament (ACL): Prevents anterior displacement of the tibia over the femur and provides rotational (torsional) stability
Posterior cruciate ligament (PCL): Prevents the femur from sliding forward on the tibia (or the tibia from sliding backward on the femur)
Medial collateral ligament (MCL): Connects the femur to the tibia and stabilizes the medial side of the knee
Lateral collateral ligament (LCL): Connects the femur to the fibula and stabilizes the lateral side of the knee111
