Medical Leave Letter
sample
Subject: Medical leave of Absence Dear Mr. John
This letter is to request a leave of absence for medical reasons officially. I will be having an appendix operation on 16/07/2022 and will not work for 45 days. I plan to back to work on (DATE) if the healing goes well.
I have integrated a letter from my doctor that states the want for operation and the operation date. If any other medical records are necessary, I will be happy to provide them. Mrs. Wonda in my department will take liability for my clients, and I will be available by phone to help. I can be reached at (email id) or at (phone number).
I greatly value your kind deliberation in granting me this leave.
Sincerely,
Jina Joy
Subject: Medical Leave application with Prescription Dear Mr. John
I am writing the letter to request you to hold my unbalanced medical leave as an outcome (diagnosed disorder) as this includes a complete break owing to my absence. Because my surgeon has issued me with a medical certificate, I have sealed it.
Please let me know the subsequent steps to forward this file for leave. Your help will be very much respected.
Sincerely,
Jina Joy
