Abstract
Clinics Sidorejo Lor - Salatiga as one of the institutions of health, not optimal in the use of technology to complete its work. One of the problems faced by the medical report recording still using the conventional system is by the book. In terms of recording medical reports required accuracy for many repetitions of writing which resulted in wastage of time when the works so that the work that was done to be less effective. This is because due to lack of fees provided by the health department Salatiga. This study was conducted to produce the laboratory information system in order to solve existing problems. This system created using the technology PHP5 and MySQL as the database. The system provides information such as patient data, physician data, the data lab, employee data and inventory data.

Puskesmas Sidorejo Lor – Salatiga sebagai salah satu lembaga kesehatan, belum
teknologi PHP5 dan MYSQL sebagai databasenya. Sistem ini menyediakan informasi seperti data pasien, data dokter, data praktikum, data pegawai dan data inventaris.
