PENUGASAN KLINIS DAN RINCIAN KEWENANGAN KLINIS dr. GINANJAR BUDHI PRATHAMA, M.Kes, Sp.OT

DIREKTUR RUMAH SAKIT UMUM DAERAH KABUPATEN PACITAN

Menimbang : Dalam upaya memberikan pelayanan dan pengobatan baik dari aspek manajerial, medik, psikologis maupun sosial dan untuk mewujudkan pelayanan yang optimal, maka perlu menetapkan Penugasan Klinis dan Rincian Kewenangan Klinis dr. Ginanjar Budhi Prathama, M.Kes, Sp.OT., dalam suatu keputusan.

Mengingat : 1. Undang-undang Nomor 29 Tahun 2009 tentang Praktik Kedokteran;

2. Undang-undang Nomor 36 Tahun 2009 tentang Kesehatan;

3. Undang-undang Nomor 44 Tahun 2009 tentang Rumah Sakit;

4. Peraturan Menteri Kesehatan Nomor

755/MENKES/PER/IV/2011 tentang Penyelenggaraan Komite Medik di Rumah Sakit;

5. Peraturan Direktur Rumah Sakit Umum Daerah Kabupaten Pacitan Nomor 3 Tahun 2013 tentang Peraturan Internal Rumah Sakit (Hospital By Laws) Rumah Sakit Umum Daerah Kabupaten Pacitan;

6. Peraturan Direktur Rumah Sakit Umum Daerah Kabupaten Pacitan Nomor 2 Tahun 2015 tentang Peraturan Internal Staf Medis (Medical Staf By Laws) Rumah Sakit Umum Daerah Kabupaten Pacitan.

MEMUTUSKAN,

Menetapkan,

KESATU : Penugasan Klinis dan Rincian Kewenangan Klinis dr. Ginanjar Budhi Prathama, M.Kes, Sp.OT.

KEDUA : Penugasan Klinis dan Rincian Kewenangan Klinis sebagaimana dimaksud dalam diktum “KESATU” Keputusan ini sebagaimana tercantum dalam Lampiran Keputusan ini. KETIGA : Rincian Kewenangan Klinis dapat dikurangi atau ditambah

atas rekomendasi Komite Medis cq Sub Komite Kredensial.
