ON THE JOB TRAINING AGREEMENT

1. The Philippine Statistics Authority-Quezon Lucena City will accommodate MARY GRACE R. MANUEL to undergo On the Job Training Program.

2. The training program is designed to run for 250 hours but maybe extended upon the agreement of both parties. The following schedules will be implemented as their only time to report on their OJT Program.

MONDAY

3. The status of the student while on the training shall be that of student trainee and not employer-employee relationship.

4. The student while on training shall progress from one job to another in order to gain experience in all operations and duties.

5. The training body may provide the student trainee such allowance and other benefits as they deem it if and necessary.

6. The training body agrees to make a report to the school, practicum professor during his / her visit to the place where the student is assigned. The supervisor should evaluate the trainee at the end of the training period.

7. The student-trainee agrees further to observed the rules and regulations of the above training body and abide with all implied stated terms and conditions as stipulated in the ON THE JOB TRAINING AGREEMENT.

I, PRINCESS DIANE M. PANCHO, of legal age residing at Purok Bagong Masagana Cotta Lucena City applied for ON THE JOB TRAINING/PRACTICUM course through their request, MARYHILL COLLEGE - COLLEGE OF BUSINESS ADMINISTRATION and in consideration therefore, I hereby FREELY and VOLUNTARILY assume and impose upon myself the following duties.

That I recognize the authority of my cooperating body/company which I may be placed and submit myself to all RULES and REGULATIONS that may be imposing upon me the following duties:

That the WAIVER and ON THE JOB TRAINING AGREEMENT will be in effect for the whole duration of the training.

That I RENOUNCE and WAIVE any claim against the cooperating body/company and MARYHILL COLLEGE – BUSINESS ADMINISTRATION for any injury that I may sustain /loss that I suffer, personal pecuniary in the performance of my duties/functions.
