Petunjuk

Berikut ini terdapat sejumlah pernyataan mengenai kinerja dalam kurun waktu satu tahun terakhir.

Bapak/Ibu dipersilahkan memilih satu dari beberapa pilihan dengan memberi tanda silang (X) pada tempat

yang tersedia. Bapak/Ibu bebas menentukan pilihan yang sesuai dengan sub bagian yang Bapak/Ibu pimpin.

Untuk membantu Bapak/Ibu dalam mempertimbangkan jawaban, berikut ini diberikan suatu perkiraan

persentase tentang isi setiap butir pernyataan.

Bapak/Ibu menandai “kolom 1 (satu)” bila kinerja antara 0% s.d 20%, dan ini berarti kinerja

sub bagian “Sangat tidak baik”. “kolom 2 (dua)” bila kinerja antara 21% s.d 40%, hal ini berarti kinerja sub

bagian “Tidak baik”. “kolom 3 (tiga)” bila kinerja antara 41% s.d 60%, hal ini berarti kinerja sub bagian

“Kurang baik”. “kolom 4 (empat) bila kinerja antara 61% s.d 80%, hal ini berarti kinerja sub bagian “Baik”.

“kolom 5 (lima)” bila kinerja antara 81% s.d 100%, hal ini berarti kinerja sub bagian “Sangat baik”.
