5 ABSTRAK
Perkembangan sistem informasi mendorong manusia untuk menciptakan berbagai cara untuk mempermudah menemukan informasi. Sistem informasi dapat dirancang salah satunya adalah dalam bentuk basis web disertai pemetaan. Sistem informasi tentang Pemetaan Fasilitas Kesehatan Berbasis Web masih belum dapat ditemukan di Kota Medan.
Terdapat dua tujuan dibuatnya Pemetaan Fasilitas Kesehatan di Kota Medan Berbasis Web. Pertama, untuk menciptakan sistem informasi yang terintegrasi. Sistem informasi ini membantu dinas kesehatan untuk mengolah dan menyimpan data fasilitas kesehatan di Kota Medan. Kedua, untuk memberikan informasi fasilitas kesehatan bagi masyarakat.
Metode yang digunakan dalam merancang Pemetaan Fasilitas Kesehatan Berbasis Web ini adalah System Development Life Cycle (SDLC). Metode ini dibagi menjadi empat tahap yang terdiri atas tahap analisis sistem, tahap perancangan sistem, tahap uji coba, dan tahap implementasi.
Hasil perancangan ini dapat dilihat dalam tampilan website, yang terdiri atas informasi mengenai fasilitas kesehatan dalam bentuk tabel disertai gambar peta yang menunjukkan lokasi keberadaan fasilitas kesehatan. Untuk mengakses website, dapat dilihat pada aplikasi browser seperti Mozila Firefox, Google Chrome, atau browser lainnya pada media elektronik yang terhubung dengan jaringan internet. Website dapat diakses pada alamat www.faskesmedan.eu5.org.
Dengan adanya website ini, dapat mempermudah masyarakat untuk menemukan informasi dan lokasi fasilitas kesehatan di Kota Medan. Diharapkan website ini dapat memberikan manfaat sebagai sumber informasi dalam perkembangan sistem informasi pelayanan kesehatan masyarakat.

6 ABSTRACT
The information system development encourages people to invent various ways easily to find information. The information system can be designed, one of them is as a web based information accompanied by mapping. Information system about Web Based Health Facilities Mapping in Medan still could not be found in Medan City.
There are two purposes of Web Based Health Facilities Mapping in Medan City. First, to create an integrated information system. This information system helps health office to manage and store data of health facilities in Medan City. Second, to give the information of health facilities for public.
System Development Life Cycle (SDLC) is the method used to design Web Based Health Facilities Mapping. This method divided into four stages included system analysis, system design, trial test, and implementation stage.
The result of designing process can be access in web appearance, which consist of information about health facilities in form of tables accompanied by a map showing the location of health facilities. To access the website, it can be viewed in a browser application such as Mozilla Firefox, Google Chrome, or another browser electronic in electronic media connected to the internet network. The website can be accessed at the address www.faskesmedan.eu5.org.
With this website, the public can find the information and location of health facilities in Medan City easily. It is hoped that this website can give a benefit as a source of information in the development of public health service information system.
