Kuesioner ini merupakan model kuesioner terbuka karena jawaban seluruhnya berasal dari pihak responden. Data-data hasil kuesioner ini tidak akan disebarluaskan kepada pihak manapun karena hanya digunakan untuk keperluan riset penulis di Universitas Ekasakti Padang.
