第1节-目的Purpose

1.1为规范国际工程项目危险施工作业的管理，防范生产安全事故发生，制定本标准。This standard is developed to standardize the management of hazardous construction work in overseas projects and prevent work safety accidents.

第2节-范围Scope

2.1本标准适用于国际工程施工现场危险施工作业许可的管理。This standard is applicable to the management of permits of hazardous construction work at the construction sites of overseas projects.

第3节-术语Terminology

3.1危险作业是指包括危险性较大或超过一定规模的危险性较大的分部分项工程和可能引发群死群伤的施工作业。

Hazardous work refers to the works involving high risks or the works involving high risks and exceeding the certain scale, as well as other construction activities which may cause mass casualties.

5.4 HSE管理部门负责《危险施工作业许可证》审核，并对实施情况进行监

督。

The HSE Office is responsible for the review of the Permit to Hazardous Construction Work and the supervision of its implementation.

5.5工区（厂、队）负责《危险施工作业许可证》的申请和执行，对作业班组申请的《危险施工作业许可证》进行审查。

The working areas (/plants/teams) are responsible for the application and execution of their Permits to Hazardous Construction Work and the review of the Permits to Hazardous Construction Work applied for by their working groups.

第6节-程序Procedures 6.1作业许可办理范围Scope of Permission

6.1.1危险性较大的分部分项工程（见《国际工程项目安全技术标准》附件

1）。

Works involving high risks (see Annex I the Standard for Safety Techniques in Overseas Projects);

6.1.2超过一定规模的危险性较大的分部分项工程（见《国际工程项目安全

技术标准》附件2）。

6.1.13其他易发生群死群伤的施工作业。Other construction activities likely to result in mass casualties.

6.2作业条件确认Confirmation of Work Conditions

6.2.1项目实施前，项目经理部应根据项目安全风险评估情况，确定项目中

存在的危险作业，并作为现场安全管控重点，做好相关安全技术措施准备工作。

Before the project implementation, the Project Management Department shall, based on the safety risk assessment of the project, identify the hazardous work in the project as the focus of on-site safety control, and make preparations for necessary technical safety actions.

6.2.2危险作业计划施工时，负责该施工作业的工区施工队伍施工技术管理

人员或施工班组组长应于施工前至少7日，向项目经理部施工管理部门提出

《危险施工作业许可证》（附表1）申请。

When the hazardous work is to be implemented, the technical supervisor of the construction teams or the head of the construction group within the working area shall, at least 7 days before construction, file an application for the Permit to Hazardous Construction Work (see Annex 1) to the Construction Office under the Project Management Department.

6.2.3接到《危险施工作业许可证》申请后，生产副经理应同总工程师、安

全总监组织施工、HSE、技术、质量、物资设备等部门及工区施工队伍相关负责人、施工班组组长按照《国际工程项目班组安全标准》制定的《班组安全验证记录表》（见附表2）通过班组安全验证后进行现场作业条件确认并签字。

After receiving the application for the Permit to Hazardous Construction Work, the Deputy Manager in charge of Construction shall, together with Chief Engineer and Chief Safety Officer, organize the heads of the Construction Office, HSE Office, Technical Office, Quality Office, and Equipment and Materials Office, as well as the head of the working team in the working area, after verifying the safety of the team in accordance with Safety Verification Form of Working Team (Annex 2) from the Standard for Safety Management of Working Teams for Overseas Projects confirms the on-site working conditions and signs off on them.

6.2.4现场条件确认应根据危险作业的特点，对有关的危险因素进行检查，

检查应包括但不仅限以下内容：

In the confirmation of site work conditions, hazards inspection shall be carried out according to the characteristics of the specific hazardous work. The inspection items shall include but are not limited to the following:

1）专项施工方案是否按规定编制、审批完成；超过一定规模危险性较大的分部分项工程是否经过专家论证；

Whether the construction method statement has been prepared and approved as required, and whether the works involving high risks and exceeding the certain scale has been confirmed by experts.

2）现场是否按照现行的施工方案施工；Whether the site construction is carried out according to the current construction method statement.

3）现场的施工条件是否与施工方案一致，不一致的是否按规定重新进行了审批或专家论证；

3) Whether the site construction conditions are consistent with those indicated in the construction method statement and whether the construction method statement has been submitted for approval or experts argumentation as required if any deviation is identified.

4）特种设备是否按规定进行了检测，并持有检验合格证书；Whether the special equipment has been properly tested, as demonstrated by conformity

certificates.

5）设备的安全装置是否处于安全状态；Whether the safety devices of the equipment is in safe status.

6）特种作业人员是否持证；Whether the special operation workers have related certificates.

7）作业人员是否进行了安全教育培训、风险告知和安全技术交底；

Whether the workers have received safety training and have been informed of potential risks and necessary safety measures.

8）是否制定了有关的应急预案或现场处置方案；Whether the relevant emergency preparedness plans or site emergency disposal methods have been developed.

9）有关的个人劳动防护用品配备是否齐全；Whether the required personnel protective articles have been provided in a complete way.

10）是否设置了必需的安全防护设施、安全警示标志；Whether necessary safety protection facilities and warning signs have been provided.

11）脚手架钢管、扣件等临时设施是否经过有关方检测合格；Whether the temporary facilities, such as the steel pipes and fasteners of scaffolds, have passed the tests of relevant parties.

12）作业现场有毒有害气体是否进行了检测；Whether the working site has been inspected for toxic and harmful gases.

13）是否配备了满足施工要求的通风设备；Whether ventilation equipment meeting the construction requirements has been provided.

14）易燃易爆等危险品是否按规定储存、运输和管理；Whether the

flammables, explosives, and other hazardous items have been stored, transported and managed as required.

15）不良地质是否经过了监测，是否满足施工条件；Whether the

unfavorable geological conditions have been monitored and whether the construction conditions are met.

16）作业时段天气、水文等情况是否良好。Whether the weather,

hydrological conditions and other ambient conditions are satisfactory during work.

6.2.5项目总工程师、生产副经理和安全总监应当根据检查结果对作业条件

进行复核。总工程师负责有关工程技术质量内容的检查复核，生产副经理负责设备、安全防护设施的检查复核，安全总监负责监督现场条件确认的落实情况。

The Chief Engineer, Deputy Manager in charge of Construction and Chief Safety Officer of the project shall review the work conditions according the inspection results. The Chief Engineer shall be responsible for the review of technical quality;

the Deputy Manager in charge of Construction shall be responsible for the review of equipment and safety protection facilities; and the Chief Safety Officer shall supervise the confirmation of on-site conditions.

6.3施工作业许可Permits to Construction Work

6.3.1现场作业条件确认符合要求后，由生产副经理签发《危险施工作业许

可证》，并向项目经理报告危险作业的准备及实施情况。

After the site work conditions are confirmed satisfactory, the Deputy Manager in charge of Construction shall issue the Permit to Hazardous Construction Work, and report to the Project Manager the preparations and implementation of the hazardous work.

6.3.2未取得《危险施工作业许可证》或现场作业条件确认不符合要求的不

得进行施工。

No construction shall be allowed when the Permit to Hazardous Construction Work has not been obtained or the site work conditions are confirmed unsatisfactory.

6.3.3《危险施工作业许可证》有效期一般为30天。连续施工且工作条件和

周围环境无变化的可根据实际适当延长，但延长的时间最长不超过30天。

The Permit to Hazardous Construction Work is valid generally for 30 days (normally has an effective period of 30 days), which maybe extended when necessary in case the construction work has to be continued without changes in work conditions and surrounding environment. The extension shall not exceed 30 days.

6.3.4危险作业施工时，实行工区施工队伍主要负责人或班组组长带班，并

设专人进行现场监管。

During hazardous construction work, the primarily responsible person of the construction teams or the head of the working group in the working area shall take a leading role, and dedicated personnel shall be assigned to supervise on the site.

6.3.5危险作业施工过程中，出现下列情况的，应及时采取应对措施，并重

新申请和签发《危险施工作业许可证》。

Appropriate countermeasures shall be taken in a timely manner and the Permit to Hazardous Construction Work shall be reapplied for, if any of the following occurs during hazardous construction work:

1）发生险情或未遂事故的；Dangerous incidents or near misses;

2）发生重伤或一般及以上生产安全事故的；Serious injuries or ordinary or above production safety accidents;

3）施工作业条件或施工周围环境变化较大的；Major changes in construction work conditions or surrounding environment;

4）专项施工方案发生改变或施工方案与现场施工条件不一致的；Changes in the Construction Method Statement or inconsistency between the Construction Method Statement and the site construction conditions;

5）设备或特种设备出现故障的；Failure of equipment or special equipment;

6）受台风、地质灾害影响的；Adverse impacts of typhoon or geological hazards; and

7）其他施工条件变化较大的。Major changes in other construction conditions.

6.3.6《危险施工作业许可证》应一式二份，一份留施工作业许可申请人，

一份留项目安监部门备案。

The Permit to Hazardous Construction Work shall be made in duplicate, one for the applicant, one for the Safety Supervision Department of the project for filing.

6.4监督管理Supervision

6.4.1工区（厂、队）及时汇总本单位施工中存在的危险作业，总体掌握危

险作业分布情况，并根据施工生产实际情况，加强各类管理资源配备。

The working areas (/plant/teams) shall summarize the hazardous work in their respective construction activities in a timely manner, master the distribution of the hazardous work, and strength the allocation of various management resources based on reality.

6.4.2危险作业施工时，工区（厂、队）应安排有关专业技术人员进行现场

指导和监督，保证操作规程、施工方案和有关安全技术措施的落实。

arrange related professional personnel to provide guidance and supervision, so that the operating procedures, construction method statement and the required safety actions are put into practice.

6.4.3施工管理部门应会同HSE管理部门督促工区（厂、队）严格执行危险

作业施工许可制度，安排专职安全管理人员进行旁站监督，并定期对落实情况进行监督检查。

The Construction Office shall, together with the HSE Office, urge the working areas (plant/teams) to strictly implement the permit to hazardous construction work system, assign full-time safety officers for on-site supervision, and regularly supervise and inspect the implementation of the system.

第7节-记录Records

项目经理部应完整保存体现危险施工作业许可作业过程的文件、表格、记录、图片、视频等资料，并分类建档管理。记录资料至少应包括危险施工作业许可证等。

The Project Management Department shall keep all the documents, forms, records, pictures, videos and other materials recording the process of Permit to

Hazardous Construction Work, and classify them for filing. The record materials shall at least include the Permits to Hazardous Construction Work.
