* Only the work that is detailed below is covered by this permit.
Review the permit requirements completely prior to commencing work.
* Hanya pekerjaan tertera dibawah ini yang dicakup oleh Surat Izin Bekerja ini. Perhatikan persyaratan surat izin dengan menyeluruh sebelum memulai pekerjaan.
** No employee may enter a confined space (or hazardous atmosphere) without being attached to a lifeline and without another employee in attendance, at the entrance, at all times.
** Tidak boleh ada karyawan yang memasuki ruang tertutup (atau atmosfer yang berbahaya) tanpa menggunakan tali penyelamat dan tanpa kehadiran karyawan lain, pada jalan masuk, sepanjang waktu.
*** Only Section Heads may issue these permits. *** Hanya Kepala Seksi yang boleh mengeluarkan izin ini.

the original of this permit on his person at all times. * Orang yang bertanggung jawab langsung untuk melakukan tugas tersebut harus menyimpan salinan asli surat izin ini setiap saat.
** The person directly responsible for doing the job, must personally lock-out the equipment / machinery involved and must retain the key on his person.
** Orang yang bertanggung jawab langsung untuk melakukan tugas harus tersebut, harus me-lock-out sendiri peralatan/permesinan yang terkait dan menyimpan kunci lock-out bersamanya.
*** Only the person directly responsible for doing the job, may remove the lock-out device. He may only do so following a complete Safety check.
*** Hanya orang yang bertanggung jawab langsung untuk melakukan tugas boleh melepaskan alat lock-out. Ia hanya boleh melakukannya setelah melakukan pemeriksaan Safety yang lengkap.
**** Upon completion of the job the person responsible for doing the job, must personally ensure that the area is left in a clean and tidy condition.
**** Saat tugas tersebut selesai, orang yang bertanggung jawab untuk melakukannya, harus memastikan sendiri bahwa daerah ditinggalkannya dalam keadaan bersih dan rapi.
