PREFACE

Praise our gratitude for the presence of God Almighty, for His bless and grace, so that the implementation of this International Conference can be held with the theme “On Handling Non-Communicable Disease During Covid 19 Pandemic”

which aims to share information about current issues regarding Non-Communicable Disease during the Covid- 19 Pandemic, Facilitating a meeting forum for scientists, researchers, academics and students to exchange experiences and research results on all aspects of handling, and controlling Covid-19 transmission to people at risk and people with non-communicable diseases and facilitating forums to share interventions in dealing with the prevention and control of non-communicable disease during the Covid-19.

The International Conference is expected to be a means in efforts to prevent and control public health problems through research based on science and technology that is built from research and publications activities an the applications of research results in solving health problems in the community, in terms of this is a matter of NCDs prevention an control. The industrial era 4.0 also requires academics, students, and health observers to always update their abilities through research and scientific activities, to contribute to the handling of NCDs, especially in the Covid-19 Pandemic. Our gratitude goes to the Director of Ministry of Health Polytechnic in Semarang, resource persons, paper contributors and all related parties so that the international conference can be held well.

PEER GROUP SUPPORT IN PREVENTION OF TEENAGE GIRL’S ANEMIA

Abstract

Anemia was the most common nutritional problem worldwide which was mainly due to iron deficiency. Teenage girl were proned to suffer from anemia because of a lot of blood lost during menstruation. One of the government's efforts to meet iron intake was the supplementation of blood-supplemented tablets for young women. Health education of Peer group support method will have a more positive effect on compliance with blood-supplemented tablets in preventing anemia. The purpose of this study was to analyze the effectiveness of peer group support in preventing anemia in teenage girl. This type of research was pre-experimental with posttest only with control group design. The study population was 114 students at MTS Maarif 2 Muntilan with a sample of 64 students consisting of 32 students as the intervention sample and 32 students as the control sample. The sampling technique used purposive sampling. Data analysis used the Mann Whitney test. The results showed that most of the students in the intervention group were in the good adherence category, while in the control group there were still 25% of students with low adherence. There was a difference in compliance with the consumption of blood-added tablets in an effort to prevent anemia among adolescent girls in the intervention group and the group without intervention (p value: 0.007). Young women were advised to choose a positive peer group environment by joining peer group education, peer counselors and PIK-R in schools and communities to support the prevention of iron deficiency anemia

1. Introduction

Anemia is the most common nutritional problem around the world, mainly due to iron deficiency (World Health Organization, 2001).

Anemia in girls is higher than boys. Teenage girl are prone to suffer from anemia because of a lot of blood loss during menstruation. Anemia in teenage has a negative impact on decreased immunity, concentration, learning achievement, teenage fitness and productivity. Teenage girl who suffer from anemia are at risk of developing anemia during pregnancy. This will have a negative impact on the growth and development of the fetus in the womb and have the potential to cause complications of pregnancy and childbirth, and even cause the death of mothers and children (Kementerian Kesehatan Republik Indonesia, 2018).

The prevalence of anemia in Indonesia increased by 11.8% from 37.1% in Riskedas 2013 to 48.9% in Riskesdas 2018, with the proportion

of anemia in the 15-24 year age group of 84.6%

and the 25-34 year age group 33.7% (Badan Penelitian dan Pengembangan Kesehatan, 2013), (Badan penelitian dan Pengembangan Kesehatan Kementerian Kesehatan RI, 2018). This data clearly confirms that the rate of anemia among teenage is dominant, namely 84.6%. This shows that the condition of anemia in teenage still requires serious attention, given the huge impact of anemia on the health of teenage girl.

One of the government's efforts to meet iron intake is the supplementation of blood-supplemented tablets for teenage girl. The results of the 2018 Riskesdas showed that the percentage of girls who received blood supplemented tablets in Indonesia was 76.2%, while the percentage of teenage girl who received blood added tablets at school was 80.9%. Of the 80.9% who received blood added tablets at school, 98.6% consumed blood added tablets <52 tablets and only 1.4% consumed blood added

tablets ≥ 52 tablets. This shows that the level of adherence to taking blood-supplemented tablets is still low, because only 1.4% consume blood-added tablets ≥ 52 tablets. The percentage of teenage girl in Central Java Province aged 12-18 years who received blood added tablets was 56.52% lower than the percentage who received blood added tablets for pregnant women, namely 92.29% (Badan penelitian dan Pengembangan Kesehatan Kementerian Kesehatan RI, 2018).

Research of (Nuradhiani, Briawan, &

Dwiriani, 2017) states that the average compliance in consuming blood-added tablets is 10.7%. This is also supported by research (Hurfiati, Nat, Kandarina, & Helmyati, 2018) that the coverage of blood-supplemented tablets in 2017 is quite high, namely at 98.8%, but the adherence rate of teenage girl in consuming blood-added tablets is still low at 20 %.

Compliance will increase if supplementation is accompanied by direct observation when taking blood supplemented tablets, counseling for teenage girl, teacher monitoring, parental support and peer group support (Apriani &

Syafiq, 2018). Peer group method health education will have a more positive effect.

Information that is sensitive and uncomfortable when conveyed by adults can be conveyed by peers using age-appropriate language. Thus, information is more complete, easy to understand and ultimately the goal can be achieved. In addition, as peer educators, peers not only provide information but also act as role models in healthy behavior (Rofi’ah, Widatiningsih, & Vitaningrum, 2017).

Based on this, researchers are interested in conducting research on “Peer Group Support in Prevention of Teenage Girl’s Anemia”.

2. Method

This type of research is pre-experimental with posttest only with control group design. The study population was all 114 female students at MTS Maarif 2 Muntilan with a sample of 64 students consisting of 32 students as the intervention sample and 32 female students as the control sample. The sampling technique used purposive sampling based on inclusion and exclusion criteria. The inclusion criteria in this study were female students in good health and not suffering from complications during the study such as thalassemia, malaria, tuberculosis and HIV, willing to become respondents and able

to consume blood supplement tablets and There are no contraindications to blood supplementation such as hypersensitivity, hemochromatosis, hemolytic anemia, hemosiderosis, active peptic ulcer, regional enteritis and ulcerative colitis. The exclusion criteria in this study were students were not at the location when the study took place. The research data analysis was carried out by means of univariate analysis with frequency distribution, bivariate analysis was carried out by using the Mann Whitney test (Dahlan, 2019).

3. Result and Discussion

The results of the analysis in this study showed the frequency distribution of the compliance value of blood-added tablet consumption in teenage girl with the highest peer group support intervention of 100%, namely 18 (56.3%) people and the lowest was 25%

adherence of 1 person (3,1 %). The average value of adherence to the peer group support intervention group was 85.16 and the standard deviation was 19.939. The results showed that the mean value of adherence was higher in the group with peer group support intervention than in the control group without peer group support intervention. These results indicate that adherence to consumption of teenage girl in the peer group support intervention group shows significant results.

This study is in line with research (Kavle &

Landry, 2017) which states that adherence is defined as adhering to a weekly regimen.

Compliance with the consumption of blood-added tablets among women in three regions in Cambodia was 55% for female students, 57% for female factory workers and 71% for women in rural areas with peer education interventions. Based on these results, it was proven that the most effective in increasing adherence to taking blood supplement tablets was in the peer educator group. Peer-group support was also associated with significant

compliance changes.

Research of (Mesra & Fauziah, 2016) states that the most dominant variable related to teenage sexual behavior is peer. Teenage are very open to peer groups. They hold discussions about romance, philosophy of life, recreation, jewelry, clothing for hours. Peer influence forms a very strong bond. Teenage behavior is much influenced by friends in the group. Peers are friends who are at the same age and between them there is a friendship. The role of friends / peers in teenage is very large in everyday adolescent life.

Peer counselors are very appropriate to support a positive health behavior. This is evidenced by the community service activities carried out by (Riyanti & Legawati, 2018) with the results of peer counselor mentoring activities in the prevention of anemia in teenage girl which can increase knowledge, attitudes and behavior of teenage girl in preventing anemia. This peer counselor is very potential because of the tendency of teenage to choose peers as a place for discussion and information reference. The counseling given by peers is expected to be more trusted, so that teenage are more open to conveying any problems they face. Peers have an important role in the social life and development of teenage. Information on reproductive health obtained through peers can encourage teenage to have better knowledge.

The frequency distribution of adherence to blood-added tablet consumption in teenage girl without peer group support intervention was the highest at 100%, namely 12 people (37.5%) and the lowest was at the 25% adherence level of 8 people (25%). The control group's average compliance value was 64.06 and the standard deviation was 31.068. These results indicate that adherence to blood-supplemented tablet consumption in teenage girl in the group without peer group support intervention is still quite low, because the low level of adherence is still quite

high. The results of the study (Widiastuti &

Rusmini, 2019) state that there are still many cases of anemia in students who are taking the blood supplement tablet consumption program.

This could be related to the lack of awareness in consuming blood supplement tablets.

The behavior of teenage in consuming iron tablets is influenced by several factors. The results of the study (Apriani & Syafiq, 2018) state that teenage adherence to iron tablet consumption is still low due to lack of education, nutrition, low quality tablets, no supply of drinking water for tablet consumption in schools and lack of teacher support in monitoring. In addition, decreased compliance is also caused by side effects that are felt from iron tablet supplements such as nausea, vomiting, stomach pain, black stools and other reasons such as not getting support or permission from parents and forgetting to take iron tablets. Research of (Susanti, Briawan, & Martianto, 2016) states that the reasons for supplementing teenage girl in Tasikmalaya Regency for not consuming Fe tablets are boredom or laziness (29.1%), broken / missing tablets (20.0%) and forgetfulness (19, 4%).

The most common complaints submitted by the subjects after taking supplements were dizziness (38.8%), nausea (34.3%) and easy drowsiness (21.1%).

The results of statistical analysis showed a significance value of 0.007 (<0.05), which means that there was a significant difference in adherence to taking blood-added tablets in an effort to prevent anemia in the peer group support intervention group and the group without peer group support intervention. In other words, peer group support is effective in increasing adherence to blood supplement tablet consumption in teenage girl. Research of (Shah et al., 2016) shows that weekly blood supplementation under the supervision of trained peer educators can reduce the severity of anemia at the youth community level and increase adherence to the consumption of blood-supplemented tablets. In this study, the results obtained after peer educator intervention the prevalence of anemia in tribal areas in India

decreased by 21.5% in teenage girls and 25% in boys.

The incidence of iron deficiency anemia in teenage girl, especially in urban areas, is related to protein intake and iron intake (Sari, Dardjito,

& Anandari, 2016). Research of (Jaelani, Simanjuntyak, & Yuliantini, 2017) also states that one of the factors associated with anemia is iron intake. Adherence to teenage girl in consuming iron is influenced by environmental support. In this case family and peers. Research of (Risva, Suyatno, & Rahfiludin, 2016) shows a relationship between environmental support and the habit of consuming blood-added tablets with a value of p = 0.003 (p <0.05) and OR = 3.133 which means that respondents with good environmental support have the possibility of consuming tablets. blood added 3.1 times greater than respondents with poor environmental support.

Research of (Amsari & Nurhadianti, 2020) states that there is a significant positive relationship with a positive direction between peer social support and the compliance of students in implementing discipline at the Islamic Modern Boarding School. Peer social support is a factor that plays an important role in increasing compliance, the higher the peer social support obtained by the students, the higher the students' compliance with the rules of using billingual language. According to this study, the social support factor in the form of emotional support from family members or Peer Group Support is an important factor in implementing discipline in Islamic boarding schools. Thus Peer Group Support is very important in forming a positive behavior in teenage, including the consumption of blood-supplemented tablets in an effort to prevent iron deficiency anemia in teenage girl.

4. Conclusion and Sugestion

Adherence to the consumption of blood-added tablets in an effort to prevent anemia in teenage girl in the peer group support intervention group showed that most respondents had good adherence, only 1 respondent with low adherence. In the group without peer group support intervention showed that most of the respondents had good adherence, but respondents with low adherence were still 25%. There are differences in compliance with the consumption of blood-added tablets in an effort to prevent anemia in teenage girl in the group

without peer group support intervention and in the peer group support intervention group.

Teenage girl are advised to comply with the supplementation program for blood supplementation according to the rules, with the hope that no more teenage girl will experience anemia, besides choosing positive peer group support by joining peer educators, peer counselors and the Center for Information and Youth Counseling in schools and communities.

Health Institutions can collaborate with schools to support the supplementation program for blood-supplemented tablets by involving the role of peer group support to educate, monitor and support their peers to increase compliance with the consumption of blood-added tablets in teenage girl
