Description of the University of California, San Diego Healer Education Assessment and Referral (HEAR) Program Screening Tool
The screening tool includes a Stress and Depression Screening Questionnaire from the American Foundation for Suicide Prevention’s web-based Interactive Screening Program (ISP).
Potential respondents are informed that: (1) the purpose of the ISP is to provide a confidential
service to those who may or may not be experiencing burnout, depression, and other problems
that can interfere with personal and professional functioning; (2) the brief questionnaire requires
less than five minutes and is hosted on a secure website; (3) their identity will be fully protected
via encryption and will remain unknown even to members of the HEAR program; and (4)
completing the online questionnaire and participating in the program is voluntary and
confidential.
