For the Internal Report, it is completed with an explanation of the signs, history, physical examination, differential diagnosis, investigation carried out (if any), diagnosis, prognosis and therapy. For the Surgical Report also must report all cases encountered which in its handling require operative and X-Ray or USG. Reports written, including: signal, anamnesa, physical examination, type of operative actions taken, pre-operative therapy and post-operative given (if any). Radiology Reports must also report all cases encountered

(17)

which in their handling require X-Ray or USG. Reports written include: indication, history, physical examination, type of X-ray action or ultrasound performed including the position and focus of the organ, and the therapy given (if any)

d. Individual Task Description :

The group assignments report scripts written in Times New Roman letters; font 12; 1.5 spaces in narration or space 1 if in table form, on A4 size paper. The report script is collected as scheduled to supervisor (1 script)
