Fee Details : HCE has paid Rs.73,050/- (Rs.3,750/- through online challan no. 658696653, Dt.02.07.2019 + Rs. 69,300/- vide DD. No. 298168, Dt.01.07.2019) towards CFE fee under Orange-Haz category on the investment of Rs.1.50 Crores is Rs.7,500/- + CFO fee under Orange-Haz category on the investment of Rs.1.50 Crores is Rs.38,000/- and BMW fee on 50 beds is Rs.
27,500/- for a period of 10 years i.e., up to 28.02.2029 as the facility is commissioned in the month of March 2019 and the issuance of CFO, HWM & BMW may be examined up to 28.02.2029.

1. This is a 50 bedded hospital located at D.No 2-269, Society Colony, Madanapalle, Chittoor District, Andhra Pradesh.
2. The HCE has applied for BMW, CFO & HWM order for 50 beds and paid fee of amount Rs.73,050/- (Rs.3,750/- through online challan no. 658696653, Dt.02.07.2019 + Rs. 69,300/- vide DD. No. 298168, Dt.01.07.2019) towards CFE fee under Orange-Haz category on the investment of Rs.1.50 Crores is Rs.7,500/- + CFO fee under Orange-Haz category on the investment of Rs.1.50 Crores is Rs.38,000/- and BMW fee on 50 beds is Rs. 27,500/- for a period of 10 years i.e., up to 28.02.2029 as the facility is commissioned in the month of March 2019 and the issuance of CFO, HWM & BMW may be examined up to 28.02.2029.
3. This HCE is operating X-Ray-1, operation theatre-2, Casuality-1, waiting launge – 3 Nos, Lab-1, General wards-2, Special rooms-10, ICU-1. Labour Rooms – 3 Nos, Miner OT -1 No, Sterilization room – 1 No.
4. The HCE has installed 1 x 125 KVA DG set as a standby of AP Transco supply.
5. The HCE has not constructed STP in their premises
Hence the issuance of CFO (Fresh) may be considered up to 28.02.2029 duly stipulating necessary conditions

The time allotted (SLA) 23.07.2019 Hence, the CFO was issued to the industry on 10.07.2019 with a validity period upto 28.02.2029.
The issue is placed before the CFO Committee for ratification.
